Caseload management in community children's nursing.

AIM: Little is known about the working practices of community children's nurses and how they manage the complexities of working with children and young people with life-limiting, life-threatening and chronic ill-health conditions and their families. This action research project aimed to find ways of managing community children's nursing caseloads that would improve the efficiency, effectiveness and equity of services and inform negotiations with service commissioners. METHODS: A data collection template comprising six input categories was adapted from the Cornwall Community Trust's health vis

To ECV or not to ECV? The current evidence base concerning external cephalic version.

External cephalic version (ECV) is the technique of attempting to turn a baby in the womb from a head-up to a head-down position. The practice is grounded on evidence that vaginal breech birth (VBB) presents greater short-term risks for babies than caesarean section (CS) (Hofmeyr et al 2011), but that labour and vaginal birth also offer benefits to both mothers and babies. Therefore, if we can turn babies to a head-down position, we can reduce the risks associated with both VBB and CS, and enable mother and baby to benefit from labour and birth.

Narratives of care providers concerning picking behavior among institutionalized dementia sufferers.

To illuminate the meaning of picking behavior in institutionalized people with dementia, the narratives of 15 care providers were interpreted using a method inspired by Ricoeur's phenomenologic hermeneutics. The care providers saw the behavior as a form of meaningful communication. The behavior was described as occurring in combination with wandering and in connection with restlessness and stress related to fatigue, difficulty communicating, and lack of occupation. The behavior also was seen as an attempt to engage in meaningful activities. Some care providers reported that the picking behavio

Look before you leap: lessons learned when introducing clinical simulation.

Before investing in a human patient simulator, we designed a preliminary study that examined student responses to a laboratory exercise that used lower-fidelity simulation. Our purpose was to compare beginning-level, baccalaureate nursing students' self-reported assessment in the domains of confidence, ability, stress, and critical thinking before and after they participated in the simulation. Results showed statistically significant improvement in all domains for skills in urinary catheterization, intravenous and nasogastric medication administration, and sterile dressing change.

Induced hypothermia for neonates with hypoxic-ischemic encephalopathy.

Hypoxic-ischemic encephalopathy causes significant morbidity and mortality in neonates. Preventing the secondary reperfusion injury that occurs following a hypoxic-ischemic event is paramount to ensuring the best possible neurologic outcome for the neonate. Induced hypothermia is currently being studied in various institutions as a means of neuroprotection for neonates at risk of severe brain injury following a hypoxic-ischemic event. This article highlights the pathophysiology of hypoxic-ischemic encephalopathy and the rationale behind the effectiveness of induced hypothermia. Nursing care an

Using the connecting process to meet family caregiver needs.

Individuals who engage in caregiving responsibilities for dependent family members in the home setting face challenges that are little understood by others outside the circle of care. Because a large portion of energy is devoted to caregiving tasks, caregivers potentially fail to devote attention to meeting their own needs. The purpose of this study was to develop a beginning substantive theory describing the needs perceived by caregivers and the processes by which these caregivers are able to get their individual needs met. Grounded theory methodology was used to facilitate the identification

Social support in the workplace: nurse leader implications.

The state of the science as it relates to the stress-mediating role of social support in the health care work environment is explored. The ability to build a nursing leadership workforce is dependent upon a type of social support mechanism that mediates the inherent stress that comes along with managerial positions. Independent of the inevitable work-related stress associated with the nurse manager role, social support provided to current and aspiring nurse leaders is assumed to assist with coping and thriving in the work environment.

Experiences of psychiatric nurses exposed to hostility from patients in a forensic ward.

BACKGROUND: Hostile behaviour is becoming a way of life in South Africa. Hostility prevails at all settings, including in the health sector. In a forensic ward psychiatric nurses are subjected to hostile behaviour by the patients. AIM: The aim of the present study was to explore and describe the psychiatric nurses' experiences of hostile behaviour by patients in a forensic ward and make recommendations for nurse managers to empower these psychiatric nurses to cope with the patients' aggression. METHOD: Qualitative, in-depth, phenomenological interviews were conducted with nine psychiatric nu

Mothering a 3- to 6-year-old child with hemiparesis.

Stroke is estimated to occur in approximately 2.5-3.2 children per 100,000 annually. While some aspects of mothering a disabled child are currently known, there are other aspects of this experience that remain unknown. Much of the research focuses on children who are severely disabled, thus leaving a gap in what is known about the experience of mothers of mildly to moderately disabled children. Little is understood about mothering children who have a less severe disability, and in particular, a 3- to 6-year old child with hemiparesis. Therefore, the purpose of this study was to describe the ex

Ritualized practices among caregivers at meals in geriatric care.

The aim of this study was to examine the procedures of the caregivers in their daily work with meals in geriatric care, emphasizing ritualized practices, praxis and norms. An ethnographic approach was used. Registered nurses, enrolled nurses and nurse's assistants observed in the study responded to a questionnaire and, finally, were interviewed. The findings indicated that the meals mainly took the form of ritualized practices, and praxis depended on the ability and needs of the patients as well as on the working conditions. The guiding norms identified were 'to provide a home-like situation w

To push ... or not to push?

I was formally asked by my supervisor of midwives to reflect upon two episodes of care. Both of these involved women who would be termed as 'low risk' primigravida and in both instances the women had considered whether, had they been advised against pushing when it was 'too soon', they might have avoided caesarean sections. To help this process I used my own reflection triangle: 'What happened ... Was it OK ... What about next time?' informed by the Gibbs model (1998). In this paper I present my reflections alongside my reflection triangle.

Burnout in renal care professionals.

Burnout is defined as "a syndrome of emotional exhaustion, depersonalisation and reduced personal accomplishment that can occur among individuals who work with people in some capacity", and it can be considered as a result of long-term exposure to occupational stress. Frequently reported occupational stressors among caring professionals are those intrinsic to the job, related to patient demands, related to roles within the organisation, and those related to relationships at work and career development. In renal care however, there are some unique characteristics such as technologically advance

Managing stomatitis in patients treated with Mammalian target of rapamycin inhibitors.

Mammalian target of rapamycin (mTOR) inhibitors are a class of targeted cancer therapeutic agents with clinical benefit for multiple tumor types. Oral ulcerations are a common side effect of mTOR inhibitors; however, the clinical findings resemble aphthous stomatitis rather than the mucositis seen with chemotherapy. Consequently, the appearance of aphthous-like oral ulcerations has been referred to as mTOR inhibitor-associated stomatitis (mIAS). The severity of mIAS can be minimized by following common preventive steps and initiating treatment at the first sign of mouth discomfort, thereby red

Mad talk: attending to the language of distress.

This paper will examine how the narrative of one woman, Madeleine, can be constructed as symptomatic of the diagnosis of schizophrenia, and how it can also be read from other perspectives, particularly a poststructural feminist one. The readings are presented as possibilities for understanding the woman's experiences and the implications of this for mental health nursing practice. A poststructural feminist reading acknowledges the gendered experiences of subjectivity and how those experiences are constructed in language. The purpose of this paper is to identify for mental health nursing practi
