A patient observation chart was developed to combine physiological observation charting with the calculation and recording of an early warning score (EWS); this was named the Physiological Observation Track and Trigger System (POTTS). Reference to a table of instructions is no longer needed and the EWS can be calculated with minimal error. Increased frequency and accuracy of EWS recording assist in identifying patients at risk of deterioration and enhance patient safety.

The aim of the study was to evaluate the occurrence of clinical and biochemical symptoms of osteoporosis in 15 children aged from 10 to 18 years. A naturalistic observation study of adolescent tobacco use was conducted to corroborate previous studies which used self-report questionnaire or structured interview methods to study this problem behavior. Four siblings with hepatic fibrosis are described. The liver damage in these patients was secondary to chronic ingestion of massive doses of vitamin A for congenital ichthyosis. Although the extrahepatic manifestations were helpful in the diagnosis of hypervitaminosis A, the distinctive features of hepatic histopathology were confirmatory. A clinical observation was made on 30 cases of urogenital tuberculosis diagnosed and treated at the urological department of our Hospital between January, 1976 and December, 1984. Furthermore, 20 of them were examined for drug resistance and investigated for this tendency. They accounted for 0.23% of the outpatients. Sixty-one male urethritis cases, 28 gonococcal urethritis and 33 nongonococcal urethritis (NGU), were out-patients at the Department of Urology, Asahi General Hospital, during the 4 months, Oct. 1, 1984-Jan. 31, 1985.
