Immunohistochemical expression of SPARC and MMP-9 in odontogenic keratocysts and orthokeratinized odontogenic cysts.

OBJECTIVE: Odontogenic keratocysts (OKCs) are locally aggressive odontogenic cysts with high recurrence propensity, whereas orthokeratinized odontogenic cysts (OOCs) are indolent. Secreted protein acidic and rich in cysteine (SPARC) and matrix metalloproteinase 9 (MMP-9) have been implicated in tumor invasion. This study aimed to evaluate and compare the immunohistochemical expression of SPARC and MMP-9 in sporadic OKCs and OOCs, and to further explore the potential association between these two proteins in both types of cysts. DESIGN: SPARC and MMP-9 expression were evaluated in 24 sporadic …

A case of orthokeratinized odontogenic cyst suspected to be a radicular cyst.

This report describes a case of orthokeratinized odontogenic cyst arising in the mandibular molar region of a 39-year-old man. Under the initial clinical diagnosis of radicular cyst, root canal treatment was performed on the mandibular right second molar. The treatment that continued for six months did not achieve healing. Subsequently surgical intervention was selected since the tooth fracture was found, and the prognosis was judged to be poor. After atraumatic tooth extraction, the apical cystic lesion was enucleated, and the tooth was replanted. A definite diagnosis of orthokeratinized odon

Immunoexpression of CXCL12 and CXCR4 in Radicular Cysts, Dentigerous Cysts, and Odontogenic Keratocysts.

The aim of this study was to evaluate the immunoexpression of chemokine CXCL12 and its receptor CXCR4 in radicular cysts (RCs), dentigerous … hogenesis of RCs, DCs, and OKCs. These proteins may be particularly relevant for the development of odontogenic cysts with less aggressive biological behavior, irrespective of their nature (inflammatory or developmental). In RCs, the expression of CXCL12 and CXCR4 may not be related to the intensity of the inflammatory infiltrate or the status of cystic epithelium.

Evaluation of mast cells in periapical cysts, dentigerous cysts, and keratocystic odontogenic tumors.

BACKGROUND: Several cell types are associated with the development of cystic and tumoral odontogenic lesions. Among inflammatory cells, mast cells can be associated with their pathogenesis. The aim of this study was to analyze mast cells in periapical cysts, dentigerous cysts, and keratocystic odontogenic tumors. METHODS: Tissue sections were submitted to toluidine blue staining and immunohistochemistry with antibody anti-tryptase (clone G3). Mast cells were quantitated using Image-Pro Plus software to obtain the mean number of mast cells in three regions: epithelial, superficial portion of t

Ghost cell odontogenic carcinoma arising in the background of a calcifying odontogenic cyst.

Ghost cell odontogenic carcinoma (GCOC) is a rare malignant neoplasm, representing 3% of all ghost cell lesions of the jaws. They can arise de novo or from a pre-existing calcifying odontogenic cyst (COC) or dentinogenic ghost cell tumour (DGCT). A systematic review of the literature reported only 12 cases of a GCOC arising from a pre-existing COC. This report highlights an additional case of a GCOC arising from a pre-existing COC after 3 years in an adolescent male. The patient initially presented with a painless swelling of the right mandibular corpus. Panoramic radiographic examination show

Calcifying odontogenic cyst with odontogenic keratocyst: a case report and review of the literature.

The calcifying odontogenic cyst (COC), first identified as a separate and distinct lesion by Gorlin et al. in 1962, is an uncommon benign lesion, consisting of a proliferation of odontogenic epithelium and scattered nests of ghost cells and calcifications that may form the lining of a cyst or present as a solid mass. The COC occurs alone or occasionally with odontomas or other odontogenic tumors, and it is this variable histology and clinical behavior that has raised the question of whether or not it is a cyst or a true neoplasm. The odontogenic keratocyst (OKC) is a locally aggressive odontog

Magnetic resonance imaging: a useful tool to distinguish between keratocystic odontogenic tumours and odontogenic cysts.

In contrast to odontogenic cysts, keratocystic odontogenic tumours often recur and require more aggressive surgical treatment, so we tried to find features that distinguished between them on magnetic resonance imaging (MRI). Without knowing the diagnosis, two radiologists reviewed intensity (low, intermediate, or high) and homogeneity (homogeneous or heterogeneous) of signals in short-tau-inversion-recovery (STIR), T1- and T2-weighted, and fat-suppressed, contrast-enhanced MRI in 20 consecutive patients with oval, radiolucent lesions of the mandible on panoramic radiography, and who were subse…

Differentiation of odontogenic keratocysts from other odontogenic cysts by the expression of bcl-2 immunoreactivity.

Odontogenic keratocysts (OKC) present an aggressive course with a marked tendency to recurrence. The epithelium of OKC is thought to have an intrinsic growth potential and has been shown to present a higher rate of proliferation as compared to other types of cyst. bcl-2 has a role in the extension of cell survival. The objective of the present study was to evaluate the bcl-2 protein expression in different odontogenic cysts. A total of 19 dentigerous cysts (DC), 20 radicular cysts (RC) and 14 OKC were used in the present study. DC and RC showed an almost complete negativity for bcl-2. OKC, on …

Glandular Odontogenic Cyst-A Case Series.

The glandular odontogenic cyst (GOC) is an uncommon jawbone cyst with a challenging diagnosis because of the presence of several clinical and histopathologic overlaps with other odontogenic lesions and central mucoepidermoid carcinoma. To date, less than 200 cases of GOC have been published in the literature with reliable clinical and histopathologic information. Furthermore, the lack of a well-documented GOC case series impairs a more detailed understanding about the biological behavior and appropriate management of the lesion. Therefore, in this study, we report 3 well-documented cases of GO

Nasolabial Cyst Associated with Odontogenic Infection.

The nasolabial cyst or Klestadt cyst is a relatively uncommon nonodontogenic cyst that develops in the nasal alar region; it has uncertain pathogenesis. This lesion has slow growth and variable dimensions and is characterized clinically by a floating tumefaction in the nasolabial fold area around the bridge of the nose, causing an elevation of the upper lip and relative facial asymmetry. Diagnosis is primarily made clinically; if necessary, this is complemented by imaging. This paper reports the case of a 39-year-old male patient who complained of pain in the right upper premolar region and po

Periapical radiolucency mimicking an odontogenic cyst.

AIM: To present a clinical case of a giant cell lesion located in an unusual site, initially misdiagnosed and treated as an odontogenic cyst. SUMMARY: Periapical radiolucencies often suggest the presence of odontogenic pathosis, usually inflammatory granulomas or cysts. The high frequency of such lesions tends to lead clinicians to arrive at a diagnosis without completing a comprehensive assessment of the patient or carrying out the full range of available diagnostic tests. A case report of a giant cell lesion, which was misdiagnosed and treated initially as an odontogenic lesion because of i

Radiological features of glandular odontogenic cyst.

OBJECTIVES: To present five new cases of glandular odontogenic cyst (GOC) and to review the radiological features at presentation as reported in the English literature. METHODS: From 1993 to 2002, five patients in our department were diagnosed with GOC based on histopathological findings and supported by radiography and CT. The radiographic features of the new GOC cases were analysed in addition to 51 literature cases. RESULTS: There were 31 male and 25 female patients, aged 14-90 years (mean 50 years). The mandible was involved in 80% and the maxilla in 20%; most of the lesions were located

[The nature of calcifying odontogenic cysts of the jaws].

OBJECTIVE: To clarify the clinicopathological and behavioral spectrum of the so-called calcifying odontogenic cysts (COC). METHODS: Retrospective analysis of 21 cases previously diagnosed as COC was undertaken to evaluate their clinical, radiographic, pathological and behavioral features. RESULTS: The lesions of this series were divided into three groups, including simple cysts, benign and malignant tumors. The cyst group was composed of 16 cases (9 men, 7 women). The age of the patients peaked at the second decade, with a predilection for the premolar region of the jaws. The clinicopatholog…

Glandular odontogenic cyst: a case report.

Glandular odontogenic cyst (GOC) is an extremely rare lesion occurring in the jawbones. Sialo-odontogenic cyst was first described as a multicystic lesion resembling a botryoid odontogenic cyst (BOC) or a central mucoepidermoid carcinoma by Padayachee in 1987. In 1988, Gardner used the term "glandular odontogenic cyst" and considered it as a histologic variant of BOC. Most authors agreed that GOC was odontogenic because of the concurrent ball-like epithelial structure, ameloblastoma, squamous odontogenic tumor-like proliferation in its wall, or hyaline bodies in the epithelium lining. Recently

[Calcifying odontogenic cyst. A clinical case].

The calcifying odontogenic cyst (COC) is a rare lesion that involves the jaws which, at times, may evolve into a solid, benign, tumor-like mass. There is no significant sex predilection, but it is probably more common in later life. The diagnosis is base only on histologic examination that represents, in the best of cases, the only way to differentiate the COC from other pathologies. Such lesions include: odontogenic cyst, residual cyst, globulomaxillary cyst, lateral periodontal cyst, ameloblastoma (multicystic and unicystic), odontogenic mixoma, and still with calcifying odontogenic epitheli
