Optic chiasm involvement on MRI with ethambutol-induced bitemporal hemianopia.

While ethambutol optic neuropathy usually causes central or cecocentral scotomas, bitemporal visual field defects also have been reported. The pathogenesis of the bitemporal hemianopia has not been established. This article describes magnetic resonance imaging abnormalities involving the optic chiasm in a patient with bitemporal visual field loss. To our knowledge, these neuroimaging findings have not been previously described in association with ethambutol therapy.

Measurement of the normal optic chiasm on coronal MR images.

PURPOSE: To develop an objective method for measuring the optic chiasm and to document its normal range in size. METHODS: Measurements of the height and area of the optic chiasm, made on coronal T1-weighted MR images with the use of commercially available region-of-interest software, were obtained in 114 healthy subjects who had a total of 123 MR studies. A normal range and standard deviation were calculated, and the information was broken down by age and sex. RESULTS: The mean area of the optic chiasm was 43.7 mm2, with a standard deviation of 5.21. The mean width was 14.0 mm, with a standa…

Optic chiasm astrocytomas of childhood. 1. Long-term follow-up.

We reviewed 61 patients seen from 1975 to 1994 with a biopsy-confirmed optic chiasm astrocytoma or a characteristic clinical and imaging presentation. The mean age at diagnosis was 72 months, including 30 who were less than 5 years old. The mean tumor diameter at presentation was 31 mm among 54 tumors measured. Tumors involved the optic nerve in 41, hypothalamus in 38, optic tract in 15, and optic radiations in 2. Four patients, all with neurofibromatosis, received no treatment. Forty patients received conventional radiation therapy at a mean age of 89 months. Chemotherapy was given to 19 chil…

A glial cell derived pathway directs regenerating optic nerve axons toward the optic chiasm.

After optic nerve injury, several retinal ganglion cell (RGC) intrinsic signaling pathways have been shown to enhance RGC survival and RGC axonal growth. In contrast, few extrinsic cues have been identified that guide regenerating RGC axons toward and across the optic chiasm. Here, we use live-cell imaging in larval zebrafish to show that regrowing RGC axons initiate growth toward the midline and extend along a trajectory similar to their original projection. From a candidate genetic screen, we identify the glycosyltransferase Lh3 (also referred to as Plod3) to be required to direct regrowing …

Architecture of the optic chiasm and the mechanisms that sculpt its development.

At the optic chiasm the two optic nerves fuse, and fibers from each eye cross the midline or turn back and remain uncrossed. Having adopted their pathways the fibers separate to form the two optic tracts. Research into the architecture and development of the chiasm has become an area of increasing interest. Many of its mature features are complex and vary between different animal types. It is probable that numerous factors sculpt its development. The separate ganglion cell classes cross the midline at different locations along the length of the chiasm, reflecting their distinct periods of prod…

Ganglioglioma of the optic chiasm: case report and review of the literature.

We report a case of a hypothalamic chiasmatic ganglioglioma in a 21-year-old woman who presented with hyperprolactinemia and developed visual field defects. This circumscribed cystic lesion with an enhancing mural nodule was radiologically indistinguishable from a pilocytic astrocytoma. Although rare, gangliogliomas should be included in the differential diagnosis of lesions occurring in this area of the brain.

The early development of the optic nerve and chiasm in embryonic rat.

To establish the time course and major features of the development of the optic nerve and chiasm in the embryonic rat, the growth of axons from the retina to the brain has been studied by light and electron microscopy. On embryonic day 14 (E14), the first axons are generated by retinal ganglion cells. Fascicles of axons can be detected in the optic stalk at E14.5 and in the diencephalon by E15.0. In the vitreal retina and optic fissure, large extracellular spaces resemble the oriented channels previously described in the mouse. They form approximately 12 hours before the invasion of optic axon

Xanthomatous hypophysitis with optic chiasm edema: First documented clinical case.

Xanthomatous hypophysitis is an exceedingly rare subtype of hypophysitis that can closely mimic a cystic pituitary adenoma. We report the ca … ary macroadenoma. Notably, there was an unusual nonenhancing T2-hyperintense signal in the adjacent optic chiasm, suggestive of edema despite the absence of direct chiasm compression and absence of the posterior pituitary bright spot. Endoscopic endonasal resection demonstrated firm yellow tissue, and histopathology confirmed xanthomatous hypophysitis characte …

Cavernous malformations of the optic nerve and chiasm: illustrative case.

BACKGROUND: Cavernous malformations (CMs) of the optic pathway are rare. In this report, the authors describe a case of a CM affecting bilateral optic nerves that was managed surgically. Additionally, they provide an updated review of the literature focused on the clinical presentation, management, and postoperative outcomes related to this uncommon pathology. OBSERVATIONS: A total of 99 cases have been described in the literature, including the case reported. The patient presented with headache, encephalopathy, and bitemporal hemianopia. He was found to have an optic pathway CM and underwent

Assessment of fetal optic chiasm: an echoanatomic and reproducibility study.

OBJECTIVES: Our aims were: (1) to perform an echoanatomic correlation study, in order to confirm that the structure identified as the optic chiasm (OC) on ultrasound (US) is indeed this anatomical structure; (2) to assess and compare the reproducibility of two- (2D) and three-(3D) dimensional US in measurement of the OC in normal fetuses; and (3) to assess whether the spatial orientation of the OC changes with increasing gestational age. METHODS: For the echoanatomic study, the OC was studied in a neonatal specimen, deceased at 29 + 4 weeks, by passing a suture around the OC and visualizing t…

Uncommon cavernous malformation of the optic chiasm: a case report.

Cavernous malformation (CM) is a vascular malformation disorder characterized by a berry-like mass of expanded blood vessels. CM, originating from the optic chiasm. usually leads to chiasma syndrome presenting with bitemporal hemianopsia. We report a 28-year-old male presenting with left homonymous hemianopsia. Magnetic resonance imaging (MRI) revealed an occupied lesion located in the right side of the optic chiasm, and a clinical diagnosis of chiasmal CM was made. Microsurgical excision was performed via anterolateral pterional craniotomy. The patient showed good recovery with slight improve…

A patient with progressive headache and partial optic chiasm involvement.

A 69-year-old woman presented with a 2-month history of bilateral throbbing daily headaches and a 2-week history of progressive visual loss. Radiologic imaging, visual field test, and cerebrospinal fluid analysis findings are discussed. After treatment, the symptoms were resolved without deficit.

Bagolini striated glasses test and lesions of the optic chiasm.

PURPOSE: To examine the usefulness of the Bagolini Striated Glasses Test to identify patients with a lesion of the optic chiasm. SUBJECTS AND METHODS: Twelve patients who were diagnosed with a lesion of the optic chiasm by MRI were examined with the Bagolini Striated Glasses Test. Discrepancies of the test between monocular and binocular conditions were analyzed. Visual acuity, visual field and stereo tests were also administered and assessed. RESULTS: Nine out of 12 cases (75%) showed conflicting results with the Bagolini Striated Glasses Test between monocular and binocular conditions. Wit…
