Three grades of premaxilla deformation were revealed in children with bilateral cleft lip and palate. All patients with grade I and some children with grade II deformation received early orthopedic treatment by Sharova appliance while in patients with grade III deformation orthodontic devices with bone fixation with microimplants were used or microimplants alone for orthodontic anchorage. This approach allowed achieving full side contact of premaxilla and maxillary fragments in 1-2 months thus creating favorable conditions for surgical procedure.

BACKGROUND: Surgical fixation is the preferred method of treatment for the ruptured distal biceps tendon in active patients. To date, no fixation technique has been proven superior in a clinical setting. The purpose of the study was to systematically review the available literature on approach and fixation methods for distal biceps tendon repair in a clinical setting and to determine the optimal fixation methods of the distal biceps tendon on the radial tuberosity. Our hypothesis was that the outcomes would not be significantly different among the various fixation techniques and approaches.

OBJECTIVE: Pectus carinatum is traditionally repaired by using some modification of the open Ravitch procedure, with its possible long-term sequelae, such as poor postoperative compliance of the chest. In this study we assessed our results with a new minimally invasive repair of pectus carinatum that requires neither cartilage incision nor sternotomy.
METHODS: From June 2005, we have corrected pectus carinatum using a method analogous to the Nuss procedure for pectus excavatum repair. Thus far, we performed this intervention on 14 patients (mean age, 15 +/- 1.5 years). A steel bar has been in

Bioabsorbable implants are playing an increasing role in the surgical management of foot and ankle pathologies. Current technology allows implants to have acceptably comparable strength and pull-out characteristics to metallic implants. The advantages include elimination of secondary surgeries, biodegradability of implants placed across mobile articular surfaces, as well as acceptable biocompatibility and resorption properties to limit historical complication concerns.

Talus fractures are rare injuries that account for approximately 3% of all foot fractures; talar neck fractures account for almost 50% of all talus fractures. The appropriate diagnosis and treatment of these fractures play an important role in the patient's outcome. Treatment has evolved slowly throughout the years, from closed treatment to open reduction and internal fixation (ORIF). The treatment of type I and type II talar neck fractures is not uniform in the orthopedic community. The option of closed reduction versus ORIF is dependent upon the degree of injury, surgeon experience, and pref

Comminuted fractures of the base of the first metacarpal are difficult to stabilize with internal fixation or with plaster casts. Use of the external mini-fixator can provide good results. Complications such as pin-infections were not seen after the use of short duration of this apparatus in closed fractures.

Continuous passive motion (CPM) was used to maintain or gain hip and knee range of motion in 18 postoperative or postinjury pediatric orthopedic patients. Continuous passive motion was started in the early postoperative period and augmented with physical therapy. The device was well-tolerated in 16 of the 18 patients. In all but one, motion was improved with a progressive decrease in joint pain. In these patients, CPM was found to be a valuable tool in rehabilitation, and did not interfere with traction, open wounds, nursing care, nor external fixation devices.

Polyostotic Fibrous Dysplasia associated with McCune-Albright Syndrome presents extreme challenges for the treating physician. The bone deformities, frequent fractures and weight bearing bone pain all combine to adversely affect the child's ability to engage in the normal activities of childhood. All too often the family is told that there is no real treatment for the bone disease. As a result of the rare nature of this syndrome, few orthopedic surgeons have had extensive experience with reconstructions for the skeletal deformities. Early surgery to address the upper femoral deformity and the

The pathomechanics and treatments of hallux valgus vary widely by deformity and surgeon, and are extensively described in the orthopedic and podiatric literature. With each newly described treatment, new complications can be encountered. In this report, we describe the case of a 22-year-old woman who underwent bilateral hallux valgus repair with a tension wire and button fixation system, and developed bilateral second metatarsal stress fracture at the point of fixation application during the postoperative period. Although this fixation system has been useful in our practice, like other surgica

INTRODUCTION: Although non-fracture-related syndesmotic injuries of the ankle are relatively rare, they may lead to poor clinical outcome if initially undiagnosed or managed improperly. Despite a variety of literature regarding possibilities for treatment of isolated syndesmotic injuries, little is known about effective applications of different therapeutic methods in day-to-day work. The aim of this study was to assess the current status of the treatment of isolated syndesmotic injuries in Germany.
MATERIALS AND METHODS: An online-questionnaire, capturing the routine diagnostic workup includ

The flexible form of acquired hallux varus has historically been treated with a number of applicable soft tissue procedures, most of which combine medial capsular release with adjunct procedures aimed at restoring the transverse plane balance of the digit. In this report, a surgical technique for correction of hallux varus that employs the use of an implanted button and suture device, namely the Mini TightRope(®), is described. Rather than relying on the patient's own soft tissues, this device applies tension to anchor the hallux in the proper alignment. The device is easily adjusted during th

STUDY DESIGN: An in vitro biomechanical study of posterior lumbar interbody fusion (PLIF) with threaded cages was performed on 18 bovine lumbar functional spinal units.
OBJECTIVES: To compare the segmental stiffness among PLIF with a single long posterolateral cage, PLIF with a single long posterolateral cage and simultaneous facet joint fixation, and PLIF with two posterior cages.
SUMMARY OF BACKGROUND DATA: In most cases, PLIF with threaded cage techniques needs bilateral facetectomy, extensive exposure, and retraction of the cauda equina. Posterior element deficiency is detrimental to pos

Interbody cages in the lumbar spine have met with mixed success in clinical studies. This has led many investigators to supplement cages with posterior instrumentation. The objective of this literature review is to address the mechanics of interbody cage fixation in the lumbar spine with respect to three-dimensional stabilization and the strength of the cage-vertebra interface. The effect of supplementary posterior fixation is reviewed. Only three-dimensional stabilization evaluations in human cadaveric models are included. These studies involve the application of different loads to the spine

To assess the compressive force generated by different techniques of cerclage wiring, the compression produced by 6 different cerclage techniques, including monofilament wires, Dall-Miles cables (Howmedica, Rutherford, NJ), and hose clamps, was measured. Tests were performed on both a polysulfone cylinder model and a cadaver bone model. The hose clamp generated significantly higher compression force than any other cerclage device tested. Doubled wires produced similar compressive force as the cables. As the hose clamp is the least expensive and produces the maximum compression, it may be advan

Varus osteotomy of the distal femur is recommended for osteoarthritis of the knee with significant valgus deformity, but the operation is difficult to plan and perform. A simple technique involving a jig referenced to the line of the tibia is described. This allows accurate overcorrection by a few degrees, with impaction and secure fixation at the osteotomy. It has been successful, with no complications, in 12 consecutive knees.

The ability of several idealized tibial anchorage systems to resist distractive relative motion between the substrate and the component when subjected to eccentric loads was assessed using mechanical tests employing a urethane foam as a bone substitute and finite element models. Eccentric loads had to be placed within 30-35% of the edge of the tibial plate for separation to occur between an unanchored plate and the substrate. The finite element model predicted that a 90%/10% medial/lateral load distribution was required to maintain compressive vertical stresses beneath the unanchored plate. An
