This cross-sectional study assessed the relationship between the degree of optic nerve pallor (optic atrophy) and visual function. Using a set of "gold standard" stereoscopic slides, the severity of optic atrophy for 270 eyes, each having sustained a bout of optic neuropathy, was graded. Good visual acuity was found in 55/86 (64.0%) mild, 54/119 (45.4%) moderate, and 21/65 (32.3%) marked optic atrophy eyes. Bilateral pallor of the optic disks was observed in a 52-year-old man after dissection of an internal carotid artery. Diffuse pallor of the ipsilateral optic disk reflected infarction of the ipsilateral optic nerve and "bow-tie" atrophy of the contralateral optic disk reflected infarction of the ipsilateral optic tract. The findings were due to an occlusion of the internal carotid artery proximal to the origin of the ophthalmic artery, resulting also in insufficiency in the area of supply of the anterior choroidal artery.

Computerized pallor measurements of the optic nervehead were performed by global analysis with the Rodenstock Disk Analyzer. Total intervideographic and intravideographic variability was calculated. By employing purple colour filters with a large blue and a small red transmission, normal and atrophic optic disks were photographed on colour transparencies. Normal optic disks reflect red light, and atrophic optic disks reflect blue light. The degree of atrophy estimated by colour contrast is quantitated by microdensitometry in the blue (470 nm) and in the red (640 nm) region of the transparencies. The difference between the blue and red density is used as an expression of optic disc colour.
