Neutrophilic (lobular) panniculitis (NP) is a very rare condition that belongs to the group of neutrophilic dermatoses. We report the case of a patient with NP and review the relevant literature. NP appears as a subcutaneous nodular eruption. Histology shows a lobular neutrophilic infiltrate. NP must be differentiated from other types of panniculitis, and also from the subcutaneous septal involvement that may occur in some cases of Sweet's syndrome and from erythema nodosum. NP is significantly associated with myelodysplasia. It is highly sensitive to oral steroid therapy.

A 40-year-old woman who had been treated for mesangiocapillary glomerulonephritis by renal transplantation, and who still used furosemide, azathioprine, cyclosporine and prednisone, was admitted with multiple erythematous nodes on the lower legs. Pancreatogenic panniculitis caused by pancreatitis was diagnosed. The patient died with a haemorrhagic and necrotic pancreas one month after admission. Pancreatogenic panniculitis occurs in 2-3% of patients with pancreatitis or pancreatic carcinoma. It can be a first manifestation of an undetected pancreatic disease.

We report three cases of mesenteric panniculitis in which the disease took different courses. The first case clinically mimicked an acute diverticulitis and consequently laparotomy was performed. During this operation a large space-occupying tumour was found in the lower abdomen. After resecting this tumour mass of uncertain classification (benign or malignant) a preternatural anus of sigmoid colon was formed. Histological exploration revealed mesenteric panniculitis. Panniculitis is an inflammatory condition of adipose tissue that is mainly seen subcutaneously or intra-abdominally. There have been rare case reports of retroperitoneal involvement but no reported cases of perinephric panniculitis. To our knowledge we report the first documented case of perinephric panniculitis.

We report the development of acute, tender, erythematous plaques in a 65 year old female with Myelodysplastic Syndrome transforming to Acute Myeloid Leukaemia. The clinical presentation strongly suggested Sweet's syndrome. Histopathological examination of the plaques showed a normal epidermis, dermal and subcutaneous oedema, and large numbers of polymorphs in the panniculus. The eruption responded quickly to systemic steroids, with recrudescence when steroid dosage was reduced. She remained symptom free when prednisolone dosage was reduced more slowly.

In a 76 year old patient computed tomography of the abdomen and laparoscopic biopsy revealed a mesenteric panniculitis as cause of recurrent fever and shaking chills. The fever resolved under corticosteroid therapy and the patient remains well three month after onset of treatment.

We have reviewed the case of a young man with an omental mass diagnosed as mesenteric panniculitis. The clinical history, physical findings, and laboratory and radiologic studies were typical of this disorder. The diagnosis was suggested by CT scan, and confirmed on surgical biopsy. Our patient has had the typically benign course seen with mesenteric panniculitis.

Mesenteric panniculitis is an inflammatory condition of mesenteric fat which may result in peritoneal space occupying lesions. Only two previously reported cases in the English literature have been evaluated by computed tomography (CT). A 60 year old woman developed generalised erythema nodosum-like lesions together with hectic fever, lymphadenopathy, and hepatitis. Biopsies revealed lobular panniculitis with a benign histiocytic infiltrate and prominent phagocytosis in subcutaneous sites, lymph nodes, and bone marrow. A patient is described who developed sore throat, palpable purpura, and a distinctive panniculitis. The pathologic alterations in the subcutis were similar to what Wells described in the dermis as eosinophilic cellulitis. In our experience the presence of such a picture primarily in the subcutis is unique. Streptococcus is implicated as a possible antigenic stimulus for the process in this patient.

The Weber-Christian syndrome (relapsing nodular panniculitis) displays a clinical spectrum varying from short, self-limited, or intermittent disease episodes to persistent disease with fatal outcome.
