The neuropathology of paraneoplastic syndromes.

The term "paraneoplastic neurological syndromes" encompasses a number of uncommon disorders associated with systemic malignancies. In order to be classified a paraneoplastic neurological syndrome, the malignancies must not invade, compress, or metastasize to the nervous system. They can either focally or diffusely involve the central and peripheral nervous system or the neuromuscular junction. This paper reviews the neuropathology of the syndrome. It will first describe the clinical presentation and give an account of the systemic tumors most commonly associated with the various types of disor

A Rare Case of Motor Neuropathy Secondary to Non-Hodgkin Lymphoma.

Neurological syndromes associated with lymphomas are uncommon. These syndromes occur mainly in Hodgkin lymphoma and primarily involve the central nervous system. Neurological manifestations in the peripheral nervous system and motor neuropathies are rare and add an extra layer of diagnostic confusion. We describe the case of a 70-year-old man with a history of right foot drop, who arrived at the emergency department after sustaining polytrauma. During initial evaluation, imaging studies incidentally revealed a retroperitoneal mass, which further studies diagnosed as an aggressive B-cell lympho

Paraneoplastic Diseases of the Nervous System.

Several neurologic paraneoplastic disorders are believed to be caused by an autoimmune reaction against an antigen or antigens coexpressed by tumor cells and neurons. Of the paraneoplastic syndromes, the Lambert-Eaton myasthenic syndrome (LEMS)--in which autoantibodies downregulate voltage-gated calcium channels at the presynaptic nerve terminal--is associated with the strongest evidence of an autoimmune cause. For the other syndromes, including cerebellar degeneration, multifocal encephalomyelitis, sensory neuronopathy, limbic encephalitis, opsoclonus-myoclonus, and retinal degeneration, an a

Unusual paraneoplastic neurological presentation associated with multiple myeloma.

Various neurological complications can occur in multiple myeloma. We report a unique paraneoplastic neurological presentation that led to a multiple myeloma diagnosis. A man in his mid-40s experienced a 6-month progressive decline in vision and gait. Examination demonstrated an upper motor neuron pattern, while MRI showed bilateral corticospinal tractopathy and optic neuropathy. Workup led to a diagnosis of multiple myeloma, while other causes (eg, primary central nervous system demyelination, toxic-metabolic, degenerative) were ruled out. Paraneoplastic antibody panels were negative. There wa

Paraneoplastic neuromyelitis optica and ovarian teratoma: A case series.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare inflammatory disease of the central nervous system, characterized by the presence of auto-antibodies directed against aquaporin-4 (AQP4) expressed on astrocyte end-feet. Despite NMOSD does not primarily belong to the spectrum of paraneoplastic neurological syndromes, rare cases of association with neoplasia have been outlined. Here, we report the association of NMOSD with ovarian teratoma in 3 cases. Pathological analysis of teratomas revealed glial component strongly expressing AQP4 and closely localized to immune infiltrates. Our serie

[Paraneoplasic polyneuropathy with anti-amphiphysin antibodies].

Paraneoplastic neurological syndromes consist of a dysfunction of any part of the nervous system, in isolation or in combination, caused by a malign neoplasia, but not by the direct tissular or metastasic invasion of the tumour. Their pathogeny is explained by immunological mechanisms and they are characterised by the presence of high rates of antibodies in serum and cerebrospinal fluid. We present the case of a patient with a sensitive neuropathy that produced ataxia, and who suffered from a poorly differentiated adenocarcinoma of the lung, in whom the search for antineuronal antibodies was p

[Clinical Phenomenology of Autoimmune Encephalitis].

Antibody-associated disorders of the central nervous system constitute a heterogeneous group of disorders that can be roughly divided into two categories: Classic paraneoplastic syndromes associated with so-called well-characterized antibodies (paraneoplastic neurological disorders, PND) and autoimmune disorders with antibodies to membrane-bound or synaptic antigens (autoimmune encephalitis, AE). The discovery of autoimmune encephalitis has led to a paradigm shift in diagnosis and therapy as well as a reclassification of some neuropsychiatric syndromes that were previously classified as idiopa

Autoimmune Encephalitis in Critical Care: Optimizing Immunosuppression.

Autoimmune diseases affecting the nervous systems are a common cause of admission to the intensive care unit (ICU). Although there exist several well-described clinical syndromes, patients more commonly present with progressive neurologic dysfunction and laboratory and radiographic evidence of central nervous system (CNS) inflammation. In the critical care setting, the urgency to intervene to prevent permanent damage to the nervous system and secondary injury from the systemic manifestations of these syndromes often conflicts with diagnostic uncertainty. Furthermore, treatment is limited by cu

Onco-neurology: a clinical perspective on cancer-associated neurological complications.

Advances in cancer screening and treatment have improved survival, while neurological complications in patients with cancer are gaining clinical relevance. Many of these complications are not attributable to direct tumor invasion, compression, or metastasis, but rather reflect immune-mediated mechanisms, treatment exposure, systemic disturbances, or tumor-associated biological processes. They are commonly discussed under categories such as paraneoplastic neurological syndromes, treatment-related neurotoxicity, immune-related adverse events, or related disorders. However, these categories often

An overview on CV2/CRMP5 antibody-associated paraneoplastic neurological syndromes.

Paraneoplastic neurological syndrome refers to certain malignant tumors that have affected the distant nervous system and caused corresponding dysfunction in the absence of tumor metastasis. Patients with this syndrome produce multiple antibodies, each targeting a different antigen and causing different symptoms and signs. The CV2/collapsin response mediator protein 5 (CRMP5) antibody is a major antibody of this type. It damages the nervous system, which often manifests as limbic encephalitis, chorea, ocular manifestation, cerebellar ataxia, myelopathy, and peripheral neuropathy. Detecting CV2

Neurological immune-related adverse events of immune-checkpoint inhibitors: an updated review.

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy across multiple tumor types and disease stages. By reinvigorating antitumor immune responses, these agents may also lead to the emergence of immune-related adverse events (irAEs), including a distinct spectrum of neurological toxicities. Although rare, neurological irAEs are clinically significant due to their diagnostic complexity, potential severity, and risk of long-term neurological sequelae or death. Neurological complications of ICI therapy involve the central nervous system, peripheral nervous system, and skeletal muscl

[Autoimmune mechanisms in the pathogenesis of neurologic paraneoplastic diseases].

Neurologic paraneoplastic syndromes (NPS) are disorders of the nervous system that are associated with the presence of malignant tumors. They are also known as the remote effects of cancer. There is mounting circumstantial evidence supporting the autoimmune origin of NPS. In NPS patients, paraneoplastic autoantibodies have been identified which react with the same or similar antigenic epitopes in tumor cells and neurons. However, except for the Lambert-Eaton myasthenic syndrome (LEMS) it remains still unproven as to whether the autoantibodies are pathogenic or merely represent an epiphenomenon

Paraneoplastic disorders of the peripheral nervous system.

Paraneoplastic neurological syndromes are rare but can affect any part of the peripheral nervous system (PNS) including motor neurons, sensory ganglia, nerve roots, plexuses, cranial and peripheral nerves, and neuromuscular junctions. The type of cancer, lymphoma or solid tumour, is a determinant factor of the underlying mechanism. With solid tumour, antibodies directed to intracellular (anti-Hu or anti-CV2/CRMP5 antibodies) or surface antigens (anti-VGCC,or LGI1 and Caspr2 antibodies) have been identified while with lymphoma, the neuropathy is usually linked to a monoclonal gammopathy. Paraneoplastic autonomic neuropathies and GI dysmotility.

A number of the well-recognized autoimmune and paraneoplastic neurologic syndromes commonly involve the autonomic nervous system. In some cases, the autonomic nerves or ganglia are primary targets of neurologic autoimmunity, as in immune-mediated autonomic ganglionopathies. In other disorders such as encephalitis, autonomic centers in the brain may be affected. The presence of autonomic dysfunction (especially gastrointestinal dysmotility) is sometimes overlooked even though this may contribute significantly to the symptom burden in these paraneoplastic disorders. Additionally, recognition of

Peripheral nervous system alterations in small cell lung cancer. Clinico-pathological study.

Small cell lung cancer (SCLC) is one of the most malignant tumors, especially often associated with nonmetastatic neurological disorders, corresponding to paraneoplastic neurological syndromes. The pathogenesis of which is unknown, however, mostly attributed to autoimmune processes. The aim of the study was to determine the pattern of the peripheral nervous system damage in SCLC. To provide further data contributing to the pathomechanism underlying these syndromes, immunocytochemical studies were initiated. Autopsy material was collected from 47 cases of SCLC. All these patients were examined

Current approaches to the treatment of paraneoplastic encephalitis.

Paraneoplastic neurological syndromes (PNSs) cover a wide range of diseases and involve both the central nervous system (CNS) and peripheral nervous system. Paraneoplastic encephalitis comprises several diseases such as paraneoplastic cerebellar degeneration (PCD), limbic encephalitis (LE), paraneoplastic encephalomyelitis (PEM), brainstem encephalitis, opsomyoclonus syndrome, in addition to other even less frequently occurring entities. LE was the first historically identified CNS PNS, and similarities between other temporal lobe diseases such as herpes encephalitis have been elucidated. In t

Adenocarcinoma of the right lung, potential cause of seronegative paraneoplastic encephalitis.

Paraneoplastic neurologic disorders are syndromes related to the cancer that can affect any part of the nervous system. Paraneoplastic encephalitis, one of its types, is an inflammatory condition mediated by immune responses triggered by neuronal proteins ectopically expressed by tumours or as a result of altered immunologic responses. Presentation of a malignancy can be varied, and paraneoplastic encephalitis is one of the ways of presentation. A detailed history and examination, clinical suspicion and thorough investigations are a must for its diagnosis. An atypical neurological presentation

Managing paraneoplastic neurological disorders.

Paraneoplastic neurological syndromes (PNS) are remote effects of cancer that are not caused by invasion of the tumor or its metastases. Immunologic factors appear important in the pathogenesis of PNS because antineuronal autoantibodies and T-cell responses against nervous system antigens have been defined for many of these disorders. The immunologic response is elicited by the ectopic expression of neuronal antigens by the tumor. Expression of these so-called "onconeural" antigens is limited to the tumor and the nervous system and sometimes also the testis. At the time of presentation of the

Neurological outcomes in immune checkpoint inhibitor-related neurotoxicity.

While the spectrum of neurological immune checkpoint inhibitor-related adverse events is expanding, patients' outcomes are not well documented. This study aimed to assess outcomes of neurological immune-related adverse events and to identify prognostic factors. All patients experiencing grade ≥2 neurological immune-related adverse events identified at two clinical networks (French Reference Center for Paraneoplastic Neurological Syndromes, Lyon; and OncoNeuroTox, Paris) over five years were included. Modified Rankin scores were assessed at onset, 6, 12, 18 months, and last visit. A multi-state
