In a 12-year-old girl with Horner's syndrome and an ipsilateral accommodative paresis, intravenous administration of edrophonium chloride improved the accommodative defect on several occasions, which suggested a possible relationship to myasthenia gravis. Additional testing of the accommodative paresis revealed some of the characteristics of Adie's syndrome and suggested a relationship to pupil-sparing Adie's syndrome.

An ususual complication after stapedectomy for otosclerosis is reported. After two vain attempts elsewhere, the tympanic cavity was opened for the third time. Since the incus was missing, a malleovestibuloplasty was undertaken, using the Schuknecht wire prosthesis. Hearing was very good. One day after the operation, a facial paralysis developed, and increased over the following days. After 18 days, the nerve was found to be oedematous and swollen above the fenestra ovale upto the level of the promontory and had forced the prosthesis out of the fenestra ovale. Six weeks later, nerve function wa

This article describes an anesthetic technique that induces temporary adductor vocal fold paresis and dense sensory loss of the posterior glottis. This method allows for improved precision of treatment and patient tolerance during awake office-based laryngeal surgery. Laryngoscope, 133:1673-1675, 2023.

One of frequent complications after adenotomy and tonsillectomy is paresis of the soft palate. Literature data, four groups of causes of soft palate paresis are analysed. The treatment was given to 28 patients with soft palate paresis of different etiology. Active treatment recovered function of the palate in most of the patients.

Superior laryngeal nerve paresis and paralysis are relatively common but often difficult to diagnose with certainty. They are most commonly caused by viral infections, though other etiologies must be considered. A thorough history and physical examination, including strobovideolaryngoscopy and laryngeal electromyography, are needed for definitive diagnosis. It is essential to establish the diagnosis accurately to differentiate an apparent superior laryngeal nerve paresis from other conditions, such as myasthenia gravis. Laryngeal electromyography is used to confirm clinical impressions, as a g…

There is neuroanatomical and clinical evidence that the corticospinal tract governs the patterns of pareses in sporadic ALS. These patterns are mirrored by phylogenetically young monosynaptic corticomotor neuronal connections. It is well known that, clinically, dysfunction of the deltoid muscle contributes considerably to the early disability of the ALS patient. In this study, we prospectively compared the degree of pareses of the deltoid muscle with the triceps and biceps brachii in N = 71 patients (426 muscles). We could show that the extent of involvement of the deltoid muscle early in the

The manifestations of CNS syphilis are unfamiliar to a differential of patients with dementia to many physicians today as of the relative rarity of this condition. This is a classical case report of a patient with syphilis and dementia in a 55-year-old female. General paresis of insane is a progressive disease of the brain leading to mental and physical worsening. It is important to consider tertiary syphilis in the differential diagnosis of dementia. Conventional presentations of neurosyphilis such as tabes dorsalis and general paresis of insane are read in textbooks only and rarely encounter…

Familial hypokalaemic periodic paralysis is a rare autosomal dominant neuromuscular disease characterized by episodic attacks of flaccid paralysis with concomitant hypokalaemia. We present a case of a 15-year-old male adolescent observed in the pediatric emergency department by flaccid paresis of the 4 limbs of sudden onset and progressive worsening. In the anamnesis, corticosteroid and antihistamine intake were observed on the previous day for urticaria and family history of transient episodes of flaccid paralysis in adolescence, asymptomatic after the fourth decade of life, without an establ…

Cold paresis may occur in multifocal motor neuropathy and lower motor neuron disease. It was proposed to reflect nerve lesions where axons are depolarized due to loss of Na/K-pump activity. In those circumstances, a further decrease in pump activity by cooling may induce extra depolarization, conduction block, and weakness. Evidence for this hypothesis is incomplete because it is unknown if cold induces depolarization in human motor axons and other factors may contribute to the symptoms. To solve these questions, we examined 10 normal subjects. At 37, 25, 20, and 15°C we assessed: excitability…
