Two cases of osteochondral fractures of the patella are presented. These osteochondral fractures have been reduced and fixed with the Herbert scaphoid screw. This screw has several advantages over conventional methods of treatment of osteochondral fractures in that the screw is entirely buried in the bony fragment and no portion of the screw lies proud. The differential pitch of the two threads in the screw allows interfragmentary compression. A series of 400 patellar fractures has been reviewed with a follow-up of three to 12 years. The continuity of the extensor apparatus had been interrupted in 73 per cent of cases and in 21 per cent the fracture was compound. The best results were obtained by conservative treatment with plaster, patellar suture or partial patellectomy. Total patellectomy, in contrast, only gave satisfactory results in one case out of two.

We report the first case of the assessment and treatment of a spontaneous patellar fracture in a patient with Goltz syndrome-a rare dermal hypoplasia. This case illustrates the non-straightforward presentation of a spontaneous patellar fracture in seemingly osteoporotic bone stock further complicated by the inability to rule out osteomyelitis. In this confusing presentation, a high index of suspicion for patella fracture should be maintained for patients with knee pain and osteoporosis on x-ray with a dysplastic syndrome.

Clinical assessment of the function of the knee joint after surgically and conservatively treated fracture of the patella was carried out. On the basis of C. Ranawat criteria it was found that regardless the method of treatment applied restriction of the joint function was expressed mainly by limited range of motion and by weakening of the knee extensors. The results of surgical and conservative management are similar. The best results of treatment for fracture of the patella were achieved in patients operated with the use of wire loop-cerclage.

When accurate reduction and stable fixation cannot be achieved in comminuted patellar fractures, degenerative changes in the patellofemoral joint can cause troublesome complaints. Amongst approximately 1 500 spastic children under treatment, the authors have seen three cases of fatigue fracture of the patella. One patient was spastic and two others were athetotic. The fatigue fractures can be explained by a flexed knee gait aggravated by a contracture of the hamstring tendons. No fracture was seen in patients treated by Egger's operation. It is concluded that this procedure both prevents and cures fatigue fracture of the patella.
