Electronic positive patient identification, computer provider order entry, and electronic remote blood issue implemented in operating rooms improve the safety and efficiency of blood transfusions.

INTRODUCTION: Blood transfusions are routinely performed in operation rooms. The process chain of performing ABO type, antibody screen, crossmatch, blood transport, and patient verification is complex. A multidisciplinary task force convened to improve the process of blood ordering and utilization identified 3 issues: (1) blood orders performed on paper included transcription errors; (2) guidelines for blood ordering practices were lacking, with inappropriate and arbitrary requests; and (3) there were long intervals between ordering and receiving blood in the operating room because of its dist

Machine learning-based identification of factors associated with spontaneous abortion in patients with Systemic lupus erythematosus (SLE): Insights from the Egyptian College of Rheumatology (ECR)-SLE cohort.

BackgroundSystemic lupus erythematosus (SLE), an autoimmune disease, predominantly affects women and is associated with an increased risk of spontaneous abortion (SA). However, traditional analytical methods found a modest relationship between some factors and SLE-SA and were limited to a small sample size, frequently associated with poor predictive performance.ObjectivesThis study aimed to apply and evaluate an Extreme Gradient Boosting (XGBoost) model using routinely collected clinical data to identify patterns associated with spontaneous abortion in women with SLE and to identify the key va

Technical Evaluation: Identification of Pathogenic Mutations in PKD1 and PKD2 in Patients with Autosomal Dominant Polycystic Kidney Disease by Next-Generation Sequencing and Use of a Comprehensive New Classification System.

Genetic testing of PKD1 and PKD2 is expected to play an increasingly important role in determining allelic influences in autosomal dominant polycystic kidney disease (ADPKD) in the near future. However, to date, genetic testing is not commonly employed because it is expensive, complicated because of genetic heterogeneity, and does not easily identify pathogenic variants. In this study, we developed a genetic testing system based on next-generation sequencing (NGS), long-range polymerase chain reaction, and a new software package. The new software package integrated seven databases and provided

[Analyses of 6 misidentifications in blood sampling in our hospital].

The initial step of blood transfusion therapy is blood type grouping. ABO-mismatch blood transfusion results in serious adverse effects. 6 incidents in the process of blood type identification, which were caused by misidentification in blood sampling, have experienced in our hospital since October, 2006 to September 2009. We retrospectively examined these incidents, and discussed the reasons and countermeasures. All of 6 incidents were caused by busy residents or nurses during busy hours such as early in the morning or in the emergency. To prevent these incidents, staffs of transfusion unit an

"Radiobiometrics": Deep-learning radiograph biometrics for patient identification.

When reporting on radiology follow-up examinations, radiologists should ensure that follow-up images and reference images are from the same patient to prevent misidentification errors. This is a nontrivial task when accounting for changes in the patient's condition and differences in image acquisition. Thus, we developed a system for automatic patient identification from radiographs using convolutional neural networks. Using deep metric learning, we trained multiple models to match radiographs of the chest, knees, pelvis, and hands. We also trained a model to match chest radiographs of multipl

In search of the name.

Existing communications standards represent person name, date, time, and other ubiquitous attributes in various incompatible formats. The electronic medical record requires convergence of diverse representational systems toward a single communications standard or a harmonized group of standards. The obstacles to convergence include disparities in semantic definition, syntax, and communications protocols. To facilitate harmonization of existing standards, the message standards developers subcommittee of the ANSI HISPP (American National Standards Institute Healthcare Informatics Standards Plann

Blood sampling guidelines with focus on patient safety and identification - a review.

It has been well documented over recent years that the preanalytical phase is a leading contributor to errors in the total testing process (TTP). There has however been great progress made in recent years due to the exponential growth of working groups specialising in the field. Patient safety is clearly at the forefront of any healthcare system and any reduction in errors at any stage will improve patient safety. Venous blood collection is a key step in the TTP, and here we review the key errors that occur in venous phlebotomy process and summarise the evidence around their significance to pa

Blood transfusion sampling and a greater role for error recovery.

INTRODUCTION: Patient identification errors in pre-transfusion blood sampling ('wrong blood in tube') are a persistent area of risk. These errors can potentially result in life-threatening complications. Current measures to address root causes of incidents and near misses have not resolved this problem and there is a need to look afresh at this issue. PROJECT PURPOSE: This narrative review of the literature is part of a wider system-improvement project designed to explore and seek a better understanding of the factors that contribute to transfusion sampling error as a prerequisite to examining

Secure e-Health: managing risks to patient health data.

e-Health, as an inter-jurisdictional enterprise, presents risks to patient health data that involve not only technology and professional protocols but also laws, regulations and professional security cultures. The USA Patriot Act is one example of how national laws can shape these concerns. Secure e-Health therefore requires not only national standardization of professional education and protocols but also global interoperability of regulations and laws. Some progress in this regard has been made in the European context; however, even here developments are incomplete, and nothing similar has b

Palatal Position of Patient Tracker for Magnetic Neuronavigation System: Technical Note.

OBJECTIVE: Recently, the neuronavigation system (NS) has become an essential intraoperative tool for many neurosurgical procedures, allowing for precise lesion localization. It is particularly important to avoid errors during the navigation process. Here we report a novel technique using palatal positioning of the patient tracker to ensure optimal accuracy during magnetic navigation in various neurosurgical procedures. METHODS: This retrospective study included a total of 34 patients treated in our institution between June 2017 and January 2018. The patients were split into 2 groups who under

Transfusion recipient identification.

Recent reports from different haemovigilance systems indicate that errors in the whole-blood transfusion chain - from initial recipient identification to final blood administration - occur with a frequency of approximately 1 in 1000 events. Although mistakes occur also within the blood transfusion service, about two-thirds of errors are associated with incorrect blood recipient identification at the patient's bedside. To prevent the potentially fatal consequences of such mistakes, specific tools have been developed, including patient identification bracelets with barcodes and/or radio frequenc

[Solitary mastocytoma associated with newborn infants' identification bracelets].

We report the case of solitary mastocytoma, which appeared under the identification bracelet on the left wrist of a 2-day-old newborn infant. Stroking the lesion elicited Darier's sign. Histopathologic examination of a punch biopsy specimen revealed a dense mast cell infiltrate of the dermis, which was confirmed by Giemsa and toluidine blue stains. We believe that this case represents and unusual reactive process.

[Management directed at preventing misidentifications by introduction of the computed identification system and blood sampling by the transfusion unit; aim for good partnership between a transfusion unit and bedsides].

The initial step of blood transfusion therapy is blood type grouping. ABO-mismatch blood transfusion results in serious adverse effects. Several incidents in the process of blood sampling had been experienced in our hospital since 2006 to 2008. Therefore, we have introduced the computed identification system, and the transfusion unit has taken a part of blood sampling. Just after we introduced it in July 2010, only 7% of the doctors and the nurses used the system in blood sampling. Repeated training programs for doctors and nurses on blood sampling procedure improved the utilization to 95%. We

Enhancing unique identification of patients with malaria using biometric fingerprinting in Papua, Indonesia.

BACKGROUND: Unique patient identification is necessary for detecting malaria recurrence and facilitates individualised treatment. Reliable patient identification in settings with poor information technology infrastructure is challenging. A before-and-after study was conducted to investigate whether the use of biometric fingerprinting alongside an existing physical 'malaria card' system improved unique identification of patients with malaria at a busy, publicly-funded clinic in Papua, Indonesia. METHODS: A three-phase study was conducted at Wania clinic in Timika over a 16-month period. In Pha

Plasma proteins from several components of the immune system differentiate chronic widespread pain patients from healthy controls - an exploratory case-control study combining targeted and non-targeted protein identification.

Chronic widespread pain (CWP), including fibromyalgia (FM), is characterized by generalized musculoskeletal pain and hyperalgesia. Plasma proteins from proteomics (non-targeted) and from targeted inflammatory panels (cytokines/chemokines) differentiate CWP/FM from controls. The importance of proteins obtained from these two sources, the protein-protein association network, and the biological processes involved were investigated. Plasma proteins from women with CWP (n = 15) and CON (n = 23) were analyzed using two-dimensional gel electrophoresis analysis and a multiplex proximity extension assa
