[Nosocomial bacteremias in pediatrics].

OBJECTIVES: To identify pathogenic microorganisms responsible for hospital-acquired bloodstream infections and to evaluate the associated risk factors in pediatric units, in a case-control study over 30 months from January 1st 1997 to June 30th 1999. RESULTS: Forty-six of 855 (5.4%) positive blood cultures were attributed to nosocomial infections. They were related to 32 infectious episodes in 28 patients hospitalized for more than 48 hours. The incidence rate was 0.11 per 100 admissions. Gram-positive cocci (n = 14; 38.8%) were the most frequently isolated pathogens (7 cases of Staphylococcu

Psychosomatic disorders in pediatrics.

Psychosomatic symptoms are by definition clinical symptoms with no underlying organic pathology. Common symptoms seen in pediatric age group include abdominal pain, headaches, chest pain, fatigue, limb pain, back pain, worry about health and difficulty breathing. These, more frequently seen symptoms should be differentiated from somatoform or neurotic disorders seen mainly in adults. The prevalence of psychosomatic complaints in children and adolescents has been reported to be between 10 and 25%. These symptoms are theorized to be a response to stress. Potential sources of stress in children a

[Chronic cough in pediatrics].

The objective has been to identify the different etiologies and elaborate a diagnostic and therapeutical methodology for patients with chronic cough. During one year we studied prospectively 83 patients with persistent cough of daily appearance with an evolution of four or more weeks and no previous etiologic diagnosis. We worked on three diagnostic (D) levels. D1: Based on the anamnesis and physical examination. D2: Sequential incorporation of complementary exams. D3: Evaluation of the response to the specific treatment. We divided the population into 2 groups: G1 healthy children, G2 childre

[Disseminated mucormycosis in pediatrics].

The case of a 14 month-old infant presenting with hereditary lymphohistiocytosis is reported. After several days of post-chemotherapy aplasia a peritoneal, cutaneous (blackish necrosis), then pleuropulmonary involvement occurred. Fiberendoscopy showed an esophageal necrosis responsible for a pleural fistula. The child died after a few weeks, despite antifungal treatment. Post mortem examination found disseminated abscesses related to mucormycosis. The 41 pediatric cases in the world-wide literature are recorded.

[Aseptic meningitis in pediatrics].

We analyzed the 259 pediatric patients (69% males, 31% females) treated for aseptic meningitis at the Department of Pediatrics (Inselspital), University of Berne, Switzerland, during the 10-year period from 1978 to 1987. 79% were in- and 21% outpatients. The main study purpose was the evaluation of clinical presentation, diagnostic workup, course and prognosis of this disease entity. The clinical characteristics (fever, meningitic signs, headache, nausea and vomiting, and usually normal consciousness) were not discriminative for differential diagnosis. On the other hand, pleocytosis in cerebro…

[Opportunistic mycoses in pediatrics].

The two most important pathologic conditions leading to mycotic opportunistic infections in children are impairment of mechanisms of defense due to immunosuppressive drugs and congenital defects of immunity. Other circumstances belong to pediatrics such as prematurity or cystic fibrosis. A few examples are chosen to illustrate these situations: congenital candidiasis, chronic mucocutaneous candidiasis, chronic dermatophytic disease, neonatal candidemia, mycotic infections in chronic granulomatous disease and aspergillosis in cystic fibrosis.

Wound botulism in pediatrics.

A 6-year-old boy with congenital deafness sustained compound fractures to his left ulna and radius. Six days after the injury, he appeared lethargic, and his illness progressed to respiratory failure within three days. Although the wound at the fracture site appeared benign, cultures obtained when the wound was opened grew Clostridium botulinum, type B. Both the patient's deafness and the appearance of his wound contributed to the delay in diagnosis of wound botulism. Differential diagnosis and treatment of this rare entity are discussed.

Gallium imaging in pediatrics.

Gallium-67 citrate imaging was carried out in 59 children from 3 mo to 20 yr of age. Indications for the study included the search for occult inflammatory disease and the detection and staging of malignant disease. The Ga-67 citrate scan had a 96% reliability in confirming or excluding the site of purulent material as a cause of sepsis, and 76% reliability in detecting malignant disease. Tissue distributions in children differ from those in adults primarily in that the epiphyseal plates, spleen, and thymus may show increased activity normally and in the presence of sepsis. These variations are
