OBJECTIVE: To determine the mortality rate and its predictors in patients with a presumptive diagnosis of tuberculous pericarditis in sub-Saharan Africa.
DESIGN: Between 1 March 2004 and 31 October 2004, we enrolled 185 consecutive patients with presumed tuberculous pericarditis from 15 referral hospitals in Cameroon, Nigeria and South Africa, and observed them during the 6-month course of antituberculosis treatment for the major outcome of mortality. This was an observational study, with the diagnosis and management of each patient left at the discretion of the attending physician. Using Cox

We performed this study to determine the predictors of early and long-term survival in the surgical treatment of tuberculous pericarditis and to examine the risks of pericardiectomy and the functional outcome in patients after surgery. A retrospective analysis was undertaken in 36 consecutive patients, 26 female and 10 male, with a mean age 32.2 +/- 16.3, who underwent pericardiectomy for chronic constrictive pericarditis from February 1985 to February 2002. All patients received antitubercular therapy in the postoperative period. The operative mortality rate was 6% (2 patients); the cause of

BACKGROUND: Calcific constrictive tuberculous pericarditis is rare in the United States and even more uncommon in pregnancy, but it carries significant morbidity and requires individualized, multidisciplinary counseling.
CASE SUMMARY: A 40-year-old woman with a history of tuberculosis presented at 16 weeks' gestation with symptomatic calcified constrictive pericarditis. Despite medical management, she developed progressive hemodynamic compromise. Given her ongoing symptoms and preference to avoid pericardiectomy, she elected for second-trimester termination after shared decision-making.

BACKGROUND: Constrictive pericarditis is a well-recognized complication of tuberculous pericarditis, whereas severe ventricular systolic dysfunction is rare.
CASE SUMMARY: A 20-year-old man presented with progressive heart failure and was found to have severe biventricular systolic dysfunction (left ventricular ejection fraction <15%) with marked pericardial thickening. Conventional investigations for tuberculosis were negative. Despite empirical antituberculosis therapy and optimal heart failure treatment, he continued to deteriorate and subsequently underwent radical pericardiectomy. The po

Tuberculosis is a transmissible disease caused by the bacterium Mycobacterium tuberculosis. It is the leading cause of mortality due to infectious diseases. Tuberculous pericarditis is one of the manifestations of extrapulmonary tuberculosis and represents the primary cause of pericardial effusion in developing countries. We present the case of a 71-year-old male patient with a history of 1 month of dyspnea, accompanied by paroxysmal nocturnal dyspnea and unintentional weight loss. An echocardiogram revealed the presence of severe pericardial effusion, for which pericardiocentesis was performe

Pericarditis is a rare manifestation of tuberculosis (Tb) in children. A 14-yr-old Korean boy presented with cardiac tamponade during treatment of pulmonary tuberculosis. He developed worsening anemia and persistent fever in spite of anti-tuberculosis medications. Echocardiography found free floating multiple discoid masses in the pericardial effusion. The masses and exudates were removed by pericardiostomy. The masses were composed of pink, amorphous meshwork of threads admixed with degenerated red blood cells and leukocytes with numerous acid-fast bacilli, which were confirmed as Mycobacteri

As tuberculosis still exists in Japan, tuberculous pericarditis is a major health issue. Tuberculous pericarditis is difficult to diagnose and leads to poor outcomes when left untreated. We herein report the case of a patient who was admitted to the hospital after undergoing resuscitation for cardiopulmonary arrest. Mycobacterium tuberculosis was detected in his hemorrhagic pericardial fluid and tuberculous pericarditis was diagnosed. The administration of antituberculous medication resulted in marked improvements. A diagnosis of tuberculous pericarditis, in addition to other causes such as ma

56 year-old man was referred to the department of orthopedics in our hospital for further investigation on right inguinal pain. The patient was initially diagnosed as bacterial myelitis in right pubic bone and was treated with antibiotics. Since his symptom did not improve, the curettage was performed. Histological examination of the pubic bone obtained during the operation showed epithelioid cell granulomas with caseous necrosis, supporting the diagnosis of tuberculous osteomyelitis. Chest X-ray film revealed small nodular lesions in both upper lung fields. Sputum was positive for acid-fast b

BACKGROUND AND OBJECTIVES: In East Asia, tuberculous pericarditis still occurs in immunocompetent patients. We aimed to investigate clinical course of tuberculous pericarditis and the trends of echocardiographic parameters for constrictive pericarditis.
METHODS: We retrospectively analyzed medical records of patients with tuberculous pericarditis between January 2010 and January 2017 in Samsung Medical Center. Treatment consists of the standard 4-drug anti-tuberculosis regimen for 6 months with or without corticosteroids. We performed echocardiography at initial diagnosis, 1, 3 and 6 months l

Mycobacterium tuberculosis (TB) presenting with pericardial disease complicated by cardiac tamponade is rare in the developed world, although it occurs more frequently in the context of immunosuppression. In this report, a 74-year-old man on methotrexate for rheumatoid arthritis presented with fever, productive cough and cough-induced syncope. During his admission, he developed clinical signs of cardiac tamponade confirmed on an echocardiogram, which showed a massive pericardial effusion. He was treated with an urgent pericardiocentesis and a pericardial window. Subsequently, TB polymerase cha

Synovial sarcomas of the pericardium are extremely rare and associated with poor survival rate. The current case report describes a 45-year-old female who presented with dyspnea upon exertion, a paroxysmal cough, night sweats and recurrent pericardial effusion. The patient was diagnosed with tuberculous pericarditis and received antituberculous drug therapy. Echocardiography and magnetic resonance imaging (MRI) revealed a pericardial mass lying predominantly over the right atrium. The patient was treated by surgical excision and a subsequent histological analysis confirmed the diagnosis of a p

A conservative approach to operative intervention in the treatment of active tuberculous pericarditis was adopted and only 4 of 16 consecutive patients underwent pericardiectomy, all within 2 months of diagnosis. One patient died of constrictive pericarditis despite pericardiectomy, and one died of acute bronchopneumonia after 8 months of otherwise successful medical management. All 14 long-term survivors were reevaluated to exclude chronic constrictive pericarditis and other potential sequelae of tuberculous pericarditis. Reevaluation included physical examination, chest radiograph, electroca

Tuberculous pericarditis (TBP) is a rare but potentially life-threatening manifestation of extrapulmonary tuberculosis (TB), representing a clinically important subset of pericarditis cases. Diagnostic uncertainty arises from variable symptoms and the low sensitivity of available tests. Although culture is the gold standard, it is reliable but too slow to enable a timely diagnosis. Acute and fulminant presentations have been reported in immunosuppressed patients. A 61-year-old woman, recently receiving infliximab and corticosteroids for Crohn's disease, presented to the emergency department wi

Tuberculous pericarditis (TBP) is a rare but potentially life-threatening manifestation of extrapulmonary tuberculosis, particularly in regions with low tuberculosis (TB) prevalence. It accounts for a small fraction of pericardial effusion cases in high-income countries, yet remains a significant diagnostic challenge due to its nonspecific presentation and overlap with other infectious, inflammatory, or malignant causes. Here, we present a case of a tuberculous pericarditis in a 32-year-old male, originally from Congo, who presented with constitutional symptoms, pleuritic chest pain, and tachy

BACKGROUND: Tuberculous pericarditis is a rare manifestation of tuberculosis infection. COVID-19 pandemic poses a challenge in detecting uncommon diseases.
CASE SUMMARY: A 47-year-old man was admitted with symptoms of COVID-19 infection. Rapid progression of cardiomegaly on radiograph with clinical deterioration were suggestive of pericardial tamponade. Urgent pericardiocentesis revealed haemoserous fluid, elevated adenosine deaminase, and positive tuberculous (TB) polymerase chain reaction (PCR). He was started on anti-TB therapy and Remdesivir with marked improvement of symptoms. Repeat ech

SETTING: Tuberculosis (TB) is the most common cause of pericarditis worldwide and carries a high mortality, even with effective anti-tuberculosis treatment. In the light of a randomized control trial in 2014, the American Thoracic Society and the Centers for Disease Control and Prevention/Infectious Diseases Society of America recently revised their recommendations against the routine use of adjunctive corticosteroids.
OBJECTIVE: To evaluate the strength of evidence that resulted in this reversal of the guideline recommendations on the use of adjunctive corticosteroids in TB pericarditis by a

RATIONALE: Constrictive pericarditis (CP) is a diagnostic challenge due to its nonspecific and insidious presentation.
PATIENT CONCERNS: A 38-year-old man presented with bilateral lower limb edema and exertional dyspnea.
DIAGNOSES: Cardiac magnetic resonance imaging demonstrated pericardial thickening, late gadolinium enhancement, and interventricular septal bounce. Tuberculous CP was considered based on imaging findings, positive interferon-gamma release assay, therapeutic response, and epidemiologic background.
INTERVENTIONS: The patient was treated with anti-tuberculosis therapy without
