Differential uptake of salicylate in serum, cerebrospinal fluid, and perilymph.

After intraperitoneal administration of salicylate in anesthetized rats and guinea pigs, we found that salicylate levels in perilymph (PL) are closely related to both drug levels in cerebrospinal fluid (CSF) and in serum, with higher levels systematically observed in PL than in CSF. Further analysis suggests that salicylate is not passively transported into PL across CSF but, rather, is transported from blood directly to PL. The time course of salicylate uptake in rats reveals maximum levels at 1 1/2 hours (serum) and two to four hours (CSF and PL). On the other hand, salicylate uptake into se…

Quantitated determination of proteins in perilymph in patients with acoustic neuromas.

Perilymph, cerebrospinal fluid (CSF) and plasma from 19 patients with acoustic neuromas were examined for albumin, alpha 2-macroglobulin and IgG. Patients with otosclerosis were used as controls. A highly significant increase of the tested proteins was found in perilymph from patients with acoustic neuromas. Determination of the tau-transferrin, which is a specific CNS protein, could neither be demonstrated in plasma nor in perilymph. This is in favour of the assumption that perilymph is plasma-derived and not concentrated CSF. Various causes for the high protein concentrations are discussed, …

Distinct MicroRNA Profiles in the Perilymph and Serum of Patients With Menière's Disease.

Hypothesis: Menière's disease microRNA (miRNA) profiles are unique and are reflected in the perilymph and serum of patients. Background: Development of effective biomarkers for Menière's disease are needed. miRNAs are small RNA sequences that downregulate mRNA translation and play a significant role in a variety of disease states, ultimately making them a promising biomarker. miRNAs can be readily isolated from human inner ear perilymph and serum, and may exhibit disease-specific profiles. Methods: Perilymph sampling was performed in 10 patients undergoing surgery; 5 patients with Meniere's di…

Effect of artificial endolymph injection into the cochlear duct on perilymph potassium.

OBJECTIVE: To investigate the relationship between endolymphatic hydrops and perilymphatic potassium. METHODS: 20 pigmented guinea pigs were used: 10 for scala vestibuli study and 10 for scala tympani study. Acute endolymphatic hydrops was produced by microinjection of an artificial endolymph into the scala media. Injections were performed in the second turn at rates up to 500 nl/min for a period of 10 min. The injection volume was up to 5 microl. Endocochlear potential (EP) was monitored during injections. Simultaneous with the injections, the potassium concentrations in scala vestibuli (K(S…

Probenecid reduces cochlear effects and perilymph penetration of furosemide in chinchilla.

Previous investigation has suggested that the ototoxicity of furosemide is related to penetration of the drug into the inner ear and that active drug transport out of the inner ear may be responsible for maintaining the serum-perilymph drug concentration gradient. We further tested this hypothesis by investigating the endocochlear potential (ototoxicity) and furosemide perilymph concentrations after furosemide administration to chinchillas pretreated with the organic anion transport inhibitor, probenecid. Probenecid pretreatment attenuated the fall in endocochlear potential seen after furosemi…

Perilymph fistula: a cause of auditory, vestibular, neurological and psychiatric disorder.

It is suggested that damage by mild trauma, viruses or bone disease to the otic capsule or to the membranes between the cochlea and the middle ear is common, and involved in many syndromes of obscure etiology. The clinical perilymph fistula (PF) syndrome can consist of any combination of the following: tinnitus, deafness, phonophobia, vertigo, ataxia, otalgia, facial palsy, headache, diplopia, blackouts, psychological distress. The following testable hypotheses are proposed: otitis media is due to perilymph in the middle ear, with secondary changes resulting from infection or inflammation: oto…

Intensity-dependent changes in oxygenation of cochlear perilymph during acoustic exposure.

This study examined the effects of acoustic exposure at different intensities on local oxygenation of the cochlea. The oxygen partial pressure (pO2) of perilymph in the basal scala tympani was measured polarographically in anesthetized guinea pigs exposed to either wide-band noise at 85 dB SPL or a 10 kHz pure tone at 90, 105, or 125 dB SPL for 1 h. Cochlear temperature, heart rate, arterial blood pressure and acid-base status were monitored. The cochlear microphonics (CM) and compound action potentials (CAP) were recorded before and after exposure. There were clear intensity-dependent differe…

Diagnostic criteria of barotraumatic perilymph fistula based on clinical manifestations.

CONCLUSIONS: Patients with sudden hearing loss and dizziness after barotrauma should be suspected of barotraumatic perilymph fistula (PLF). Early surgical repair of PLF showed better hearing outcomes. Therefore, diagnostic criteria should help surgical indications of barotraumatic PLF. OBJECTIVES: The aim of this study was to establish diagnostic criteria for barotraumatic PLF. METHODS: Twenty-four patients (26 ears) underwent surgery on suspicion of barotraumatic PLF. The causes of barotrauma and clinical symptoms were analyzed by surgical findings. Diagnostic criteria of PLF were proposed …

Gentamicin concentration gradients in scala tympani perilymph following systemic applications.

It has been shown in prior studies that round window membrane (RWM) application of gentamicin produced a robust basal-apical concentration gradient in the perilymph of scala tympani (ST) with peak concentrations in the basal turn of ST. These gradients potentially contribute to the clinical efficacy and safety of intratympanic gentamicin applications for the treatment of Ménière's disease. The present study aimed to establish the distribution of gentamicin along ST perilymph after systemic applications. Gentamicin sulfate was applied intravenously in the amounts of 100, 300 and 600 mg/kg body …

[Protein concentration of perilymph in guinea pigs after noise exposure].

The protein concentration in the perilymph of noise-exposed guinea pigs was investigated using a micromethod described previously. The animals were exposed to wide band noise at 140 dB for one hour unilaterally in a closed acoustic system. Perilymph was obtained after exposure at different intervals: immediately, 6 or 24 hours later. From animals exposed to noise, it is particularly more difficult to obtain perilymph without blood contamination than from normal ones. Even post mortem, it is difficult to obtain samples without blood contamination and without hemolysis, particularly from the sca…

The missing perilymph sign on MRI indicates a perilymphatic fistula: A case report.

A perilymphatic fistula (PLF) is a rare condition in which perilymph leaks from the cochlea or vestibule into the surrounding cavities, most commonly through round and oval windows, and causes cochlear and vestibular symptoms. However, vague symptoms and the lack of a clear diagnostic test have made the existence of PLF a controversial subject for decades. Here, we report a case of definite PLF confirmed by surgery in a patient who underwent mastoidectomy 20 years prior, revealing a specific sign of missing perilymph on MRI. T2-weighted MRI revealed a nodular bright signal in the cochlea and v…

Value of Endolymphatic Hydrops and Perilymph Signal Intensity in Suspected Ménière Disease.

BACKGROUND AND PURPOSE: Ménière disease is characterized by endolymphatic hydrops, whereas perilymphatic enhancement on MR imaging has been suggested to be of additional value in diagnosing Ménière disease. This study evaluates the presence of endolymphatic hydrops and perilymphatic enhancement in patients with Ménière disease and with other vertigo-associated inner ear pathology. MATERIALS AND METHODS: A 3D-FLAIR sequence 4 hours after intravenous gadolinium injection was performed to visualize the endolymph and perilymph in 220 patients suspected of having Ménière disease. Patients' ears we…

Freeze-fracture studies on the perilymph-endolymph barrier in experimentally induced hydrops.

A double-blind study was performed on nine pigmented guinea pigs which were unilaterally operated on in order to create an endolymphatic hydrops. After 12-18 months, both inner ears of the animals were removed and investigated by the freeze-fracture technique. The ultrastructure of the epithelial cells lining the endolymphatic compartment of the cochlea and the vestibular apparatus was analyzed. Special attention was directed to possible changes of the tight junctions (zonulae occludentes) in these sites. The number and depth of strands between the cells of sensory and non-sensory epithelia of

Non-collagen proteins of stapedial bone matrix in perilymph of otosclerotic patients.

Non-collagen proteins extracted from the otosclerotic stapes footplate, superstructure and temporal cortical bone were compared with the protein patterns of normal and otosclerotic perilymph by analytical isotachophoresis (ITP). Normal and otosclerotic perilymph yielded basically identical isotachophoretic subfractions. Thirteen subfractions were detected moving as anions at pH 9.6 in the normal perilymph, versus 16 subfractions of similar character in the otosclerotic perilymph. Of these 16 protein subfractions, one (which was not detected in the normal perilymph) could be found in the ITP-gr…

Sudden or fluctuating hearing loss and vertigo in children due to perilymph fistula.

Five cases are presented of children with rapid onset of sensorineural hearing loss, disequilibrium, or both, who were found at exploratory tympanotomy to have a perilymph fistula. Four of the children had histories suggesting that antecedent barotrauma or physical exertion contributed to the development of the fistula. One child with congenital unilateral craniosynostosis had a residual temporal bone abnormality on the same side as the perilymph fistula. Two children had identifiable anatomic abnormalities in the middle ear. A classification of perilymph fistula is proposed that describes a c…
