We have compared the effects of ondansetron and perphenazine on vomiting after tonsillectomy in 216 healthy children, aged 2-12 yr. The study was randomized, stratified, blocked and double blind. Anaesthesia was induced with propofol i.v. or by inhalation of halothane and nitrous oxide. Ondansetron 150 micrograms kg-1 or perphenazine 70 micrograms kg-1 was administered i.v. after induction of anaesthesia in a double-blind manner. Using data from a therapeutic drug monitoring database, kinetic interactions between the neuroleptics zuclopenthixol and perphenazine and tricyclic antidepressives were studied. Out of 290 patients monitored for amitriptyline and 611 patients monitored for nortriptyline, 77 patients were comedicated with perphenazine and 50 patients with zuclopenthixol. Perphenazine belongs to the group of phenothiazine-based neuroleptic drugs frequently used in the long-term treatment of psychotic disorders. In this work, a new capillary zone electrophoresis (CZE) method for the rapid determination of perphenazine (PPZ) in pharmaceutical preparations and human blood serum was developed. Using solid-phase extraction (SPE) as a preclean/purification and preconcentration step before CZE analysis, a detection limit of 3 ng/mL for the monitoring of PPZ in blood serum (for a signal-to-noise ratio of 3) was reached.
