Preventive Approach to Phantom Limb Pain in Lower Limb Ablation Surgery: A Systematic Review.

There is accumulating evidence that mitochondrial dysfunction is associated with the contribution of diabetes to Alzheimer disease (AD) progression. Neuronal mitochondrial proteins are found in plasma neuronal-derived exosomes (NDEs) at levels that reflect those in brain neurons. Here, we tested the performance of mitochondrial proteins in plasma NDEs to predict cognitive decline and brain injury in participants with diabetes. The study participants with type 2 diabetes mellitus (T2DM) included 41 cognitively normal control subjects, 97 individuals with mild cognitive impairment (MCI) (68 indi

The effect of mirror therapy on the management of phantom limb pain.

OBJECTIVES: In the last two decades, mirror therapy has become a frequently used method of managing phantom limb pain (PLP). However, the role of nurses in mirror therapy has not yet been well defined. This study examined the effect of mirror therapy on the management of PLP, and discusses the importance of mirror therapy in the nursing care of amputee patients. METHODS: This quasi-experimental study was conducted in the pain management department of a university hospital and a prosthesis clinic in İstanbul, Turkey, with 15 amputee patients who had PLP. Forty minutes of practical mirror thera…

The perception of phantom limbs. The D. O. Hebb lecture.

Almost everyone who has a limb amputated will experience a phantom limb--the vivid impression that the limb is not only still present, but in some cases, painful. There is now a wealth of empirical evidence demonstrating changes in cortical topography in primates following deafferentation or amputation, and this review will attempt to relate these in a systematic way to the clinical phenomenology of phantom limbs. With the advent of non-invasive imaging techniques such as MEG (magnetoencephalogram) and functional MRI, topographical reorganization can also be demonstrated in humans, so that it …

Increasing phantom limb pain as a symptom of cancer recurrence.

Previous studies have shown that phantom limb pain following major amputation reaches its greatest severity 2 to 3 weeks following amputation; it then gradually diminishes over subsequent months and years. Transient episodes of severe phantom limb pain are sometimes temporally related to specific activities such as urination, sexual intercourse, or local pressure applied to the amputation stump. Also, neuroma formation may be associated with transient episodes of increased discomfort usually associated with the application of local pressure. Phantom limb pain progressively increasing over an e…

Haunting of the phantom limb pain abolished by buprenorphine/naloxone.

Neuropathic opioid refractory phantom limb pain (PLP) following amputation can be a life long debilitating chronic pain syndrome capable of completely destroying a patient's life. The pain, its associated depression and sleep deprivation can make many patients suicidal. Ever changing and relentless, it is notoriously unresponsive to traditional cocktails of strong opioids, adjuvant pain medications, antidepressants, local anaesthetics, nerve stimulators, hypnotics and psychotropics. Drug effects are seldom more effective than placebo. We describe a successful sustained rescue of a difficult 2-…

Mirror Therapy in Unilateral Amputee Patients With Phantom Limb Pain.

UNLABELLED: Up to 90% of amputee patients have phantom limb pain (PLP), which is difficult to treat. The aim of this study was to assess the medium and long-term efficacy of mirror therapy (MT) in unilateral amputee adults with PLP. METHODS: We carried out an uncontrolled prospective longitudinal study. Twelve consecutive adult subjects with upper or lower limb amputation and PLP were selected. Weekly sessions of nurse-guided, in-person MT training lasting 1.5 h were implemented for six months. In parallel, self-administered home sessions were added 20 min per day, five days per week for 12 m…

Treating phantom limb pain: cryoablation of the posterior tibial nerve.

Phantom limb pain (PLP) is a complex pathophysiologic process involving both the central and peripheral nervous system for which there is no definitive treatment. The number of individuals living with amputated limbs is predicted to increase to 3.5 million by 2050, and up to 80% of these patients will have PLP. In this case report, we will demonstrate successful reduction of PLP in a patient with bilateral phantom toe pain utilizing nerve blockade and subsequent cryoablation of the posterior tibial nerves.

Attempting to prevent persistent postamputation phantom limb and stump pain.

Persistent postsurgical pain after amputation is common and has a number of proposed risk factors. We describe the management of a patient with persistent pain after a below-the-knee amputation. We used a combination of general, peripheral, and neuraxial anesthesia techniques to control postoperative phantom limb and stump pain in a patient at high risk for developing chronic pain. With preemptive management and mechanism-directed therapies, adequate preoperative and postoperative pain control were achieved in attempts to reduce the risk of chronic phantom limb pain.

Cortical stimulation mapping of phantom limb rolandic cortex. Case report.

Findings of intraoperative rolandic cortex mapping during awake craniotomy for a tumor in a patient with a contralateral upper-extremity amputation are presented. This patient sustained a traumatic amputation at the mid-humerus 24 years previously. Initially he had experienced rare painless phantom limb sensations but none in the past 10 years. Functional mapping during an awake craniotomy was performed to maximize safe tumor resection. Typical temporal and frontal speech areas were identified; motor representation of face and jaw extended more superiorly than sensory representation. Shoulder …

Case Report: Phantom limb pain relief after cognitive multisensory rehabilitation.

INTRODUCTION: Relieving phantom limb pain (PLP) after amputation in patients resistant to conventional therapy remains a challenge. While the causes for PLP are unclear, one model suggests that maladaptive plasticity related to cortical remapping following amputation leads to altered mental body representations (MBR) and contributes to PLP. Cognitive Multisensory Rehabilitation (CMR) has led to reduced pain in other neurologic conditions by restoring MBR. This is the first study using CMR to relieve PLP. METHODS: A 26-year-old woman experienced excruciating PLP after amputation of the third p…

Phantom limb pain: peripheral neuromodulatory and neuroprosthetic approaches to treatment.

Post-amputation phantom limb pain (PLP) is a widespread phenomenon that can have physical, psychological, and functional impacts on amputees who experience the condition. The varying presentations and mechanisms of PLP make it difficult to effectively provide long-term pain relief. Multiple neuromodulatory approaches to treating PLP have focused on electrical stimulation of the peripheral nervous system, with varying degrees of success. More recently, research has been done to study the effects of neuroprosthetic approaches on PLP. Neuroprosthetics combine the use of a functional prosthetic wi…

Maladaptive plasticity: imprinting of past experiences onto phantom limb schemata.

OBJECTIVES: Phantom limb perception is common following amputation, and is sometimes characterised by pain that resembles the characteristics, intensity or location of past pain. We tested Flor's model that phantom pain results from memory for long-lasting znoxious input. METHODS: We report a questionnaire study of 283 amputees, that explored the experience of painful, non-painful and postural somatosensory memories in the phantom. We explore the impact of pre-amputation pain and impairment duration, and complications in the limb (eg, infection, gangrene, surgery, and vascular disease). Diffe…

Training with virtual visual feedback to alleviate phantom limb pain.

BACKGROUND: Performing phantom movements with visual virtual feedback, or mirror therapy, is a promising treatment avenue to alleviate phantom limb pain. However the effectiveness of this approach appears to vary from one patient to another. OBJECTIVE: To assess the individual response to training with visual virtual feedback and to explore factors influencing the response to that approach. METHODS: Eight male participants with phantom limb pain (PLP) resulting from either a traumatic upper limb amputation or a brachial plexus avulsion participated in this single case multiple baseline study…

Phantom limb pain induced in amputee by strong magnetic fields.

An amputee (traumatic) experienced increased phantom limb pain when exposed to the magnetic fields of two magnetic resonance imagers. With a visual analog scale used to measure pain, electric stimulation studies demonstrated that the residual limb was unusually sensitive to subthreshold (for muscle twitch) levels of current. The painful symptoms produced mimicked those experienced in the presence of the imagers.
