The influence of flow separation location on eigenmode synchronization and phonation onset was investigated in a two-dimensional, aeroelastic, continuum model of phonation. A linear stability analysis showed that flow separation played a critical role in initiating eigenmode synchronization and phonation. For a given glottal configuration, a small variation in the flow separation location along the vocal fold surface may lead to a qualitatively different eigenmode-synchronization pattern, and different phonation threshold pressure and frequency. This high sensitivity suggests a need for phonat…

This paper ranks the importance of unsteady aerodynamic mechanisms in glottal flow. Particular emphasis is given to separation point motion, acceleration of glottal airflow by vocal fold motion, and viscous blockage. How nondimensional parameters such as the Reynolds, Strouhal, and Womersley numbers help in this ranking is also addressed. An equation of motion is derived which includes terms explicitly describing the effects of interest, assuming (1) a symmetrical glottis, (2) zero pressure recovery downstream of the vocal folds, and (3) a quasisteady glottal jet. Estimating the order of magni

Experts dealing with patients of chronic upper and lower airway disorders have drawn a lot of interest in the last decades to gastrooesophageal reflux disease (GORD). Nowadays it is obvious that 'occult GORD' may be an aetiological factor in this group of patients. GORD may has a role in lot of organic laryngeal diseases and functional voice disorders. Symptoms are triggered by gastric content refluxed into the oesophagus, which cause irritation and inflammation. At laryngeal and phoniatric examinations gastrointestinal signs might remain hidden behind chronic cough, hoarseness and globus sens

Recent evidence suggests that the lung-thorax system functions as a constant pressure source during phonation. However, previous animal models used a constant flow source. This article describes an in vivo canine model that maintains a constant subglottic pressure during phonation to more closely simulate the pulmonary system. At any given subglottic pressure, increasing levels of recurrent laryngeal nerve stimulation resulted in a significant rise in resistance followed by a plateau. Increasing levels of superior laryngeal nerve stimulation, however, produced no significant change in glottal …

In 8 healthy probands, 6 patients with functional voice disorders, 3 singers and 3 deaf patients we studied the regulation of the voice parameters amplitude and fundamental frequency at the beginning of phonation. Analyses were performed under the following conditions: (1) normal conditions; (2) after troubling pharyngeal and/or glottic mucosal receptors by topical anaesthesia; (3) in the case of normal hearing during disturbance of the audiophonatory feedback mechanism by binaural masking, and (4) with a combination of the latter two situations. The results are discussed.

OBJECTIVES: We describe a survival nonstimulated in vivo canine phonation model using distending laryngoscope, cramp frame, and constant humidified glottal airflow to elicit phonation.
METHODS: Five beagle dogs were involved in this study. One cuffed endotracheal tube was placed below the glottis through the tracheotomy and delivered humidified airflow to the glottis. Arytenoids approximation was maintained using a clamp under the distending laryngoscope. Acoustic and aerodynamic parameters were measured using synchronous signal collection system and analysis software. Vocal oscillation also …

Recent experimental studies showed that isotropic vocal fold models were often blown wide apart and thus not able to maintain adductory position, resulting in voice production with noticeable breathy quality. This study showed that the capability of the vocal fold to resist deformation against airflow and maintain adductory position can be improved by stiffening the body-layer stiffness or increasing the anterior-posterior tension of the vocal folds, which presumably can be achieved through the contraction of the thyroarytenoid (TA) and cricothyroid (CT) muscles, respectively. Experiments in b

BACKGROUND: Patients undergoing recurrent laryngeal nerve resection and reconstruction usually recover phonatory function within a year. However, their long-term outcomes remain unclear. This study assessed phonatory function over more than 10 years.
METHODS: Data of a cohort of 212 patients (165 female, 47 male) who underwent recurrent laryngeal nerve reconstruction between February 2005 and August 2022 were retrospectively analyzed. Maximum phonation time and mean flow rate were measured preoperatively and, periodically, postoperatively. The Voice Handicap Index-10 questionnaire was adminis…

Well-known multimass models of vocal folds are useful to describe main behavior observed in human voicing but their principle of functioning … amplitude is also controlled linearly by subglottal pressure from 0.22 to 0.26 mm/kPa. The range of phonation threshold pressure (PTP) is close to the values currently proposed, that is, 0.1 to 1 kPa and varies with the fundamental frequency. The relaxation oscillator is a simple and useful tool for modeling vocal fold vibration.

The authors describe the case history of a patient who suffered from symptoms deriving from two different origins. The patient's voice was spasmodic dysphonia-like interrupted and pressed. At the same time, his voice was powerless, too. The reason for this was that besides the spasmodic dysphonia caused by hyperkinesis, an incomplete closure of the vocal cords during phonation in the middle third was present. It was caused by the atrophy of the vocal cords. In order to eliminate the symptoms, initially we injected 25 IU Botox into the left vocal cord transcutaneously under the direction of EMG…

Complex simultaneous neuropsychophysiological mechanisms are responsible for the processing of the information to be transmitted and for the neuromotor planning of the articulatory organs involved in speech. The nature of this set of mechanisms is closely linked to the clinical state of the subject. Thus, for example, in populations with neurodevelopmental deficits, these underlying neuropsychophysiological procedures are deficient and determine their phonation. Most of these cases with neurodevelopmental deficits are due to a genetic abnormality, as is the case in the population with Smith-Ma…

INTRODUCTION: Individuals with idiopathic subglottic stenosis (SGS) are at risk for voice disorders prior to and following surgical management. This study examined the nature and severity of voice disorders in patients with SGS before and after a revised cricotracheal resection (CTR) procedure designed to minimize adverse effects on voice function.
METHOD: Eleven women with idiopathic SGS provided presurgical and postsurgical audio recordings. Voice Handicap Index (VHI) scores were also collected. Cepstral, signal-to-noise, periodicity, and fundamental frequency (F0 ) analyses were undertaken

OBJECTIVES: There are four types of signals that are typical representations of vocal fold vibratory patterns. Type 1 signals are nearly periodic, type 2 signals contain subharmonic properties, type 3 signals are chaotic, and type 4 signals are characterized as white noise. High-speed imaging allows detailed observation of these vocal fold vibratory patterns. Therefore, high-speed imaging can explore the vibratory mechanism behind each of the four types of signals.
METHODS: The glottal area time series of the four types of vocal fold vibrations were calculated from high-speed images of 10 exc

BACKGROUND: The precise comparison of the voice characteristics of Parkinson disease (PD) patients with age-matched normal subjects is still one of the important research projects. The present study aimed at comparing the voice characteristics in sustained phonations of PD patients with an age-matched control group.
METHODS: The subjects were 30 Japanese PD patients (15 males and 15 females). The control group consisted of 30 age-matched normal Japanese subjects (15 males and 15 females). Each subject was required to phonate into a mouthpiece attached to Vocal Function Analyzer (PS-77E; Nagas…

The effect of upper respiratory tract diseases on phonation has been reviewed, but little is known about the influence of lower respiratory tract diseases. In particular, the effect of asthma as a reversible obstructive small-airway disease on phonatory variables is not yet clear. We conducted a cross-sectional controlled study to evaluate the quality of phonation in a group of 34 adults with untreated mild to severe persistent asthma who were seen at the Ghaem Hospital in Mashhad, Iran. Patients with sinusitis, gastroesophageal reflux disease, or primary laryngeal disease were ineligible for …

Electroglottography is a common method for providing noninvasive measurements of glottal activity. The derivative of the electroglottographic signal, however, has not attracted much attention, although it yields reliable indicators of glottal closing instants. The purpose of this paper is to provide a guide to the usefulness of this signal. The main features that are to be found in this signal are presented on the basis of an extensive analysis of a database of items sung by 18 trained singers. Glottal opening and closing instants are related to peaks in the signal; the latter can be used to m
