Extracorporeal photochemotherapy for the treatment of refractory Crohn's disease.

Crohn's disease is an idiopathic, relapsing inflammatory disorder of the gastrointestinal tract. We prospectively evaluated the use of extracorporeal photochemotherapy (ECP; photopheresis), an immunomodulatory therapy, to treat moderate or severe Crohn's disease (Crohn's Disease Activity Index [CDAI] score > or = 220 points) in 2 patients who failed multiple treatments, including immunosuppressants and anti-tumor necrosis factor agents. After at least 24 weeks of ECP with stable concomitant therapy, the patient with moderate disease achieved a clinical response (a decrease in CDAI score from b…

Burns caused by accidental overdose of photochemotherapy (PUVA).

This study was aimed to alert the hazard of accidental adverse reactions of photochemotherapy (Psoralen-UVA or PUVA) that has been used in the treatment for some skin diseases and commercially for cosmetic tanning. Aside from the predictable side effects of PUVA such as erythema and itching, the accidental adverse reactions such as extensive burns could occasionally occur. Our observations indicated that six cases resulted from mistakes of medical personnel, and six other cases resulted from unsupervised mistakes of patients. The conditions that needed photochemotherapy were seven cases of vit…

[Treatment of organ graft rejection by extracorporeal photochemotherapy].

Extracorporeal photochemotherapy is a recently developed immunomodulator treatment against heart and lung graft rejection. The recipient's mononuclear cells are screened and irradiated with UV-A in the presence of 8-methoxy-psoralene (8-MOP), then reinjected. We treated 4 patients who had begun to reject their heart and/or lung grafts, with this technique, using one session per week for 4 weeks, followed by one session every 2 weeks for 2 months, then one session per month. In all cases, extracorporeal photochemotherapy appeared to improve the clinical and biological status of these patients. …

Applications of extracorporeal photochemotherapy in "non-oncological" diseases.

Photopheresis (ECP) is a new therapy for oncological and autoimmune diseases consisting in the reinfusion of 3-9 x 10(9) leukocytes, taken from the patient by leukapheresis, and treated in an extracorporeal system with 8-methoxypsoralen and ultraviolet light A. Nine patients affected by T cell immunomediated diseases (2 scleroderma, 1 chronic GVHD, 1 polyarteritis, 1 rheumatoid arthritis and 4 heart transplant patients with numerous episodes of acute rejection) were treated with ECP. Photopheresis was performed on 2 consecutive days every 3-4 weeks. All patients affected by autoimmune diseases

Recent advances in phototherapy and photochemotherapy of skin disease.

The therapeutic spectrum for ultraviolet radiation treatment of skin disease has continued to be broadened. Psoralen photochemotherapy is beneficial in chronic lichenoid graft-versus-host disease and disseminated granuloma annulare. This treatment is now being found more useful in atopic eczema and chronic photosensitivity with some modifications of the therapy. UV phototherapy has also been found useful in mild to moderate atopic eczema. The nature of these treatments is also changing with greater use of selective UV phototherapy and definition of the required schedule for maintenance treatme…

[Efficacy of photochemotherapy in a case of pityriasis rubra pilaris].

The therapeutic choice in treating Pityriasis Rubra Pilaris (PRP) is difficult, particularly in extensive erythrodermic forms and in those characterized by a strong tendency to recur. None of the therapeutic modalities till now employed have been proved to be resolutive. The curative potential of photochemotherapy in the treatment of PRP has been particularly uncertain. Here a case of recurrent PRP in an adult female subject is reported. The disease covered about 90% of the skin surface and was treated by means of photochemotherapy (total dose: 179.56 J/cm2). A complete remission of the diseas…

Photochemotherapy in mycosis fungoides. A study of 73 patients.

Seventy-three patients with mycosis fungoides were treated with photochemotherapy and followed up for 2 to 39 months. Photochemotherapy produced clinical and histological clearing in a very high proportion of patients with stage 0 to II disease. The response in patients with tumours was less satisfactory, and a high proportion of these patients required local radiotherapy to resistant lesions. Maintenance treatment with photochemotherapy may keep many patients with stage 0 to II disease in remission for up to 3 years, but patients with advanced disease may relapse despite maintenance treatment…

Photochemotherapy for mycosis fungoides: a clinical and histological study.

Nine patients with stages 2 or 3 mycosis fungoides (MF) and one with Sézary's syndrome were treated with oral methoxsalen and long-wave ultr … syndrome was inadequately controlled with systemic prednisone and cyclophosphamide, but addition of photochemotherapy improved the erythema and pruritus. This shows that photochemotherapy may have a place in MF treatment both alone and perhaps in conjunction with other treatments. An early relapse occurred on stopping photochemotherapy in four cases, but one patien …

Clinical results of photochemotherapy. The Cleveland Clinic experience.

PUVA therapy has proved to be beneficial for dermatologic conditions such as psoriasis, vitiligo, and mycosis fungoides. Attempts have been made to treat other diseases with the modality; five patients with pityriasis rubra pilaris were treated, with only a partial response noted in one. A poor to fair response was seen in treating a small group of patients with cystic acne, atopic dermatitis, and neurodermatitis. As might be expected, no response was seen in one patient with Darier's disease. At present, photochemotherapy must be viewed with cautious optimism. Strict adherence to a rational t…

Acral lesions of vitiligo: why are they resistant to photochemotherapy?

BACKGROUND: Acral lesions of vitiligo are usually resistant to conventional lines of treatment as well as surgical interventions. OBJECTIVE: To clarify causes underlying resistance of acral lesions to pigmentation in vitiligo by studying some of the factors associated with mechanisms of repigmentation following photochemotherapy. METHODS: The study included twenty patients with active vitiligo. Skin biopsies were taken from lesional and perilesional skin of areas expected to respond (trunk and proximal limb) and skin of acral areas, before and after PUVA therapy. Sections were stained with H…

Successful treatment of lichen myxoedematosus with PUVA photochemotherapy.

Lichen myxoedematosus is an unusual disorder of unknown etiology and pathogenesis. Although several treatments have reportedly been tried, therapeutic efficacies are variable and unsatisfactory. A patient with severe skin changes of this disease was successfully treated with PUVA photochemotherapy. PUVA therapy was carried out using 30 mg of 8-methoxypsoralen orally, and subsequent exposure to UVA starting at 4 J/cm2. The eruption disappeared almost completely after 35 treatments at a cumulative dose of 202 J/cm2. Histologically, mucin deposition was greatly diminished after the therapy. The p…

The benefits and risks of long-term PUVA photochemotherapy.

In 1974 a new photobiologic principle i.e. light + drug, called photochemotherapy was discovered in Boston and immediately confirmed in Vienna. Psoralen + UVA (PUVA) photochemotherapy has now been applied to the treatment of more than 24 heterogeneous groups of diseases, especially psoriasis and mycosis fungoides. After 24 years of experience in thousands of patients with psoriasis and 23 other skin disorders, virtually the only risk is the development of squamous-cell carcinomas. This risk is low with two exceptions: previous history of treatment with ionizing radiation or inorganic trivalent…

Extracorporeal photochemotherapy in systemic sclerosis and severe morphea.

BACKGROUND: Extracorporeal photochemotherapy (EP) is a new immune-modulating therapy which combines cytapheresis and extracorporeal irradiation of leukocytes by UVA. Recently, patients with systemic sclerosis (SS) have been treated by this method, but the efficacy of EP is still controversial. OBJECTIVE: Our purpose was to evaluate the efficacy and safety of EP in SS and in severe morphea. METHODS: In an open study, 7 patients with SS and 2 patients with severe morphea were treated by EP and were evaluated after 6 months of treatment. No other treatments were allowed during this period. RES…

Photochemotherapy of oral lichen planus. A controlled study.

Photochemotherapy with 8-methoxypsoralen and long-wave ultraviolet light (PUVA) has become a useful alternative in dermatologic therapy. PUVA therapy has been successfully used in the treatment of severe psoriasis and cutaneous lichen planus. The aim of this investigation was to use PUVA in the treatment of oral lichen planus (OLP). Eighteen patients with long-standing, bilateral, and severe OLP of the buccal mucosa participated in the investigation. A dose of 0.6 mg/kg 8-methoxypsoralen was administered orally 2 hours before long-wave ultraviolet light irradiation was done. The patients were …

Vitiligo-like leucoderma during photochemotherapy for mycosis fungoides.

We describe four patients with mycosis fungoides (MF) in whom depigmentation, and also leucotrichia in one, occurred following the resolution of the eruption during phototherapy (psoralen plus ultraviolet A treatment in three patients, climatotherapy in one). In all cases, the depigmentation was localized to the area of the pre-existing MF lesions. There was no clinically obvious phototoxicity. Biopsy study including S100 staining in all cases, and electron microscopy in one case, demonstrated the total absence of melanocytes, with no evidence of MF. It is suggested that the phototherapy may h

[PUVA-bath photochemotherapy in Hallopeau's acrodermatitis continua suppurativa].

A 73-year-old man presented with severe, relapsing acrodermatitis continua of Hallopeau, which had been resistant to prior local and systemic therapy for eight years. The patient was treated with selective hand PUVA-bath photochemotherapy. The cumulative dose of UVA used over 10 weeks of treatment was 54.6 J/cm2 on the palms and 26.8 J/cm2 on the dorsum of the hands. The single UVA doses ranged from 0.5 to 2.5 J/cm2 on the palms and 0.2 to 1.4 J/cm2 on the dorsum of the hands. After 16 treatment sessions, the acrodermatitis continua started to improve, and after 24 treatments, had cleared comp…
