Acetaminophen analgesia in children: placebo effect and pain resolution after tonsillectomy.

BACKGROUND: Pharmacodynamic models of acetaminophen analgesia in children have not explored the efficacy of single oral doses greater than 40 mg/kg. METHODS: Children aged 9.0 +/- 3.0 years (+/- SD) and weight 37.9+/- 16.6 kg undergoing outpatient tonsillectomy were randomised to receive acetaminophen elixir 40 mg/kg (n = 12). high dose acetaminophen elixir 100 mg/kg (n =20) or placebo (n=30) 0.5 -1 h preoperatively. No other analgesics were given. Individual acetaminophen serum concentrations and pain scores [visual analogue scale (VAS) 0-10] were measured over a 4-8 h postoperative period.

Effect of prazosin vs placebo on chronic left ventricular heart failure.

The effect of the vasodilator prazosin vs placebo on exercise duration until marked dyspnea, and on left ventricular function measured by echocardiography, was evaluated in a double-blind, randomized study in 24 patients with chronic left ventricular failure despite digitalis and diuretic therapy. Compared with the double-blind placebo, prazosin reduced resting systolic and diastolic blood pressure and systolic blood pressure times heart rate, improved clinical symptoms, decreased cardiothoracic ratio measured by chest roentgenography, decreased left ventricular and left atrial dimensions, imp

The Randomization Effect in a Double-Blind Placebo-Controlled Depression Study.

BACKGROUND: Double-blind, placebo-controlled trials are often affected by higher-than-anticipated placebo responses that may contribute to study failure. It is common to observe symptomatic improvement immediately after randomization regardless of treatment allocation. METHODS: This post hoc analysis examined the early randomization effect in a recent 4-week double-blind, placebo-controlled study that examined a novel presynaptic mGluR2 inhibitor, MK-1942, in a daily or twice-weekly dosing strategy versus placebo as an adjunctive treatment in acutely depressed participants with major depressi

Effect of terfenadine and placebo on symptoms after nasal allergen provocation.

Terfenadine (60 mg b.i.d.) was compared with placebo in a randomized, double-blind, cross-over study in twenty-three patients with birch pollen allergy. The patients were treated during two 7-day periods separated by a 2-week wash-out period during a season free from birch pollen. Nasal provocations with birch pollen extracts were made at study entry and after each treatment period. Blockage (rhinomanometry), secretion (weight) and sneezings (number) were compared between treatments. The results showed a significant inhibitory effect of terfenadine on secretion and sneezing, but no significant

Placebo effect in surgery for Meniere's disease: three-year follow-up.

In 1981 we published the results of a double-blind, placebo-controlled study in which the efficacy of a regular endolymphatic sac mastoid shunt was compared with a purely placebo operation (mastoidectomy) in controlling the symptoms of 30 patients with typical Meniere's disease. Minor differences could be demonstrated between the shunt and the sham operation, but the greatest difference was between the preoperative and postoperative scores, and both groups improved significantly. It was concluded that the impact of the various endolymphatic sac shunts on the symptoms of patients with Meniere's

Nocebo is the enemy, not placebo. A meta-analysis of reported side effects after placebo treatment in headaches.

The aim was to determine the magnitude of the nocebo (adverse effects following placebo administration) in clinical trials for primary headache disorders. We reviewed randomized, placebo-controlled studies for migraine, tension-type headache (TTH), and cluster headache treatments published between 1998 and 2009. The frequency of nocebo was estimated by the percentage of placebo-treated patients reporting at least one adverse side effect. The dropout frequency was estimated by the percentage of placebo-treated patients who discontinued the treatment due to intolerance. In studies of symptomatic

Direct comparison of nocebo, deceptive placebo and open-label placebo effects on sustained visual attention.

Positive and negative expectations crucially shape how we perceive our surroundings, including our emotional and cognitive abilities. Prior work shows that cognitive abilities in different domains can be modulated by placebo or nocebo effects, in clinical cohorts, but also in the absence of disease. However, these studies show inconclusive results and do not directly compare different expectation modulations with each other. This preregistered online study tested the effects of three types of expectations on cognitive performance in a visual attention task, using a mixed design. Participants (

Impact of active placebo controls on estimated drug effects in randomised trials: a systematic review of trials with both active placebo and standard placebo.

BACKGROUND: An estimated 60% of pharmacological randomised trials use placebo control interventions to blind (i.e. mask) participants. However, standard placebos do not control for perceptible non-therapeutic effects (i.e. side effects) of the experimental drug, which may unblind participants. Trials rarely use active placebo controls, which contain pharmacological compounds designed to mimic the non-therapeutic experimental drug effects in order to reduce the risk of unblinding. A relevant improvement in the estimated effects of active placebo compared with standard placebo would imply that t

Oral tetracyclines may not be effective in treating acne: dominance of the placebo effect.

OBJECTIVE: Bisphosphonates are chemically stable analogs of pyrophosphate compounds, which have been used to treat multiple disorders of calcium metabolism. Although bisphosphonates have been employed for many years and have demonstrated an excellent safety profile, severe osteonecrosis of the jaw (ONJ) has been described in patients with bone metastases who have been treated with bisphosphonates. METHODS: In this review we describe the reasons for ONJ and discuss the varying effects of different bisphosphonates on the development of ONJ. Bisphosphonates tend to accumulate in bone, subject to

Modelling and simulation of placebo effect: application to drug development in schizophrenia.

High and variable placebo effect (PE) within and among clinical trials can substantially affect conclusions about the efficacy of new drugs in the treatment of schizophrenia and other neuropsychiatric disorders. In recent years, it has become increasingly difficult to prove drug efficacy against placebo, and one of the reasons is that the placebo response has increased over recent years. The increased placebo response over the years is partly explained by unidentified parallel interventions, patient factors, issues with trial designs, and regional variability or demographic differences. In add

An estimate of placebo effect of repetitive transcranial magnetic stimulation in epilepsy.

OBJECTIVE: Low-frequency repetitive transcranial magnetic stimulation (rTMS) is emerging as a therapeutic tool in epilepsy. In recent years, several open-label trials have shown an encouraging reduction in seizure frequency in patients with epilepsy. However, the data from controlled trials are mixed with respect to antiepileptic rTMS efficacy, and the field would benefit from further carefully controlled trials. Prior to initiating new trials, it is important assess the magnitude of the placebo effect of presently used sham rTMS methods. METHODS: We systematically analyzed individual subject

Effect of acyclovir on infectious mononucleosis: a double-blind, placebo-controlled study.

Thirty-one patients with clinical and laboratory diagnoses of infectious mononucleosis who had had symptoms for seven or fewer days were randomized for intravenous treatment with acyclovir (10 mg/kg) or placebo at 8-hr intervals for seven days in a double-blind trial. Clinical signs and symptoms were registered, and excretion of virus in the saliva as well as antibody responses in sera and saliva were assessed before, during, and at regular intervals in the six months after treatment. Acyclovir significantly (P less than .001), but reversibly, inhibited oropharyngeal shedding of Epstein-Barr v

Effect of terbutaline on premature labor. A double-blind placebo-controlled study.

The effect of terbutaline, a "selective" beta2-receptor stimulator, was compared with that of placebo in 30 patients in premature labor. Treatment consisted of an intravenous infusion for at least 8 hours, and then of subcutaneous injections four times daily for 3 days together with peroral treatment, which was continued until the end of week 36 of pregnancy. In 12 of 15 terbutaline-treated patients (80 per cent) premature labor was arrested beyond the treatment period, compared with three of 15 in the placebo group (20 per cent). This difference is statistically significant (p less than 0.01)

A Comprehensive Scientometric Analysis of Placebo and Nocebo Effects in Pain Research: From Placebo Mechanisms to Clinical Trials.

Placebo and nocebo phenomena are central in neuroscience, psychology, and clinical research, yet comprehensive bibliometric analyses remain limited. This study mapped placebo and nocebo research in pain, distinguishing mechanistic studies from randomised controlled trials (RCTs), using data from the Web of Science Core Collection up to March 2025, which yielded 1246 mechanistic studies and 35,134 RCTs. Analyses performed with the Bibliometrix R package included descriptive statistics, co-citation and keyword co-occurrence networks, and local citations to assess internal impact. Mechanistic stu

The neurobiology of placebo effects in sports: EEG frontal alpha asymmetry increases in response to a placebo ergogenic aid.

The performance enhancing (ergogenic) placebo effect is elicited by an inert treatment and caused by positive affective appraisal of effort perception. Frontal alpha asymmetry (FAA) is a neurobiological correlate of positive affect. This study investigates, whether receiving an ergogenic placebo increases FAA and whether scores on the behavioral inhibition and activation system (BIS/BAS) scales affect this increase in FAA. Nineteen competitive male cyclists (37.26 ± 9.82 years) performed two maximum effort time trials. The first served as baseline for the second intervention time trial, where

Placebo Effect in Levodopa-Induced Dyskinesia: A Systematic Review and Meta-Analysis.

BACKGROUND: Levodopa-induced dyskinesia (LID) is a complication of Parkinson's disease (PD) treatment. While the placebo effect is a known component in PD clinical trials, its specific magnitude in the context of LID is not well characterized. OBJECTIVE: To quantify the magnitude of the placebo response in LID trials and to identify potential moderators of this effect. METHODS: We conducted a systematic review and meta-analysis of double-blind, placebo-controlled trials in patients with PD and LID. The primary outcome was the change in dyskinesia severity in the placebo arm, pooled using ran
