Two cases of adult-onset idiopathic thrombocytopenic purpura (ITP), refractory to conventional therapy, were treated. Because of recent successful reports of plasma exchange in ITP, this technique was performed on each patient. In both cases, the therapy was unsuccessful in controlling their disease, suggesting that in refractory adult-onset ITP, plasma exchange may not be an effective mode of therapy.

To determine the potential steroid sparing effect of plasma exchange in pemphigus we enrolled 40 patients in a multicenter randomized study. Thrombotic thrombocytopenic purpura (TTP) is a life-threatening disease related to the formation of microvascular thrombosis and subsequent organ failure. The disease is accompanied with microangiopathic haemolytic anaemia, consumptive thrombocytopenia and lies on a severe deficiency in ADAMTS13, the von Willebrand factor-cleaving protease.
