Popliteal cysts, although commonly seen, are rarely associated with motion restriction or calcification. Radiological features are of soft-tissue swelling, with occasional reports of calcifications or small osteochondral bodies inside the cysts. We report a giant osteochondral body in a popliteal cyst, with significant mechanical block to flexion. This type of mass has to be differentiated from synovial osteochondromatosis, calcifications in the cyst, extraosseous and intraarticular osteochondromas. Complete excision of the cyst resulted in complete recovery of range of motion.

Fifteen patients presented during a four-year period with a pseudothrombophlebitis syndrome, subsequently confirmed arthrographically as being due to popliteal cyst rupture. Initially the majority of patients (73%) were erroneously diagnosed as having calf vein thrombosis, and were anticoagulated for periods of three to ten days. Clinical differentiation from venous thrombosis was usually impossible. Contrast arthrography was the definitive diagnostic investigation, revealing popliteal cysts in all patients, and an active synovial leak into the calf in 13 cases (87%). The mean delay in perform…

The popliteal cyst is a common condition that causes disturbing symptoms. Several arthroscopic techniques are combined to treat the enlargement of communication and decompose the cyst wall, aiming to reduce recurrence. We propose a technique that involves resecting the posteromedial valve mechanism and excising the cyst wall through a 2-posteromedial portal approach. This technique is particularly helpful for visualization and facilitates complete resection of the cyst wall and capsular fold, even in large cysts, with low complication rates.

A 71-year-old man with rheumatoid arthritis complained of pain and swelling in the left calf. X-ray examination of the calf disclosed radiolucencies suggestive of soft tissue gas; however, subsequent investigation revealed no evidence of infection and an arthrogram showed a dissected popliteal cyst in the area of the gas. Gas-like radiolucencies in a popliteal cyst are an unusual finding which has not been previously reported.

When treating a popliteal cyst in the knee joint, it is often necessary to simultaneously treat the intra-articular lesions so as to reduce the recurrence of the cyst after the operation. Herein, we propose an arthroscopic resection technique for popliteal cyst, in which the position of the popliteal cyst is indirectly located by finding out the location of the semimembranosus tendon during the operation, thereby completing the arthroscopic resection of the popliteal cyst. This method does not involve the posteromedial compartment of the knee joint. Arthroscopic treatment can be performed on t…

In children and adults, a popliteal cyst frequently coexists with an intra-articular disorder of the knee. Its mode of presentation consists of a palpable mass at the level of the popliteal fossa. The cyst is a recess of the synovial cavity of the knee, often associated to a chronic effusion. In children, the etiology may be primary with a development directly from the medial gastrocnemius-semimembranous bursa. In adults, it is usually secondary to degenerative or inflammatory disease of the knee. A communication between the intraarticular space and the cystic cavity can be often found. The ai…

BACKGROUND: This prospective study was designed to investigate the aetiology of popliteal (Baker's) cysts.
METHODS: Some 426 legs in 312 individuals were investigated with ultrasonography for the presence of a popliteal cyst. Participants included healthy volunteers, patients with a painful knee, and a group of patients with signs and symptoms of deep vein thrombosis (DVT). The location, thickness and content of all cysts were recorded. In addition, the popliteal fossa was dissected in four cadaver knees.
RESULTS: The prevalence of popliteal cyst increased with age and was significantly high…

The value of ultrasound in popliteal cyst diagnostics and treatment has been presented. Five and 7.5 MHz real time scanners were used in comparative clinical studies on 70 patients. High correlation of preparative ultrasound diagnoses and the intraoperative findings was established. We believe, that ultrasound as a non-unvasuve method will claim a growing importance in the diagnosis of popliteal cyst.

Ultrasonography of the knee joints in 55 patients with rheumatoid arthritis permitted diagnosing of popliteal cysts in 14 of them including 7 patients without any clinical symptoms. The method permitted measurements of cyst size and assessment of cysts before and after therapy. Ultrasonic B-scanning is a non-invasive, informative and reliable technique for the detection and assessment of popliteal cysts.

The computed tomographic (CT) findings of two popliteal cysts are presented, and the correlated anatomico-pathologic changes are discussed. The characteristic findings include thin, well-defined cyst wall, fluid density contents, central septum or septi, and medial popliteal location with mediocaudal extension. These findings, however, may vary due to herniation or rupture of the synovial membrane, gelatinous cyst contents, or metaplastic change of the cyst wall. Differential diagnoses include liposarcoma, popliteal aneurysm or hematoma, thrombophlebitis, xanthoma, fibrosarcoma, or other soft …
