[Pressure topography of the portal vein system and its clinical implications].

Pressure topography of the portal vein system was investigated in 10 healthy dogs and 26 patients with portal hypertension. The pressure in all of the tributaries of the portal vein was significantly higher than that of the portal trunk in healthy dogs, with the pressure of left gastric vein being the highest. The normal pressure gradient and the hepatopetal blood flow were evidently shown. In case of portal hypertension, the pathophysiologic pressure topography of the portal venenous system was miscellaneous with the pressure of left gastric vein being lower than that of the portal trunk in 4

MR angiography and dynamic flow evaluation of the portal venous system.

We studied the value of MR angiographic techniques in imaging the portal venous system. Projection angiograms were created by postprocessing a series of two-dimensional, flow-compensated gradient-echo images. Flow velocity was determined by a bolus-tracking method with radiofrequency tagging and multiple data readout periods. Each image was acquired during a breath-hold. MR angiography was applied to six normal subjects and four patients with abnormal hemodynamics in the portal venous system. Flow velocity determined by MR was correlated with the results of duplex sonography. The main portal v

Clinical Applications of 4D Flow MRI in the Portal Venous System.

Evaluation of the hemodynamics in the portal venous system plays an essential role in many hepatic pathologies. Changes in portal flow and vessel morphology are often indicative of disease.Routinely used imaging modalities, such as CT, ultrasound, invasive angiography, and MRI, often focus on either hemodynamics or anatomical imaging. In contrast, 4D flow MRI facilitiates a more comprehensive understanding of pathophysiological mechanisms by simultaneously and noninvasively acquiring time-resolved flow and anatomical information in a 3D imaging volume.Though promising, 4D flow MRI in the porta

The classification based on intrahepatic portal system for congenital portosystemic shunts.

BACKGROUND/PURPOSE: Liver transplantation was previously indicated as a curative operation for congenital absence of portal vein. Recent advances in radiological interventional techniques can precisely visualize the architecture of the intrahepatic portal system (IHPS). Therefore, the therapeutic approach for congenital portosystemic shunt (CPS) needs to be reevaluated from a viewpoint of radiological appearances. The aim of this study was to propose the IHPS classification which could explain the pathophysiological characteristics and play a complementary role of a therapeutic approach and ma…

Gonadotropin-inhibitory hormone (GnIH) secretion into the ovine hypophyseal portal system.

GnIH was first identified in avian species, and there is now strong evidence that it is operant in mammals as an inhibitor of reproduction. Mammalian gonadotropin-inhibitory hormone (GnIH)-3 is encoded by the RFRP gene in neurons of the dorsomedial nucleus. These neurons project to the median eminence, predicting a role as a secreted neurohormone and regulation of the pituitary gonadotropes. To determine whether GnIH-3 is a secreted neurohormone, we measured its concentration in hypophyseal portal blood in ewes during the nonbreeding (anestrous) season and during the luteal and follicular phas

The effects of exercise on portal venous system in splenectomized adults.

OBJECTIVES: This study was designed to assess the effects of exercise on the portal venous system in splenectomized adults. BACKGROUND: Splenectomy is a surgical intervention commonly performed at surgery clinics and recognized as a cause of portal vein thrombosis. Intensive exercise increases blood flow to the contracting muscles, causes hypercoagulability and vasoconstriction in the splanchnic area, hence the portal vein diameter, blood flow velocity and discharge decrease. METHODS: Forty adults participated in this investigation. Subjects were trained to run for twenty minutes on a treadm

Establishing a popliteal portal using the METRx system under ultrasound guidance.

This report describes a special technique for safe establishment of a popliteal portal with the METRx system (Medtronic Sofamor Danek, Memphis, TN) under ultrasound guidance with the patient in the awake state. Herbert screws located at the posterior tibial plateau were removed via this portal in a minimally invasive way. Before surgery, we performed surface mapping of the screw tract with an image intensifier and target needling under ultrasound guidance. The METRx system was used to establish the soft-tissue corridor along the guide pin. Screw removal and tibial plateau resurfacing were perf

Magnetic resonance imaging in the evaluation of the portal venous system.

Magnetic resonance (MR) imaging is used in assessing the portal venous system through qualitative and quantitative methods. Magnetic resonance angiography can be performed using time-of-flight or phase-contrast techniques. Time-of-flight techniques (which use gradient echo images to display bright blood or spin echo images to display black blood) are relatively standardized and commercially available. These techniques are used to display liver morphology, portal vein patency, portal venous collaterals, and surgically created portosystemic shunts. Magnetic resonance is equivalent to angiography

[Portal system obstruction of delayed onset following neonatal Staphylococcus aureus infection].

Two neonates underwent staphylococcal septicemia with multiple intrahepatic abscesses, following umbilical catheterization in one case and due to superinfected heel puncture in the other case. At the first examination, liver ultrasonography showed multiple hypoechogenic areas and assessed patency of the portal vein flow. In spite of clinical, biological and sonographic recovery within the following weeks, portal hypertension due to seemingly late portal vein thrombosis occurred. These clinical reports indicate the need for a protracted echosonographic supervision of neonatal intrahepatic absce

The effect of decreased portal blood flow on the biliary system.

A disturbance in the regional portal blood flow adjacent to the dilated bile duct in the liver is often observed in patients with hepatolithiasis. The effects of this disturbed portal blood flow on the biliary system, with or without cholangitis, were therefore investigated. Young rabbits were divided into the following four groups; (1) controls that had a laparotomy only (n = 3), (2) those that had a ligation of the portal branch of the right posterior lobe (RP lobe) (PL) (n = 10), (3) those that had tubing inserted into the bile duct through the duodenal papilla (BS) (n = 10), and (4) PL + B

Anomalies of the intrahepatic portal venous system in congenital hepatic fibrosis.

Duplication of intrahepatic venous channels was observed in 9 of 13 children with congenital hepatic fibrosis and portal hypertension. Portal hypertension due to other causes was not associated with venous duplication. This anomaly may be a distinctive feature of congenital hepatic fibrosis with portal hypertension.

Portal systemic encephalopathy presenting with dressing and constructional apraxia.

We report a case with portal systemic encephalopathy who presented with dressing and constructional apraxia and subtle weakness of the left hand. We initially suspected a cerebrovascular attack in the right cerebral hemisphere, but brain T1-weighted magnetic resonance (MR) imaging revealed high intensity in the basal ganglia and hyperammonemia was detected. We performed abdominal MR angiography, which visualized an intrahepatic portal systemic shunt. Cerebral blood flow, measured by xenon-enhanced computed tomography, was decreased in the bilateral, but more dominantly right-sided, parietal wa…

Intractable ascites in systemic mastocytosis treated by portal diversion.

A 50-year-old male presented with intractable ascites due to systemic mastocytosis. The diagnosis of systemic mastocytosis was established by histology of the bone marrow which showed mast cell infiltration and fibrosis. Ascites was related to portal hypertension which was documented by esophageal varices at endoscopy and by an increase of wedged-free hepatic venous pressure gradient. Liver biopsy disclosed dense fibrosis of hepatic arterial and portal venule walls, resulting in complete obstruction of some portal radicles. Peliosis hepatis and fibrous deposits in the walls of hepatic venules

Idiopathic portal hypertension associated with progressive systemic sclerosis.

Herein we describe the clinical course of a case of idiopathic portal hypertension accompanied by splenomegaly, portal hypertension, and pancytopenia in addition to progressive systemic sclerosis. Immunological studies revealed positive antinuclear antibodies, hypergammaglobulinemia with increases in IgG and IgA, a decrease in peripheral T lymphocytes, and a decrease in the rate of lymphocyte blast transformation induced with phytohemagglutinin. The spleen weighed 1070 g, and the portal pressure was 270 nm H2O. Esophageal varices were also noted. These findings suggest the possibility of assoc

Systemic venous blood return via the portal vein.

The patterns of collateral circulation which develop following obstruction of the inferior vena cava have been classically divided into three groups depending upon the level of obstruction, i.e., infrarenal, middle caval, and upper caval. The portal vein can play an active role at all three levels. Multiple potential routes to the portal vein are described and 6 cases are presented.

Massive spontaneous portal-systemic shunting without varices.

Using umbilical vein portal phlebography, a group of patients, often with previously unsuspected liver disease or portal hypertension, have been identified with massive spontaneous portal-systemic shunts. In all cases, the collateral circulation was a single large vessel. The splenoadrenorenal, umbilical vein, or inferior mesenteric vein routes were the common pathways. These natural shunts were functional and effective in the alleviation of portal hypertension without evidence of esophageal varices or other collaterals. Portal flow was usually retrograde, perhaps contributing to the small atr
