Intermittent mandatory ventilation (IMV): a primary ventilatory support mode.

Respiratory therapy should be directed at underlying pathophysiology, not symptomatology. Mechanical ventilation, oxygen, and CPAP should be administered to patients independently and in appropriate amounts. Removal of each of these therapeutic interventions should occur in a similar fashion. The method for determining optimal mechanical ventilation, oxygen concentration, and CPAP level is not unlike that recommended for many other therapeutic interventions. Each should be applied to achieve a predetermined goal, each should be continually reevaluated, and each should be withdrawn when indicat

Role of hyaline membrane disease in production of later childhood lung abnormalities.

Between 1961 and 1970, 177 survivors of hyaline membrane disease have been followed clinically and roentgenologically for a minimum of three years to determine the long-term pulmonary outcome. Of these, 153 have been seen for at least six years and 61, for 11 years or more. A total of 175 batteries of pulmonary function tests have been performed on 129 children at age 7 and/or 11 years, Final roentgenograms showed fibrosis in 12 instances, and these changes were positively correlated with the severity of the initial disease, the use of a positive pressure respirator (but not negative pressure

Mechanical Ventilation in Patients with Acute Brain Injuries: A Pathophysiology-based Approach.

Applying mechanical ventilation and selecting ventilatory strategies in patients with acute brain injuries, especially those with lung damage, is challenging. Static (positive end-expiratory pressure) and dynamic (intratidal) changes in ventilator pressure, via complex pathways, influence cerebral arterial inflow and cerebral venous pressure and thus cerebral blood volume and intracranial pressure. In this process, the relationship between airway pressure and pleural and transalveolar pressures, heavily affected by elastance of the chest wall and lung, respectively, plays a central role. This

Changes in arterial pressure during mechanical ventilation.

Mechanical ventilation induces cyclic changes in vena cava blood flow, pulmonary artery blood flow, and aortic blood flow. At the bedside, respiratory changes in aortic blood flow are reflected by "swings" in blood pressure whose magnitude is highly dependent on volume status. During the past few years, many studies have demonstrated that arterial pressure variation is neither an indicator of blood volume nor a marker of cardiac preload but a predictor of fluid responsiveness. That is, these studies have demonstrated the value of this physical sign in answering one of the most common clinical

In vivo calibration of esophageal pressure in the mechanically ventilated patient makes measurements reliable.

BACKGROUND: Esophageal pressure (Pes) can provide information to guide mechanical ventilation in acute respiratory failure. However, both relative changes and absolute values of Pes can be affected by inappropriate filling of the esophageal balloon and by the elastance of the esophagus wall. We evaluated the feasibility and effectiveness of a calibration procedure consisting in optimization of balloon filling and subtraction of the pressure generated by the esophagus wall (Pew). METHODS: An esophageal balloon was progressively filled in 36 patients under controlled mechanical ventilation. VBE

Altering the mechanical scenario to decrease the driving pressure.

Ventilator settings resulting in decreased driving pressure (ΔP) are positively associated with survival. How to further foster the potential beneficial mediator effect of a reduced ΔP? One possibility is promoting the active modification of the lung's "mechanical scenario" by means of lung recruitment and positive end-expiratory pressure selection. By taking into account the individual distribution of the threshold-opening airway pressures to achieve maximal recruitment, a redistribution of the tidal volume from overdistended to newly recruited lung occurs. The resulting more homogeneous dist

[Surgical treatment of the unstable thorax in respiratory insufficiency].

Sixteen patients with traumatic flail chest underwent surgical stabilization using procedures introduced by Rehbein, Judet and Vecsei. Compared with sixteen patients treated by internal stabilization with intermittent positive pressure respiration, there are some advantages: time of artificial respiration is shortened, early mobilization is possible, nursing is easier, the costs are lower. The procedure is indicated in those patients, in whom long-term artificial respiration is not necessary for other reasons.

[Surgical treatment of the flail chest (author's transl)].

Six patients with traumatic flail chest underwent surgical stabilization using a procedure introduced by Brunner, Hoffmeister and Koncz (2). Compared with internal stabilization by intermittend positive pressure respiration (IPPR), there are some advantages: Time of artificial respiration is shortened, early mobilization is possible, nursing is easier. The procedure is simple to do and is indicated in those patients, in whom longterm artificial respiration is not necessary for other reasons.

Non-invasive positive pressure ventilation for acute respiratory failure: justified or just hot air?

Non-invasive positive pressure ventilation (NIV) is the provision of mechanical positive airway pressure ventilatory support through the patient's upper airway through mask interface. Conditions in which it has been shown to be effective are acute cardiogenic pulmonary oedema and acute hypercapnic exacerbations of chronic obstructive pulmonary disease. In such conditions, NIV is associated with reduced intensive care unit demands, a reduction in intubation rates, reduced health-care expenditure and improved survival. Other conditions, such as hypercapnia of other cause, hypoxaemic respiratory

A new continuous positive airway pressure (CPAP) device.

While continuous positive airway pressure (CPAP) is being increasingly employed for many forms of acute respiratory failure and postoperative hypoxaemia, most CPAP systems are either 'home made' or incorporated in sophisticated ventilators used in intensive care units. This paper describes the development of a continuous low flow CPAP system using a large latex reservoir bag and venturi generated positive end expiratory pressure (PEEP). The system has been successfully used for over twelve months in the general wards, the intensive care unit, accident and emergency department, the recovery war

Prone to Success: Strategies to Improve Outcomes in Adult Respiratory Failure Patients.

Acute hypoxemic respiratory failure and acute respiratory distress syndrome (ARDS) are life-threatening conditions marked by impaired gas exchange due to ventilation-perfusion mismatch and intrapulmonary shunting, often from inflammatory alveolar-capillary injury. Recognizing ARDS and initiating lung-protective strategies, such as low tidal volume ventilation, individualized positive end-expiratory pressure, and prone positioning are crucial. Prone positioning improves oxygenation and outcomes in intubated patients and may benefit select awake patients when used early with close monitoring. No

Noninvasive Ventilator Devices and Modes.

Noninvasive ventilation has become an increasingly common treatment strategy for patients with diverse conditions involving chronic respiratory failure. An intimate understanding of various advanced respiratory devices and modes is essential in the management of these patients. Pressure-limited modes of ventilation are more commonly used than volume modes for noninvasive ventilation owing to enhanced patient comfort and synchrony with the ventilator, as well as improved leak compensation. Common pressure modes include spontaneous/timed and pressure control, with volume-assured pressure support

Noninvasive positive pressure ventilation: Increasing use in acute care.

In the past 2 decades, noninvasive positive pressure ventilation (NIPPV) has been increasingly used in acute respiratory failure to avoid the risks associated with intubation. It is now considered standard first-line therapy in several situations. In this review, we summarize how NIPPV has evolved, the current level of evidence that supports its use in various clinical situations, its potential contraindications, and its limitations in acute respiratory failure.

The role of the expiratory phase in obstructive sleep apnoea.

The role of the expiratory phase in obstructive sleep apnoea (OSA) is not well known. The aim of our study was to verify the contribution of expiratory narrowing to apnoea in a group of OSA patients by evaluating the effects of short-term treatment with continuous positive airway pressure (CPAP), intermittent positive pressure ventilation (IPPV) and bi-level positive airway pressure (BIPAP). We studied a selected group of 10 OSA patients whose therapeutic pressure level of CPAP was at least 10 cm H2O. During CPAP therapy the mean apnoea/hypopnoea index (AHI) and oxyhaemoglobin desaturation ind

A Comparison of Proximal and Tracheal Airway Pressures During Pressure Controlled Ventilation.

BACKGROUND: Airway pressure is usually measured by sensors placed in the ventilator or on the ventilator side of the endotracheal tube (ETT), at the Y-piece. These remote measurements serve as a surrogate for the tracheal or alveolar pressure. Tracheal pressure can only be predicted correctly by using a model that incorporates the pressure at the remote location, the flow through the ETT, and the resistance of the ETT if the latter is a predictable function of Y-piece flow. However, this is not consistently appropriate, and accuracy of prediction is hampered. METHODS: This in vitro study syst
