The increased use of herbal therapy and other dietary supplements in the past decade has increased the risk of complications for patients undergoing surgery. The use of herbal products is projected to rise in the United States. Therefore, it is important for the perianesthesia nurse to be aware of the herbal products that patients are taking and the possible anesthesia and/or surgical complications related to herbal products. Current resources are reviewed in this article to increase awareness for the perianesthesia nurse. Recommendations for establishing local protocols are given, and patient

This study examines factors influencing the length of intensive care unit stay for patients after coronary artery bypass surgery. Profiles of patients with selected lengths of ICU stay were identified for Group 1 (< or =1 day) and Group 2 (> or =2 days). Medical records of 175 patients who had undergone this procedure at an urban teaching hospital were reviewed. Patients who had a 1-day ICU length of stay were younger (mean=62.39, SD=10.88) and had comorbidities such as hypertension. Those patients with an ICU length of stay 2 days or longer were older (mean=68.18, SD=11.84) and had preoperati

BACKGROUND: Postoperative nausea and vomiting (PONV) affects 30% of surgical patients undergoing anesthesia. PONV is a substantial cause of increased length of stay, cost of care, and unanticipated patient admission and readmission.
LOCAL PROBLEM: Seventy percent of patients received PONV prophylaxis; PONV assessment was performed in only 63% of patients in this hospital system.
METHODS: A standardized PONV preoperative assessment checklist and a best practice advisory (BPA) were implemented in our electronic medical record (EMR) and charting system. Anesthesia providers and postanesthesia c

The National Institute for Occupational Safety and Health (NIOSH) recently highlighted the presence of waste anesthetic gases (WAGs) in Postanesthesia Care Units (PACUs), raising significant concerns about occupational exposures in these environments. Exposure to WAGs is associated with a range of adverse health effects, from symptoms such as headaches and0 fatigue to more severe outcomes, including cancer, genetic mutations, reduced fertility, miscarriages, and congenital anomalies. The primary source of WAGs in PACUs is patient exhalation, as individuals leave the operating rooms (ORs) with

BACKGROUND: A multidisciplinary effort was undertaken to determine whether patients could safely bypass the postanesthesia care unit (PACU) after same-day surgery by moving to an earlier time point evaluation of recovery criteria.
METHODS: A prospective, outcomes research study with a baseline month, an intervention month, and a follow-up month was designed. Five surgical centers (three community-based hospitals and two freestanding ambulatory surgical centers) were utilized. Two thousand five hundred eight patients were involved in the baseline period, and 2,354 were involved in the follow-u

BACKGROUND: Supracondylar fractures of the humerus (SCH) are the most common fractures about the elbow in children that require operative stabilization. Considering the possible complications involved including nerve deficit and compartment syndrome, documentation is crucial to good patient care. It also is of prime importance for justification or defense of our care should this arise. One of the common concerns in transition from written documentation to an electronic medical record (EMR) is availability of proper documentation. We sought to develop an established EMR protocol to streamline a

PURPOSE: Inadvertent perioperative hypothermia, defined as a core temperature below 36 °C, can lead to complications but is preventable when nursing interventions follow clinical practice recommendations. At an urban outpatient surgery center in Kirkland, Washington, a quality improvement initiative was implemented to improve thermoregulation practices to 80% over 8 weeks.
DESIGN: A rapid cycle quality improvement project, guided by the Institute of Medicine's safety aim, was carried out using the Plan-Do-Study-Act model.
METHODS: Quantitative data were collected via a chart review with the

BACKGROUND: Obstetric patients may have long postanesthesia care unit (OB-PACU) stays after surgery because of residual regional block or other conditions. This study evaluated whether modified discharge criteria might allow for earlier discharge without compromising patient safety.
METHODS: Data were prospectively collected for 6 months for all patients (N=358) who underwent cesarean section or tubal ligation and recovered in the OB-PACU. Regional anesthesia was used in 94% of patients. The duration of anesthesia and PACU stays, the presence and treatment of events in the PACU, and the regre

Computerization of these protocols is in progress. This project is funded for one year with projected completion in January 1994. This study will form a framework in which further research can be completed. Utilizing protocols will allow the measurement of nursing decision making by testing relationships between parameters and interventions and by identifying rules for decision making. For example, questions that may be answered include which physiologic parameters do clinicians treat and in what order or priority, as well as what is the impact on patient outcomes in regard to cost of care and

BACKGROUND: Robotic technology has the potential to assist nurses, but the complexity and unpredictability of health care environments cannot be replicated in a laboratory setting. Furthermore, there is a lack of experiential evidence that robotic technology will meaningfully impact nursing. Collaborative development of technology and real-world usability studies offers the ability to address problems early in the design process when functional changes can be implemented.
OBJECTIVE: The purpose of this study was to use an observational study and systematically evaluate the work system of inpa
