The nocebo effect, negative treatment outcomes arising from patient expectations, therapeutic context, or clinician communication, plays a possibly significant yet often under-estimated role in psychotherapy. Psychotherapy can be conceived as a symbolic drama in which patients can experientially realize their capacity to change. Methods derived from hypnosis can empower therapy without the use of formal trance. A case conducted by Milton Erickson is presented and deconstructed in order to illuminate Erickson's therapeutic patterns. A model is offered for adding drama to therapy, and the model is placed into a larger model of choice points in psychotherapy.

Many patients with dysthymia respond to antidepressant pharmacotherapy. Psychotherapy of dysthymic patients has received less study, but it may have efficacy as a primary treatment or as complement to pharmacotherapy. A list of psychoreactive and neurotic disorders is given, which are often seen in general practice and which are suitable for psychotherapy. These disorders are shortly discussed. Subsequently, a survey of the most important psychotherapeutic methods is given and conditions for a positive outcome are described. Finally, the authors suggest further information on this subject for general practitioners, among others in Balint groups.

The more recent literature on the reliability and the status of psychiatric diagnosis is reviewed and confronted with the latest experimental results on the indication for psychotherapy. Accordingly it can be hypothesized that at least for outpatient psychiatry/psychotherapy there is no meaningful relationship between diagnosis and indication. Diagnoses do not imply indications for psychotherapy. This assumption is verified by the results of an outpatient fieldstudy and the implications are discussed.

This is a case report of a patient who stutters and was treated for problems of self-esteem with focused brief psychotherapy. The literature by and large is against the use of individual psychotherapy for neurotic problems in patients who stutter. The countertransference may account for the unwarranted pessimism in the literature about such patients. In this case the successful resolution of countertransference feelings facilitated the achievement of the goals of therapy. Further reports on such treatment attempts are warranted. A review of previous studies of the psychological reactions to blindness is presented from the early anecdotal material to more recent systematic research. The psychodynamics of a case study of a patient with acquired blindness secondary to accidental shooting by her son are discussed.
