A study sample of 51 patients with acute and transient psychotic disorder (ATPD) (ICD-10) is presented. The findings suggest that, in hospital settings, ATPD is a non-frequent condition with onset in early adult life and most often associated with female sex, good premorbid social functioning and no or minor/moderate psychosocial stressors. The DSM-IV criteria distribute the patients into three diagnostic categories: schizophreniform disorder (41%), brief psychotic disorder (33%) and psychotic disorder not otherwise classified (25%). A high prevalence (63%) of personality disorders (PD) is rev

BACKGROUND: In certain disorders the boundaries between thoughts, obsessions, overvalued ideas and delusions are not always clearly delineated.
AIM: To find out whether delusions can be distinguished from the convictions that often accompany anorexia nervosa, obsessive compulsive disorder (OCD), body dysmorphic disorder (BDD) and hypochondriasis, all of which apparently may involve impaired reality testing.
METHOD: The literature was reviewed with the help of PubMed, using as key words 'delusions' in combination with 'hypochodriasis', 'anorexia nervosa', 'body image', 'obsessive compulsive d

IMPORTANCE: Methylphenidate is the leading pharmacological treatment for attention-deficit/hyperactivity disorder (ADHD) in childhood and adolescence. Individuals with ADHD have a higher risk of psychosis, but the long-term relationship between methylphenidate and risk of developing psychotic disorders is unknown.
OBJECTIVE: To estimate the relationship between methylphenidate treatment and the risk of nonaffective psychosis in children and adolescents diagnosed with ADHD.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study included instrumental variable analysis of data linkage from multip

The aim of the paper is to analyze the case of a man suffering from paranoid schizophrenia who experiences delusions and hallucinations concerning gender change, and to present ashort overview of the literature. The data presented in the case study were collected during a clinical interview, in the six-month diagnostic process. The interview was partly structured; the battery of tests were also used: MMPI-2, SCID-I, SCID-II, IPP, MoCA, and WAIS-R. A case study of a person whose birth-assigned sex was male but who identifies as female. In the diagnostic process, it turned out that he had delusi

While alcohol-induced psychotic disorder (AIPD) is well recognised, relatively little is known about the condition. We undertook a review of the literature to identify studies on the epidemiology, clinical manifestations, underlying neurobiology and treatment of AIPD. Few prospective studies have been conducted on AIPD. Recent advances have focussed on epidemiological, phenomenological, neuro-imaging, treatment and outcome issues. Current evidence suggests AIPD can be clinically distinguished from alcohol-withdrawal delirium and schizophrenia. The disorder may be more common than previously re

BACKGROUND: Persons with psychotic disorder may have poorer visual acuity (VA). The aim of the study is to investigate in a general population the prevalence of impaired habitual VA and self-reported difficulties in vision among persons with different psychotic disorders.
METHOD: A nationally representative sample of 6,663 persons aged 30 or older whose binocular VA for distance and for near vision was measured with current spectacles, if any. Diagnostic assessment of DSM-IV psychotic disorders used both SCID interview and case note data. Life-time ever diagnoses of psychotic disorders were c

Patients with a psychotic disorder have certain symptoms, movement disorders resulting from the pharmacological effects of medication. Knowing about this is important to the patients' dental treatment. A very small group will present with delusions about teeth or the benevolent or malevolent intentions of the dentist. Negative symptoms are an important characteristic of schizophrenic psychoses, meaning the absence of normal behaviour. Patients neglect personal hygiene and their lives have hardly any structure. They do not concern themselves with appointments, daily routines and social expectat

Autism spectrum disorders (ASD) and early-onset psychosis (EOP) are neurodevelopmental disorders that share genetic, clinical and cognitive facets; it is unclear if these disorders also share spatially overlapping cortical thickness (CT) and surface area (SA) abnormalities. MRI scans of 30 ASD, 29 patients with early-onset first-episode psychosis (EO-FEP) and 26 typically developing controls (TD) (age range 10-18 years) were analyzed by the FreeSurfer suite to calculate vertex-wise estimates of CT, SA, and cortical volume. Two publicly available datasets of ASD and EOP (age range 7-18 years an

Reduced cortical thickness has been demonstrated in psychotic disorders, but its relationship to clinical symptoms has not been established. We aimed to identify the regions throughout neocortex where clinical psychosis manifestations correlate with cortical thickness. Rather than perform a traditional correlation analysis using total scores on psychiatric rating scales, we applied multidimensional item response theory to identify a profile of psychotic symptoms that was related to a region where cortical thickness was reduced. This analysis was performed using a large population of probands w

Converging findings point to brain- and body-wide energetic dysregulation in psychotic disorders, implicating metabolic abnormalities across different levels of investigation, including genetic, cellular, postmortem, and in vivo neuroimaging studies. The present review synthesizes this literature documenting aberrant energy production and use, specifically related to mitochondrial and redox dysregulation, in psychotic disorders. Mitochondrial DNA, copy number variants, polygenic risk, and candidate genes involved in metabolic pathways converge on glycolytic, mitochondrial, redox, and oxidative

Sensory impairments, including visual and auditory loss, are common across the lifespan and are increasingly recognized as risk factors for psychiatric disorders, particularly psychosis. Evidence suggests that disruptions in sensory processing, multisensory integration, and self-other signal differentiation may contribute to cortical disorganization overlapping with mechanisms implicated in psychotic illness. The psychiatric consequences of sensory impairment are influenced by chronicity and timing of onset, with congenital vs acquired loss conferring differing risks and clinical presentations

INTRODUCTION: Illness course plays a crucial role in delineating psychiatric disorders. However, existing nosologies consider only its most basic features (e.g., symptom sequence, duration). We developed a Dynamic Causal Model (DCM) that characterizes course patterns more fully using dense timeseries data. This foundational study introduces the new modeling approach and evaluates its validity using empirical and simulated data.
METHODS: A three-level DCM was constructed to model how latent dynamics produce symptoms of depression, mania, and psychosis. This model was fit to symptom scores of n

An opportunity has opened for research into primary prevention of psychotic disorders, based on progress in endophenotypes, genetics, and genomics. Primary prevention requires reliable prediction of susceptibility before any symptoms are present. We studied a battery of measures where published data supports abnormalities of these measurements prior to appearance of initial psychosis symptoms. These neurobiological and behavioral measurements included cognition, eye movement tracking, Event Related Potentials, and polygenic risk scores. They generated an acceptably precise separation of health

In the development of the International Classsification of Disorders (ICD-11), the World Health Organisation (WHO) has used a systematic approach with a focus on the utility of the revised classification criteria. For all major groups of mental and behavioural disorders working groups were set up, and these reviewed the scientific evidence and developed suggestions for revised classification criteria. These initial proposals, then submitted to public consultation and review, resulted in updated proposals. The chapter title for the primary psychotic disorders will be "Schizophrenia and other pr

BACKGROUND: Recent findings suggest that dopaminergic abnormalities found in psychotic disorders may be secondary to nitric oxide dysfunctions. Nitric oxide seems to influence glutamatergic and dopaminergic neurotransmission, both of which have been associated with psychosis.
OBJECTIVE: To search and review published works which examined the influence of nitric oxide in psychotic disorders subjects.
METHODS: The research was executed in the on-line collections of Pubmed and ISI Web of Science. The key aspects utilized were "Psychotic Disorders AND Nitric Oxide", "Psychosis AND Nitric Oxide",

BACKGROUND: Psychosis is three times more common in people with an intellectual disability than in those without an intellectual disability. A low intelligence quotient (IQ) is a defining characteristic for intellectual disability and a risk factor for poor outcome in psychosis. Clozapine is recommended for treatment-resistant psychosis. The effect of psychotropic medication can be different in people with intellectual disability; for example, they may be more prone to side effects. People with an intellectual disability and psychosis form a special subgroup and we wanted to examine if there i

Cardiovascular disease (CVD) is a well-described complication of schizophrenia, however, mechanisms connecting CVD with other facets of psychotic disorders, such as neurocognition, are not understood. The current study examined folate metabolism as a potential mechanism of CVD and neurocognitive deficits by: 1) using endothelial dysfunction as a biomarker of CVD, and 2) comparing enzymes associated with neurocognition, CVD, and critical to folate metabolism, methylenetetrahydrofolate reductase (MTHFR) and catechol-o-methyl transferase (COMT). Endothelial function was assessed in 147 participan

OBJECTIVE: To characterize the affective dimensions of psychopathology in patients with a nonaffective psychosis and to examine their validity against a number of external variables.
METHODS: Five-hundred and thirty-five patients with a lifetime DSM-IV diagnosis of nonaffective psychosis were assessed during the index episode for 22 affective symptoms. These symptoms were factor analyzed and the resulting factor scores were examined for associations with a number of risk factors, illness characteristics, index episode psychopathology and outcome variables.
RESULTS: One-hundred and fifty-six
