The authors describe one case of quadruplet pregnancy create difficulties in diagnosis and conclude with unfavourable outcome. One of fetuses have hydrops foetus generalisatus, another foetus is malformative and the rest two fetuses are normal anatomy. A case of a quadruplet pregnancy containing a complete mole and three fetuses is reported. A 29-year-old Japanese woman who had received clomiphene therapy was delivered of a complete mole, one dead and two living fetuses, and three placentas. Deoxyribonucleic acid fingerprint analysis proved the androgenesis of molar tissue and the trizygosity of the three fetuses.

Prescribing clomiphene citrate for infertility patients has been taken very lightly. High order pregnancy can happen with this treatment. In addition to high perinatal mortality and morbidity of multiple pregnancy, the economic and social strains on the national health system and the family is great. The case presented showed that although the outcome of pregnancy was successful but the cost to the national health was tremendous. It is therefore recommended that measures should be taken to avoid high order births. Moreover, delaying the delivery, if possible, will be cost effective.

OBJECTIVE: We report a case of conjoined twins in a trichorionic quadruplet pregnancy after ovulation induction with clomiphene citrate for anovulation.
METHODS: The diagnosis of a trichorionic quadruplet pregnancy was made in routine ultrasound at 11 weeks' gestation.
RESULTS: Conjoined twinning was thoracopagus and presented with increased nuchal translucency. Selective termination of the conjoined twins was performed with potassium chloride injection. The patient delivered 2 healthy infants after an uneventful follow-up.
