Nonoperative Management of Cervical Radiculopathy.

Cervical radiculopathy describes pain in one or both of the upper extremities, often in the setting of neck pain, secondary to compression or irritation of nerve roots in the cervical spine. It can be accompanied by motor, sensory, or reflex deficits and is most prevalent in persons 50 to 54 years of age. Cervical radiculopathy most often stems from degenerative disease in the cervical spine. The most common examination findings are painful neck movements and muscle spasm. Diminished deep tendon reflexes, particularly of the triceps, are the most common neurologic finding. The Spurling test, s…

Paraspinal pyomyositis associated with radiculopathy.

A 57-year-old woman presented with a fever and left-sided hip and back pain. The hip pain was explained by L3 radiculopathy because the pain was distributed on the L3 dermatome accompanied by allodynia. On the other hand, the back pain was located on the left side 1 cm from the spinal process of the L3-4 vertebrae. Enhanced computed tomography of the abdomen revealed an abscess in the left paravertebral muscle, and pyomyositis was diagnosed. Following percutaneous drainage via ultrasound, both the hip and back pain immediately improved. Paraspinal pyomyositis as well as lesions inside the spin…

Electrophysiological evaluation in lumbosacral radiculopathy.

BACKGROUND: Prevalence of electrophysiological abnormalities in patients with lumbosacral radiculopathy was evaluated. METHODS: A cross-sectional study was performed on 97 consecutive patients with the clinical diagnosis of lumbosacral radiculopathy. Complete neurological examinations and standard electrophysiological evaluation was performed on the patients. RESULTS: Patients under study had the mean age of 46.4 ± 13.1 years (mean ± standard deviation). There were positive MRI findings in 64% of the patients. In 43% L5 root and in 40% S1 root was involved. Abnormal electrophysiological find…

Genetic disorders producing compressive radiculopathy.

Back pain is a frequent complaint seen in neurological practice. In evaluating back pain, neurologists are asked to evaluate patients for radiculopathy, determine whether they may benefit from surgery, and help guide management. Although disc herniation is the most common etiology of compressive radiculopathy, there are many other causes, including genetic disorders. This article is a discussion of genetic disorders that cause or contribute to radiculopathies. These genetic disorders include neurofibromatosis, Paget's disease of bone, and ankylosing spondylitis. Numerous genetic disorders can …

Clinical evidence of chemical radiculopathy.

It is universally accepted that an anatomic abnormality such as a herniated disc or spinal stenosis can lead to radicular leg pain. There is … rpose of this paper is to report upon patients who satisfy strict criteria affirming a diagnosis of radiculopathy in the presence of normal imaging studies. The study was designed as a prospective case series of patients fulfilling inclusion and exclusion criteria at a university hospital outpatient physiatric spine practice. Inclusion criteria consisted of sym …

Surgical management of cervical radiculopathy.

Cervical radiculopathy presents as pain in a dermatomal distribution. Despite conservative nonoperative therapy, a large subset of patients will require surgical intervention. Indications for surgery include recalcitrant radiculopathy despite nonoperative treatment for more than 6 weeks and progressive motor deficit or disabling motor deficit (deltoid palsy, wrist drop) prior to 6 weeks. Anterior and posterior approaches have both yielded successful results in appropriately selected patients. Anterior cervical diskectomy and fusion is the generally preferred treatment for radiculopathy when th…

Conservative treatment of cervical radiculopathy.

To assess the efficacy of nonsurgical treatment of cervical radiculopathy, 50 patients with objective signs and symptoms of cervical discogenic radiculopathy (CDR) without myelopathy, treated conservatively on an outpatient basis with a mean follow-up of 9.4 months were retrospectively reviewed. Then an additional 33 patients were entered sequentially onto a prospective study to assess conservative treatment; they were followed for 100 days or longer with a mean follow-up of 24.7 months. All 83 patients (50 + 33) had objective neurologic deficits (sensory, motor, or both).Of the initial 50 pat…

Lumbar radiculopathy and intraspinal gas.

Gas within the spinal canal is uncommon and has been associated with trauma, infection and disc degeneration and vacuum phenomenon. We report a 73-year-old man with a history of intermittent low back pain who developed sharp low back pain with radiation to the left calf and ankle when ambulating or standing. Relief was obtained with sitting or lying down, which led to the clinical impression of spinal stenosis. The neuromuscular exam was unremarkable and no sciatic nerve tension sign was present. Electrodiagnostic studies were consistent with an acute S1 radiculopathy, while a dermatomal somat…

Cervical spondylotic myelopathy and radiculopathy.

A review of the extensive literature on cervical spondylotic myelopathy and radiculopathy shows that the clinical picture and pathology are well defined, and the complex pathogenetic mechanisms are better understood. With recent advances in investigative procedures: computed tomography, magnetic resonance imaging and somatosensory evoked potentials, the diagnosis can be more accurate and the assessment more complete. Careful selection of patients for the appropriate treatment modality (conservative, anterior or posterior surgery) is crucial to the success in management.

Muscular hypertrophy after chronic radiculopathy.

In four patients, calf muscular hypertrophy developed after the onset of sciatica. Hypertrophic muscles were weak and showed electromyographic signs of denervation. In all cases, calf muscle biopsy showed striking hypertrophy of type 1 and, especially, type 2 muscle fibers. This hypertrophy was associated with other signs indicating a neurogenic lesion. Muscle hypertrophy is a rare finding in neurogenic lesions. Stretch and exercise of muscle are probably the causative factors.

Atypical Radiculopathy in an Oncology Patient.

Practitioners face significant challenges when evaluating patients with cancer who suffer from back pain. Back pain is one of the most common reasons for patients to visit medical providers. This case study will review a patient with cancer presenting with atypical radicular pain.
