Rational prescribing in paediatrics in a resource-limited setting.

INTRODUCTION: There is evidence of inappropriate medication use, causing unnecessary costs for health systems, particularly those with limited resources. Overprescription is commonly reported and can lead to antibiotic resistance. Prescribing patterns differ between countries; little is known about paediatric prescribing practices in Africa. OBJECTIVES: To investigate prescribing practices in children in The Gambia, West Africa. METHOD: A retrospective survey of prescribing practices in children under 5 years of age based on WHO protocol DAP/93.1 was conducted. Twenty government-run health c

A case of precocious puberty in a setting with limited resources.

Precocious puberty is a rare condition characterized by the development of secondary sexual characteristics before the median age for the sex. It is either gonadotropin dependent also called as central or gonadotropin independent also known as peripheral type. Hypothalamamic Hamartoma is a common cause of the central or precocious puberty due to organic brain lesion. Here we present a two year male who presented us with precocious puberty due to a hypothalamic Hamartoma.

Telestroke care in resource-limited settings: A World Stroke Organization scientific statement.

BACKGROUND: Telestroke services represent a crucial strategy for expanding access to acute stroke care in regions with limited local stroke expertise. However, existing recommendations and guidelines are predominantly derived from evidence generated in high-income countries and based on generally resource-intensive models. Such models and evidence may not be directly transferable to resource-limited settings. This Scientific Statement aims to summarize current knowledge on telestroke in resource-limited settings, highlight key considerations for establishing networks, and provide practical rec…

Diagnostic Challenges of Methanol Poisoning in Resource-Limited Settings: A Case Series.

Methanol poisoning is a life-threatening emergency, but diagnosis is challenging in resource-limited settings lacking toxicology testing. This case series presents three cases of methanol poisoning: two patients with neurological symptoms and unexplained high anion gap metabolic acidosis were initially misdiagnosed as stroke or diabetic ketoacidosis and died after transfer, whereas the third patient received prompt empirical hemodialysis based on clinical suspicion and survived with full recovery. These cases illustrate that early recognition and empirical hemodialysis guided by clinical judgm…

Pull-Through for Hirschsprung's Disease: Insights for Limited-Resource Settings From Mbarara.

INTRODUCTION: In many resource-limited settings, patients with Hirschsprung's Disease (HD) undergo initial diverting colostomy, followed by pull-through, and finally, colostomy closure. This approach allows for decompression of dilated and thickened bowel and improved patient nutritional status. However, this three-stage approach prolongs treatment duration, with significant stoma morbidity, costs, and impact on quality of life. Our aim was to determine whether pull-through for HD can safely be performed with simultaneous stoma closure, reducing treatment approach from three to two stages. ME…

Implementation of HIV prevention interventions in resource limited settings: the partner project.

Evidence-based HIV prevention interventions have been translated to a variety of contexts across sub-Saharan Africa. Non-specialized community health center (CHC) staff members have been successfully engaged to deliver the interventions, which can be integrated into pre-existing HIV service programs in community-based health care delivery sites. This manuscript describes the process of implementing the Partner Project, a couples HIV risk reduction intervention, and examines the ability of CHC staff to achieve risk reduction outcomes comparable to those of the highly-trained research staff. Experience of Médecins Sans Frontières in laboratory medicine in resource-limited settings.

In medical humanitarian assistance, the diagnosis of diseases plays a crucial role. Laboratory investigations are one of the main diagnostic tools utilized in Médecins Sans Frontières' (MSF) programs. Currently MSF supports and/or operates more than 130 laboratories in approximately 45 countries. The variety of analysis offered depends largely on the context of the program and the availability of context adapted tools and ranges from sophisticated laboratories specializing in tuberculosis culture to small laboratories within a primary health care program or operating as mobile clinics. The lar

Prevention of HIV infection among injection drug users in resource-limited settings.

Injection drug use contributes to considerable global morbidity and mortality associated with human immunodeficiency virus (HIV) infection and AIDS and other infections due to blood-borne pathogens through the direct sharing of needles, syringes, and other injection equipment. Of approximately 16 million injection drug users (IDUs) worldwide, an estimated 3 million are HIV infected. The prevalence of HIV infection among IDUs is high in many countries in Asia and eastern Europe and could exacerbate the HIV epidemic in sub-Saharan Africa. This review summarizes important components of a comprehe

Diagnostic and treatment difficulties of pyelonephritis in pregnancy in resource-limited settings.

Limited microbiology services impede adequate diagnosis and treatment of common infections such as pyelonephritis in resource-limited settings. Febrile pregnant women attending antenatal clinics at Shoklo Malaria Research Unit were offered urine dipstick, sediment microscopy, urine culture, and a 5-mL blood culture. The incidence of pyelonephritis was 11/1,000 deliveries (N = 53 in 4,819 pregnancies) between January 7, 2004 and May 17, 2006. Pyelonephritis accounted for 20.2% (41/203) of fever cases in pregnancy. Escherichia coli was the most commonly isolated pathogen: 87.5% (28/32) of organi…

Integrated Approach to Diagnosing Limb-Girdle Muscular Dystrophies in Resource-Limited Settings.

Limb-girdle muscular dystrophies (LGMD) are a genetically heterogeneous group of neuromuscular disorders characterized by overlapping clinical features, including progressive muscle weakness and wasting, elevated creatine kinase (CK) levels, and motor and skeletal abnormalities. Due to phenotypic overlap and variable severity, clinical diagnosis alone is often unreliable. Incorporation of genetic investigations to aid diagnosis/differential diagnosis has accelerated the diagnostic process but remains largely limited to developed countries. Here, we employed genetic testing as a first-tier appr

Airway Management in the ICU and Emergency Department in Resource-Limited Settings.

Airway management is central to the care of critically ill patients, yet it remains one of the most challenging interventions in emergency departments and intensive care units. Patients often present with severe physiological instability, limited cardiopulmonary reserve, and high acuity, while clinicians often work under constraints related to time for preparation, equipment availability, trained workforce, monitoring, and access to advanced rescue techniques. These challenges are particularly pronounced in low- and middle-income countries and other resource-limited or austere environments, wh

Crossing the blood-brain barrier: neurotrauma in disasters and resource-limited settings.

Traumatic brain injury (TBI) is a major cause of morbidity and mortality in natural disasters, armed conflict, and other resource-limited settings, where advanced imaging, invasive neuromonitoring, and specialized neurosurgical care, common in high-resource environments, are often unavailable. This narrative review summarizes the global health impact of TBI in austere conditions and outlines key management strategies and ethical considerations relevant to these contexts. Evidence from civilian disaster responses, military conflicts, and global health guidelines highlight the need for adaptable…

Role of Preoperative Echocardiography in Resource-Limited Settings and Delayed Patient Care.

Preoperative echocardiography has long been used as an effective diagnostic tool in the preoperative assessment of surgical candidates. In elderly patients, hip fractures represent a critical medical emergency with a high risk of complications that can occur if not addressed promptly. Unnecessary preoperative investigations may lead to delays and potentially compromise the quality of patient care. The motivation to evaluate the utility of preoperative echocardiography in select clinical scenarios, particularly in low- and middle-income countries (LMICs) where resources are limited, arose from

AL Cardiac Amyloidosis in Resource-Limited Settings: Diagnostic Challenges and Therapeutic Gaps.

Amyloid light chain (AL) cardiac amyloidosis remains a diagnostic challenge in resource-limited settings. We present 3 cases with varying heart failure presentations, all demonstrating concentric left ventricular hypertrophy with characteristic apical sparing strain pattern on echocardiography. Serum free light chains were consistently abnormal, though immunofixation was negative in 2 patients. Traditional screening biopsies were negative in 2 cases, requiring endomyocardial biopsy for diagnosis. Mass spectrometry for amyloid typing was unavailable; immunofluorescence performed in 1 patient co…

Black coloured colonies on Tinsdale medium: A challenge in resource-limited settings.

INTRODUCTION: The upper respiratory tract harbours diverse microbial flora that contribute to homeostasis and immunity but may occasionally become pathogenic. The upper respiratory tract acts as a point of entry for various pathogenic bacteria, which can have local or systemic manifestations. Many non-pathogenic Corynebacterium species exist as commensals, but a few species can produce toxins and can have systemic manifestations. Identifying such pathogenic Corynebacterium at the earliest is a diagnostic challenge faced by microbiological laboratories. To shorten diagnostic turnaround time, st
