My Home is my Castle? The Role of Living Arrangements on Experiencing the COVID-19 Pandemic: Evidence From Germany.

During the early months of the COVID-19 pandemic in Germany, social restrictions and social distancing policies forced large parts of social life to take place within the household. However, comparatively little is known about how private living situations shaped individuals experiences of this crisis. To investigate this issue, we analyze how experiences and concerns vary across living arrangements along two dimensions that may be associated with social disadvantage: loneliness and care. In doing so, we employ quantitative text analysis on open-ended questions from survey data on a sample of

Experiences and care trajectories of persons living with Hepatitis B in Senegal: A qualitative study.

Hepatitis B virus (HBV) infection is a public health concern in Senegal, where individuals encounter multifaceted barriers in their care trajectories. This qualitative study aimed to identify the individual, social, and structural determinants influencing HBV care delivery to inform targeted interventions for improving access. Between February and April 2021, we included 29 adults living with HBV from SEN-B, a prospective HBV cohort in urban Senegal providing biannual clinical monitoring for HBV-related liver complications. We conducted individual semi-structured interviews, during which parti

Relational, cognitive, and structural social capital in shaping livelihood resilience in informal agri-food systems.

INTRODUCTION: Smallholder livelihoods in the Global South operate under persistent uncertainty shaped by price volatility, climate variability, and limited institutional protection. While informal agri-food systems are often framed as inefficient or transitional, their persistence raises questions about how coordination and resilience are sustained in the absence of formal governance structures. This study examines how the relational, cognitive, and structural dimensions of social capital interact to shape livelihood resilience. METHODS: This study draws on qualitative research conducted in E

[Repetitive impulse-associated behavioral disorders in Parkinson's disease].

Parkinson's disease (PD) is associated with a number of behavioral disorders which may cause considerable social, professional or financial problems. Impulse control disorders (ICDs), such as pathological gambling, binge eating, compulsive shopping and hypersexuality occur in approximately 13-14% of PD patients. Further behavioral disorders are the dopamine dysregulation syndrome (DDS), a substance dependence characterized by craving for dopaminergic substances and punding (prolonged repetitive activities which are not goal-oriented).Treatment-related risk factors are dopamine agonists for ICD

Navigating the Intrahepatic Cholestasis of Pregnancy: An Autobiographical Case Report.

Intrahepatic cholestasis of pregnancy (ICP) is a liver condition commonly occurring during pregnancy, with an unclear etiology. This condition not only causes significant discomfort for the mother due to severe itching but also poses serious risks to the fetus. Effective and timely management of ICP, including diagnosis, consistent monitoring, and treatment, is crucial to mitigate maternal discomfort and prevent fetal complications. The challenges in managing ICP include the absence of clear initial diagnostic criteria, delays in lab results, evolving treatment guidelines, and the financial bu

Too much or too little? A short-term longitudinal study of youth's own economic resources and risk behaviour.

This study examined socioeconomic differences in risk behaviours according to youth-oriented measures of economic resources. Using a representative sample of Swedish adolescents (n = 3,939, 51% females), the associations that youth's own economy shared with smoking, drinking and conduct problems were examined. Data was based on population register and self-report information when participants were in grades 8 (T1 aged 14-15) and 9 (T2 aged 15-16). Missing activities due to financial constraints and having a cash margin were each positively associated with concurrent risk behaviours. However, l

What patients want to know about genetic testing for kidney disease.

Previously, genetic kidney disease was often recognised when family members shared clinical features. Now, many genetic kidney diseases are diagnosed when testing demonstrates a pathogenic variant in a gene associated with the disease. Detection of a genetic variant also identifies the mode of inheritance, and suggests family members at risk. The genetic diagnosis has additional advantages for patients and their doctors even when no specific treatment is available since it often indicates likely complications in other organs, the clinical course, and management strategies. Generally, informed

'I just want to be able to make a choice': Results from citizen deliberations about mammography screening in Ontario, Canada.

Despite Canada's long history with mammography screening, little is known about citizens' perspectives about mammography and how best to support women to make informed choices about screening. To address this gap, a series of four citizen deliberation events were held in 2015-16 in Ontario, a Canadian province with an organized population-based breast screening program in place since 1990. Forty-nine individuals participated in four citizen panels, each comprising an information session highlighting the evidence about mammography, and large- and small-group deliberations about approaches to su

Understanding how education/support groups help lone mothers.

BACKGROUND: Lone-mother led families are at increased risk of psychosocial disadvantage, social isolation and mental health morbidity. Community-based programs are more accessible for families seeking assistance. We examine the experiences of eight lone mothers participating in a larger randomized controlled trial (RCT) of a community-based education/support group program using mixed methods. METHODS: A purposeful sample of eight mothers participating in the intervention arm of an RCT of community-based support/education groups was selected for the qualitative study. Adherence to ready-to-use therapeutic food in older children with sickle cell anaemia and severe acute malnutrition: a mixed-methods study in a low-income setting.

INTRODUCTION: Children with sickle cell anaemia (SCA) in Sub-Saharan Africa face a higher risk of malnutrition, increasing morbidity and mortality rates. We conducted a randomised controlled feasibility trial to treat children aged 5-12 years with SCA and severe acute malnutrition (body mass index Z-score <-3.0) in Kano, Nigeria (n=108). Despite high rates of ready-to-use therapeutic food (RUTF) consumption, as measured by returned RUTF sachets, suboptimal weight gain was observed, possibly due to widespread sharing of RUTF. In this ancillary study, we aimed to identify factors influencing adh

Tracking the impact of COVID-19 in Victoria, Australia: Shocks, vulnerability and insurances among residents of share houses.

COVID-19 is re-shaping cities and regions, as residents respond to large disruptions to employment and social interaction and threats to public health. While the impacts of COVID-19 are extensive, certain groups are more vulnerable than others. Our research examines the impact of COVID-19 on members of share houses in the state of Victoria, Australia. This cohort is more likely to be young, casually employed, living in informal arrangements and at risk of homelessness than the broader population. We propose a conceptual framework for investigating the factors driving vulnerability to shocks an

A Governance Framework for Diagnostic Quality in Hospital-Radiology Joint Ventures.

RATIONALE AND OBJECTIVES: Hospital-radiology joint ventures (JVs) are forming at an accelerating pace as health systems seek to recapture outpatient imaging volume and radiology groups pursue capital and operational partnerships. 3 decades of peer-reviewed literature show a reproducible risk: when financial incentives are not structurally separated from clinical authority, diagnostic quality degrades. MATERIALS AND METHODS: At an academic medical center governing a nascent hospital-radiology JV, we developed the Blueprint for Diagnostic Imaging Excellence by synthesizing principal-agent and s

What Does the Institute for Safe Medication Practices' Survey Tell Us About the Impact of Shortages on Patient Safety?

The continuing crisis with drug shortages and supply chain disruptions results in ongoing patient safety and financial concerns. The Institute for Safe Medication Practices (ISMP) and ECRI conducted a survey from June 29, 2023, to July 27, 2023, inviting practitioners to share their experiences with drug, supply, and equipment shortages during the previous 6 months. Practitioners provided insight about drug, single-use supplies (e.g., tubing, syringes, cassettes), and durable medical equipment (e.g., infusion devices) shortages. Almost half (44%) of the survey respondents reported shortages im

Current epidemiologic evidence and case reports of occupationally acquired HIV and other bloodborne diseases.

Studies in which noninfected persons shared households with human immunodeficiency virus (HIV)-infected family members and friends, and had substantial though non-sexual contact with them revealed no transmissions of the HIV virus. These results suggest a similar low risk for healthcare workers, particularly those who have little or no contact with blood. However, occupational transmission of HIV, when it does occur, is most commonly caused by needlestick injuries. Nurses make up the largest group--nearly 80%--of hospital workers who contract HIV infection on the job. Generally, the risk ratio

The Potential Benefits of Deferred Payment for a Hypothetical Gene Therapy for Congestive Heart Failure: A Cost-Consequence Analysis.

BACKGROUND: The emergence of potentially curative pharmacologic treatments that deliver long-term clinical benefits with a limited number of doses may create short-term budget challenges for payers as their unit price can be high. OBJECTIVE: This paper tests the clinical and financial properties of a deferred payment model (DPM) in hypothetical therapy for congestive heart failure (CHF) from the perspective of payers, manufacturers, and patients. METHODS: We present an empirical analysis of longitudinal data for cardiovascular admissions and mortality using a Markov transition model for pati

Bridging policy and practice: implementation of emerging infectious disease risk assessment in China from a One Health perspective.

BACKGROUND: Risk assessment of emerging infectious diseases (EIDs) enables the early identification of potential threats and informs timely public health responses. A One Health perspective may enhance its comprehensiveness by integrating human, animal, and environmental factors. However, current understanding of the policy implementation of EID risk assessment in China remains limited. This study examines the implementation of related policies in China to identify gaps between policy design and practice from a One Health perspective. METHODS: Semi-structured interviews were performed in Hain
