Open acromion fracture with a massive rotator cuff tear: A rare injury, case report.

INTRODUCTION AND IMPORTANCE: The combination of an open acromion fracture with a massive rotator cuff tear is an exceptional lesion, rarely described in the literature. It generally occurs after open shoulder trauma, and requires well-codified therapeutic measures to avoid complications that could keep the patient from being as autonomous as possible and able to perform daily activities. CASE PRESENTATION: We report the case of a 26-year-old patient with a wound on the posterior surface of the left shoulder following an attack. Standard X-ray showed a fracture of the acromion. Surgical explor

Magnetic resonance imaging evaluation of the correlation between calcific tendinitis and rotator cuff injury.

BACKGROUND: This study aims to evaluate the incidence of calcific tendinitis (CaT) in rotator cuff tears (RCTs) and to assess the correlation between CaT and RCTs with magnetic resonance imaging (MRI). METHODS: The MRI of 108 patients with rotator cuff CaT admitted to our hospital from January 2019 to January 2021 were retrospectively analyzed. Another retrospective analysis was made of 108 patients with similar age, gender, occupation, and shoulder injury side to those in the first group. The incidence of RCTs and their correlation with CaT were assessed based on an MRI of shoulder joints.

Factors associated with thoracic spinal cord injury, lesion level and rotator cuff disorders.

BACKGROUND: The study was concerned with the secondary functional compromise persons with long-term paraplegia contend with, that has been shown to be associated with wheelchair use. Of particular interest was the relationship between the level of thoracic spinal cord injury (SCI) and factors predisposing to rotator cuff disorders (RCD), one of the most common upper extremity musculoskeletal complications linked with long-term SCI. Disorders of these peri-articular structures have previously been associated with impaired trunk postural control and abnormal muscle strength ratios in both able-b

Definition of the terms "acute" and "traumatic" in rotator cuff injuries: a systematic review and call for standardization in nomenclature.

BACKGROUND: Although of high relevance for clinical decision making, there exists no consensus throughout the literature of the terms "acute" and "traumatic" used in the classification of rotator cuff tears. With differing definitions, the comparability of outcome studies may be limited. The aim was to provide a detailed systematic review of the definitions used in the literature and present a suggestion for a standardization in nomenclature based on the findings. METHODS: Four different internet databases were searched in February 2020 using the terms ("acute" OR "traumatic" OR "trauma" OR "

Clinical application of multi-parameter ultrasonic assessment for grading rotator cuff injuries: development and validation of a novel scoring system.

BACKGROUND: Rotator cuff injuries, a leading cause of shoulder pain and dysfunction, present significant diagnostic challenges. While MRI is the diagnostic gold standard, ultrasonography (US) offers a cost-effective alternative but lacks standardization. This study addresses this gap by developing and validating a novel multiparameter ultrasonic scoring system to increase the diagnostic accuracy and clinical applicability of shoulder ultrasonography for rotator cuff pathology. METHODS: A retrospective diagnostic accuracy study of 252 patients with suspected rotator cuff injury was analyzed. I…

Corticosteroid injection combined with a shoulder rehabilitation system for chronic rotator cuff injuries: a protocol for a randomised controlled trial.

INTRODUCTION: The main clinical symptoms of patients with chronic rotator cuff injuries (CRCI) include pain and/or limitation of movement, which severely reduce the function of the shoulder joint. Local injection of corticosteroid with local anaesthetics can control the inflammatory response and effectively relieve patients' pain in the short term. In addition, rehabilitation exercises are considered an important tool for improving shoulder function. However, due to the presence of pain or substandard execution of movements, it is often difficult for patients to achieve the desired therapeutic…

Biologic Treatments for Sports Injuries II Think Tank-Current Concepts, Future Research, and Barriers to Advancement, Part 2: Rotator Cuff.

Rotator cuff tears are common and result in considerable morbidity. Tears within the tendon substance or at its insertion into the humeral head represent a considerable clinical challenge because of the hostile local environment that precludes healing. Tears often progress without intervention, and current surgical treatments are inadequate. Although surgical implants, instrumentation, and techniques have improved, healing rates have not improved, and a high failure rate remains for large and massive rotator cuff tears. The use of biologic adjuvants that contribute to a regenerative microenvir

Photosensitive and extracellular matrix-mimicking hydrogels promote the tendon-bone healing in rotator cuff injury.

The healing of the tendon-bone interface remains a significant challenge due to the lack of suitable bioactive materials for promoting interface regeneration. To address this, we developed a photosensitive hydrogel system designed to mimic the extracellular matrix. This system is based on light-induced imine crosslinking between type I collagen (Col I) and o-nitrobenzyl alcohol (NB)-modified hyaluronic acid (HANB). Experimental results demonstrate that the hydrogel (Col I-HANB) exhibits excellent controllability, low swelling rate, biodegradability, and tissue-adhesive properties. Using a fluo

Mitochondrial ROS dyshomeostasis: a key driver of accelerated supraspinatus atrophy after rotator cuff injury.

Rotator cuff injuries are common musculoskeletal disorders and are frequently accompanied by progressive supraspinatus muscle atrophy, which severely compromises functional recovery and surgical outcomes. Accumulating evidence indicates that mitochondrial reactive oxygen species (mtROS) dyshomeostasis is a central pathological driver of post-injury muscle degeneration. This review synthesizes current knowledge on the anatomical and histopathological changes following rotator cuff tears and focuses on the mechanisms governing mitochondrial ROS production, clearance, and dysregulation in the sup…

Differential ubiquitin-proteasome and autophagy signaling following rotator cuff tears and suprascapular nerve injury.

Previous studies have evaluated role of Akt/mTOR signaling in rotator cuff muscle atrophy and determined that there was differential in signaling following tendon transection (TT) and suprascapular nerve (SSN) denervation (DN), suggesting that atrophy following TT and DN was modulated by different protein degradation pathways. In this study, two muscle proteolytic systems that have been shown to be potent regulators of muscle atrophy in other injury models, the ubiquitin-proteasome pathway and autophagy, were evaluated following TT and DN. In addition to examining protein degradation, this stu

The low incidence of suprascapular nerve injury after primary repair of massive rotator cuff tears.

We measured the incidence of cuff retear and injury to the suprascapular nerve after mobilization and repair of a massive rotator cuff tear. Of one hundred four rotator cuff repairs performed over a 5-year period, 10 patients (7 men and 3 women, age range 22 to 68 years) had primary repairs of massive rotator cuff tears requiring cuff mobilization and an acromioplasty as their only procedure. These patients were evaluated at a mean of 2.5 years (range 2.0 to 3.0 years) after surgery. At follow-up electromyographic examination confirmed that 1 of the 10 patients had an iatrogenic suprascapular

Network Pharmacology and Molecular Docking Reveal the Mechanism of Rhizoma Drynariae Against Rotator Cuff Injury.

Rotator cuff injury (RCI) is a refractory musculoskeletal disorder marked by chronic inflammation, tenocyte apoptosis, impaired angiogenesis, and extracellular matrix (ECM) dysregulation, with limited clinical treatments available. This study employed network pharmacology and molecular docking to systematically investigate the active components, key targets, and signaling pathways of Rhizoma Drynariae (RD) against RCI. Eighteen active ingredients of RD and 213 non‑redundant targets were identified from TCMSP, ETCM, and SymMap, while 870 RCI‑related targets were retrieved from GeneCards and OMI

HDACi vorinostat protects muscle from degeneration after acute rotator cuff injury in mice.

AIMS: Rotator cuff (RC) injuries are characterized by tendon rupture, muscle atrophy, retraction, and fatty infiltration, which increase injury severity and jeopardize adequate tendon repair. Epigenetic drugs, such as histone deacetylase inhibitors (HDACis), possess the capacity to redefine the molecular signature of cells, and they may have the potential to inhibit the transformation of the fibro-adipogenic progenitors (FAPs) within the skeletal muscle into adipocyte-like cells, concurrently enhancing the myogenic potential of the satellite cells. METHODS: HDACis were added to FAPs and satel
