Dissecting cellulitis of the scalp.

Dissecting cellulitis of the scalp is a chronic, relapsing, inflammatory disease of the scalp that results in scarring alopecia. We present a case of a 32-year-old man with recalcitrant disease who is now responding to treatment with isotretinoin. The pathogenesis, clinical presentation, disease associations, and histopathological findings are reviewed. Treatment can be challenging. The literature on medical and surgical therapeutic options is reviewed.

Hair and scalp dermatoscopy.

Dermatoscopy is a noninvasive diagnostic tool that allows the recognition of morphologic structures not visible by the naked eye. Trichoscopy (scalp dermatoscopy and videodermatoscopy) is useful for the diagnosis and follow-up of hair and scalp disorders. However, it is not widely used in the management of hair disorders. This review provides updated information from the literature and our experience on the dermoscopic features of the most common hair and scalp disorders. This will enable dermatologists to make fast diagnoses of tinea capitis and alopecia areata, distinguish early androgenetic…

Advances in scalp reconstruction.

PURPOSE OF REVIEW: The present review summarizes traditional and advanced techniques used to reconstruct defects of the scalp. These deformities range from small defects that can be closed primarily to significant defects, which require free tissue transfer. RECENT FINDINGS: Increased use of tissue expanders, advancement rotational flaps, and hair transplantation has resulted in improved cosmetic outcomes for larger defects of the scalp. Free tissue transfer has provided a revolutionary method of reconstructing subtotal and total defects of the scalp, in particular those associated with neopl…

Verrucous carcinoma of the scalp.

Verrucous carcinoma is a distinctive form of low-grade squamous cell carcinoma. It typically involves the oral cavity, larynx, esophagus, and skin. Cutaneous lesions typically arise in the genitocrural area and plantar surface of the foot, with rare case reports of verrucous carcinoma arising elsewhere on the body. Human papillomaviruses, predominately types 6 and 11, have been associated with some case reports. We present a case of verrucous carcinoma arising on the scalp with negative human papillomavirus testing in a relatively young patient.

Involvement of scalp in leprosy.

It is generally believed that hairy scalp is an immune zone for leprosy lesion, because of the higher temperature of the scalp, which is not suitable for the habitation of lepra bacilli. Here is a report of borderline tuberculoid leprosy involving scalp and the right side of the face. The patient also had features of type 1 lepra reaction and facial palsy on the right side.

[Paroxismal reactions of the scalp].

The scalp is a unique site of the human body that may exhibit some particular physiopathological reactions. A few manifestations are paroxysmal. The main ones are the acute pruritus of pediculosis, and the pruritus at onset of dandruff or seborrhoeic dermatitis. One must add the telogen effuvium after solar exposure or during the post-partum, and the anagen dystrophic alopecia following chemotherapy. Other paroxysmal events include acute canitia and alopecia areata.

Diffuse neurofibroma of scalp.

Diffuse neurofibroma is an uncommon but distinct variety of neurofibroma, usually affecting trunk, head and neck regions of adolescents and young adults. The clinical features, gross macroscopic and histopathological findings are enunciated and the criteria for instituting the preferred modality of treatment for such lesions has been reviewed, stressing upon the need to exclude the neurofibromatoses preoperatively.

Sequestrated meningocele of the scalp.

A case of occipital sequestrated (rudimentary) meningocele in a 2-year-old girl is presented. The swelling was noticed at birth and did not grow over time. The skull radiograph showed no bone defect and ultrasound and computed tomography examinations demonstrated cystic mass with no connection to dura. The aim of this report is to draw the attention of radiology literature readers to this entity and to elaborate on the role of imaging in the preoperative assessment of such cases. The relation of sequestrated meningocele to the other conditions with ectopic meningeal tissue in the scalp is addr…

Scalp pain and hyperlipidaemia.

Acute pancreatitis and eruptive xanthomata are the only recognised direct complications of severe hypertriglyceridaemia, although peripheral neuropathy has been described in patients with hyperlipidaemia. We describe a patient with mixed hyperlipidaemia presenting with severe scalp pain and eruptive xanthomata. Both resolved with treatment. We suspect that high triglyceride concentration can affect the function of sensory nerve fibres.

Closure of a scalp defect.

In repairing cutaneous defects, the facial plastic and reconstructive surgeon is faced with many specialized areas of tissue. Reconstruction of thin eyelid skin and thick, sebaceous nasal skin requires different methods. The unique characteristics of the scalp make it one of the greatest repair challenges in the head and neck region, sometimes requiring multiple different reconstructive techniques for the same defect.

Arteriovenous malformations of the scalp.

We report twenty-four patients with cirsoid aneurysms of the scalp. For nine patients (38%), the lesions were related to trauma. Each of the patients presented with a pulsatile scalp swelling with a bruit. No focal neurological deficits were noted in any of the patients. Scalp malformations in all patients were confirmed by selective internal and external carotid angiography, with no intracerebral component revealed in any of the patients. Twenty-one patients had the lesions surgically excised, with good results. The remaining three refused surgical intervention. Meticulous surgical technique,…

Lipomatosis of the scalp and macrocephaly.

In summary, a case of macrocephaly and lipomatosis of the scalp and forehead has been presented. The phenotypic and clinical features found in our patient are in accordance with previous reports. The differential diagnosis with other hamartoneoplastic syndromes has been evaluated and discarded, concluding that the present report is in accordance with Bannayan-Zonana syndrome. However, as in other inherited traits, finding a genetic test to diagnose this entity remains a challenge.
