Scimitar syndrome: an uncommon cause of wheezing.

Scimitar syndrome is a rare congenital anomaly characterized by total or partial anomalous venous drainage of the right lung to the inferior vena cava. The infantile form presents with congestive heart failure and respiratory distress in early months of life. We report here the case of a 6-month-old male child who presented with second episode of respiratory distress and wheeze. Poor response to bronchodilators led to fibreoptic bronchoscopy and computed tomographic scan that revealed bronchial anomalies with aberrant drainage of right lung into the inferior vena cava by scimitar vein. The cas…

[Clinical and echocardiographic characteristics of scimitar syndrome].

Seven patients with scimitar syndrome underwent retrospective clinical and echocardiographic examination. The findings were: respiratory infection with dyspnea on moderate exercise in 90%, scimitar sign in four (57%), dextrocardia in five (71%), and interatrial septal defect in five (71%), one of whom had patent ductus arteriosus. Overall, two patients had patent ductus arteriosus: one also had aortic coarctation and the other, a bicuspid aortic valve. Dilation of the right cavities was found in five (71%) and blunt edge in five (71%). In two patients, anomalous drainage was into the right atr…

[Scimitar syndrome in the newborn child and infant].

We present 10 symptomatic patients with scimitar syndrome. Their age was below 1 year and four were newborn babies. In 5 patients there was secundum atrial septal defect and in one there was a patent ductus arteriosus. Sixty per cent of the patients had pulmonary sequestration. The right lung was moderately or severely hypoplasic in 8 cases and seven had frequent respiratory infections. The anomalous venous drainage produced a left-to right shunt at atrium level which in our series had a value of 2.87 +/- 0.5. Five patients had pulmonary hypertension either moderate or severe. The severity of …

Heart Transplantation in a Case of Scimitar Syndrome.

We present a rare clinical scenario of a patient with tetralogy of Fallot, hypertrophic cardiomyopathy, and concomitant scimitar syndrome. We created a scimitar vein cuff from a recipient heart, performed its translocation, and subsequently performed heart transplantation. This ingenuity reduces the likelihood of scimitar vein obstruction.

Lugones' technique for correction of Scimitar syndrome.

Several surgical techniques have been described to channel the right pulmonary venous return to the left atrium in the Scimitar syndrome which includes direct reimplantation, graft interposition, or repair. Because these techniques can result in pathway obstruction, Lugones has devised a technique which uses in situ pericardium to construct a wide tunnel connecting the pulmonary venous return to the left atrium. We report the use of Lugones technique in an adult with Scimitar syndrome.

Scimitar syndrome in a case with VACTERL association.

VACTERL association and Scimitar syndrome are rare congenital diseases. In this study, we report on a neonate with prenatal suspicion of VACTERL association and small left-sided cardiac structures, which, only on postnatal angiography, could be revealed to be part of a Scimitar syndrome. As this is the second reported case of VACTERL association and Scimitar syndrome, the presence of Scimitar syndrome should be considered in the prenatal and postnatal evaluation of VACTERL association.

Respiratory outcome in children with scimitar syndrome.

OBJECTIVE: To evaluate respiratory morbidities and lung function tests in the cohort of patients with scimitar syndrome evaluated at our center since 1976. STUDY DESIGN: Eighty-one children were investigated. Twenty-six patients died, all with the infantile form. The median duration of follow-up of surviving children was 7.2 years. RESULTS: A high rate of respiratory morbidities was measured, with 38% and 43% of children reporting pulmonary infections or wheezing episodes during the last 12 months of follow-up, respectively. One-third of children have been rehospitalized for a respiratory ca…

The scimitar syndrome: an Italian multicenter study.

BACKGROUND: Scimitar syndrome is a rare congenital heart disease. To evaluate the results of managing this malformation surgically, we have embarked on a multicenter Italian study involving seven different centers and reporting the largest published series in the medical literature. METHODS: From January 1997 to December 2007, 26 patients with scimitar syndrome who underwent surgical correction were included. Primary outcomes include hospital mortality and the efficacy of repair at the follow-up. RESULTS: Median age was 11 years (interquartile range, 1.8 to 19.9 years). Nineteen patients (73…

Tetralogy of Fallot associated with scimitar syndrome.

We present a case of tetralogy of Fallot associated with Scimitar syndrome. The patient was an 11-month old female who underwent successfully total repair of her lesion, including rerouting of the anomalous pulmonary vein to the left atrium. The diagnosis was suspected from the chest x-ray and echocardiography, and confirmed by angiography. To the best of our knowledge only 2 additional cases have previously been reported.

Intrapulmonary ectopic liver associated with scimitar syndrome.

We report a case of intrapulmonary ectopic liver associated with Scimitar syndrome. A two month old male child who presented with features of congestive cardiac failure underwent extensive cardiac and radiological investigations and was diagnosed with Scimitar syndrome. He was also found to have a mass in the lower lobe of the right lung, the possibilities being either a pulmonary sequestration or a diaphragmatic hernia. As his dyspnea progressively worsened, a right lower lobectomy was performed at the age of two years. Intraoperatively, no connecting pedicle or hernia sac was seen. The resec…

Hypoplastic left heart associated with scimitar syndrome.

A child with the unique combination of hypoplastic left heart syndrome (HLHS) and scimitar syndrome is presented. Her HLHS was diagnosed in utero, and her scimitar syndrome was discovered during her immediate newborn period. She underwent a successful Norwood operation complicated by supraventricular tachycardia given her Wolf-Parkinson-White syndrome. She has also undergone successful Glenn shunt and at this writing is thriving.
