The scrotum is a superficial structure and clinical examination is frequently not enough for making a specific diagnosis. In acute scrotal pain US can confirm the presumptive clinical diagnosis and provide additional relevant information. In testicular torsion, color-Doppler imaging has a central role since it has become possible to identify it at early stage by showing absence of perfusion in the affected testis before any gray-scale abnormality. A 7-year-old boy who presented with a painful left hemiscrotal mass was diagnosed with acquired lymphangioma of the scrotum. Chronic friction from a cast for Perthes' disease might have been the cause of sudden enlargement of a congenital lymphangioma of the scrotum. Magnetic resonance imaging (MRI) was useful for preoperative diagnosis and determining the extent of the scrotal lesions. Total excision of the mass leaving the overlying skin was successfully performed. The clinical significance of MRI for preoperative diagnosis and planning surgical resection of this lesion is discussed.

Aggressive angiomyxoma is a rare benign tumor, arising in the pelvic soft tissues and perineum. It occurs almost exclusively in adult females. It is locally infiltrative and exhibiting a high risk of local recurrence. However, it is slow growing and does not metastatize. We report a case of aggressive angiomyxoma in an adult male which arose in the scrotum.

OBJECTIVE: To describe a case of scrotal cavernous hemangioma.
METHODS: A 4-year old boy with a painful left scrotal mass is described. The epidemiology, natural history, diagnosis, differential diagnosis, treatment and prognosis of this tumor are reviewed.
RESULTS: Analysis of the surgical specimen disclosed a scrotal cavernous hemangioma.
CONCLUSIONS: A scrotal mass diagnosed at an uncommon age or atypical site warrants considering hemangioma as a possible etiology.

The acute pediatric scrotum is a common clinical condition that can present a diagnostic dilemma for even the most experienced physician. The issue of primary importance in each case is to ensure testicular viability through proper evaluation, prompt diagnosis, and surgical intervention if necessary. A review of the various etiologies of acute scrotal disorders in children aids in the formulation of a logical and systematic approach to their accurate diagnosis and management.

The acute scrotum remains a difficult clinical challenge because of numerous etiologies including inflammatory processes, testicular torsions, traumas, and tumors that can justify early surgery. Ultrasound study appears to be the imaging modality of choice to confirm the clinical assessment. The natural history, management and prognosis of scrotal carcinoma are reviewed in 19 cases and a method to stage the tumor is presented. Ten patients were rendered free of evidence of cancer, 9 of whom have survived more than 5 years. Wide local excision of the primary lesion with radical ilioinguinal dissection for proved inguinal lymph node metastases is the present recommendation. For non-resectable regional metastatic disease no treatment of established effectiveness is available. Irradiation has produced benefit on rare occasions and hemipelvectomy may be considered in selected cases.
