Mortality with brainstem seizures from focal 4-aminopyridine-induced recurrent hippocampal seizures.

OBJECTIVE: Sudden unexplained death in epilepsy is the leading cause of death in young adult epilepsy patients, typically occurring during the early postictal period, presumably resulting from brainstem and cardiorespiratory dysfunction. We hypothesized that ictal discharges in the brainstem disrupt the cardiorespiratory network, causing mortality. To study this hypothesis, we chose an animal model comprising focal unilateral hippocampal injection of 4-aminopyridine (4-AP), which produced focal recurrent hippocampal seizures with secondary generalization in awake, behaving rats. METHODS: We s…

Do seizures beget seizures? An assessment of the clinical evidence in humans.

For more than a century, epilepsy was characterized as a chronic disease, with little chance of remission or cure. It was also considered a progressive disease in which seizures led to more seizures. Experimental work in animals provided additional support for the notion that seizures could beget seizures. However, the earliest clinical observations in humans were based on highly selected, largely refractory patients. Furthermore, the experimental work in animals bore little relation to naturally occurring seizures and epilepsy in humans. Evidence from multiple sources regarding the nature and…

Outcome of neonates with electrographically identified seizures, or at risk of seizures.

A prospective study was conducted to investigate survival at 1 month and survival and developmental outcome at 1 year in a cohort of 53 neonates either suspected of or at risk of having seizures. For all patients, presence of seizures, diagnoses, and structural abnormalities were identified. If seizures were present, seizure variables were quantified. Correlations between neonatal parameters and subsequent outcome were investigated. Forty-three patients survived the first month of life. Background EEG was the only significant predictor of survival at 1 month. Three patients died after 1 month,…

Functional seizures are associated with cerebrovascular disease and functional stroke is more common in patients with functional seizures than epileptic seizures.

PURPOSE: To characterize the relationship between functional seizures (FSe), cerebrovascular disease (CVD), and functional stroke. METHOD: A retrospective case-control study of 189 patients at a single large tertiary medical center. We performed a manual chart review of medical records of patients with FSe or epileptic seizures (ES), who also had ICD code evidence of CVD. The clinical characteristics of FSe, ES, CVD, and functional stroke were recorded. Logistic regression and Welch's t-tests were used to evaluate the differences between the FSe and ES groups. RESULTS: Cerebrovascular diseas…

Updates in Neonatal Seizures.

Neonatal seizures are a common medical emergency, necessitating prompt treatment. The most common etiologies include hypoxic-ischemic encephalopathy, ischemic stroke, and intracranial hemorrhage, with numerous other uncommon etiologies. Accurate diagnosis requires conventional video-electroencephalogram (cvEEG) to distinguish seizures from nonepileptic movements and to detect electrographic-only (subclinical) seizures. Treatment prioritizes rapid medication administration, with phenobarbital as first-line treatment and the only Federal Drug Administration (FDA)-approved medication for neonatal…

Towel induced reflex seizures.

Reflex epilepsies are a unique and heterogeneous group of epilepsies characterized by recurrent seizure activity evoked by a specific external sensory stimulus or internal cognitive process. Reflex seizures can be part of other epilepsy syndromes including focal and generalized syndromes and have a growing spectrum of presentations. We report a further subtype of reflex seizures associated with towel exposure. We give an account of a case of drug-refractory focal epilepsy admitted to the Epilepsy Monitoring Unit for presurgical assessment with 50% of seizures triggered in response to the touch…

Seizures in eating disorders.

OBJECTIVE: To complete a descriptive study of seizure etiology in a large population of eating disorder (ED) patients and to better understand whether malnutrition itself, in those with EDs, is associated with seizure development. METHOD: In this retrospective study, 75 patients with documented seizures met inclusion criteria from a total of 1664 charts. RESULTS: Prevalence of seizures in this ED cohort was found to be 4.5%, with 29.3% of individuals experiencing seizures due to psychogenic nonepileptic seizures (PNES). Other common causes of seizures included substance abuse/withdrawal (18.…

Predictors of hyperkinetic seizures.

OBJECTIVE: To identify predisposing factors for hyperkinetic seizure occurrence in a representative cohort of surgically treated patients with drug-resistant focal epilepsy. METHODS: We retrospectively recruited all seizure-free patients after epilepsy surgery with a postoperative follow-up ≥12 months. Patients were classified as presenting with hyperkinetic seizures if at least 2 episodes occurred during their disease history, based on clear-cut anamnestic description and/or video-EEG/stereo-EEG recordings. We performed univariable and multivariable logistic regression models to study the as…

[Psychogenic non-epileptic seizures].

The review provides epidemiological data and discuss the associated burden of non-epileptic seizures (PNES). Data on the prevalence, socio-demographic and clinical risk factors for the development of PNES are presented. The hypotheses of the PNES origin, including the contribution of psychological trauma, are considered. We also describe contemporary methods for differential diagnosis of epileptic seizures and PNES, including biomarkers and the use of diagnostic questionnaires. Special attention is given to the issues of the psychiatric comorbidity of PNES.

Neonatal Seizures: Core Concepts.

Seizures are the abnormal, excessive, synchronous discharge of cortical neurons that results in injury to the brain. Seizures presenting in the neonatal period may be the first and only clue to underlying neurological pathology. Despite advances in care, the mortality rate for infants experiencing neonatal seizures is still as high as 20 percent, with up to 65 percent of infants with seizures demonstrating significant morbidity. Early identification and treatment of the seizure or modifiable underlying etiology greatly reduces the extent of morbidity associated with neonatal seizures. Literatu…

Seizures in tuberculous meningitis.

OBJECTIVE: We report the frequency of seizure, its possible mechanisms and effect of seizure on the outcome of tuberculous meningitis (TBM). METHODS: Seventy-nine patients with TBM admitted during 2014-2017 were evaluated. The seizures were categorized as per International League Against Epilepsy as well as into early (within 1 month of meningitis) and late (>1 month) seizure. The possible association of seizures was recorded and the outcome was assessed using modified Rankin Scale (mRS ≤ 2 as good, and mRS > 2 as poor). RESULTS: The median age was 27 (18-76) years and 43 (54.4%) of whom wer…

Ranolazine overdose-induced seizures.

Ranolazine is a new anti-anginal medication that was approved by the US Food and Drug Administration (FDA) in 2006 for patients with symptomatic chronic angina despite optimized therapy. This paper presents a case report of a fifteen year old male patient admitted to the pediatric intensive care unit after ranolazine overdose ingestion. He had recurrent new onset seizures that are most likely due to ranolazine overdose. Seizures have never been reported with ranolazine use or abuse.

Seizures in Alzheimer's disease.

Alzheimer's disease (AD) increases the risk for late-onset seizures and neuronal network abnormalities. An elevated co-occurrence of AD and seizures has been established in the more prevalent sporadic form of AD. Recent evidence suggests that nonconvulsive network abnormalities, including seizures and other electroencephalographic abnormalities, may be more commonly found in patients than previously thought. Patients with familial AD are at an even greater risk for seizures, which have been found in patients with mutations in PSEN1, PSEN2, or APP, as well as with APP duplication. This review a…

Treatment of neonatal seizures.

Seizures occur more often during the neonatal period than at any other period of life. Precise incidence is difficult to delineate and depends on study population and criteria used for diagnosis of seizures. Controversy exists as to whether neonatal seizures themselves cause damage to the developing brain, or if the damage is primarily due to the underlying cause of the seizures. As a result of this controversy there is an ongoing discussion as to whether all seizures (both clinical and subclinical) should be treated. When (sub)clinical seizures are treated, there is no consensus about the mos…

[Brain imaging in seizures].

Cerebral imaging studies are performed systematically in children with non-idiopathic epilepsy. Magnetic resonance imaging (MRI) of the brain is the preferred technique. In emergency settings, when immediate neurosurgical treatment may be required (acute haemorrhage, etc.) non-enhanced computed tomography (NECT) is indicated, due to the shorter exam duration and the greater availability. Despite a normal NECT scan, the MRI scan is always indicated for non-idiopathic epileptic patients to search for small cortical sub-cortical abnormalities often overlooked in CT scans. On the other hand, a sec
