Surgical correction of cardiogenic shock.

Cardiogenic shock as a result of myocardial infarction demands an aggressive diagnostic and therapeutic approach to improve survival. First, the patient likely to develop shock should be identified early in his course so that interventions to reduce or contain infarct progression and thus prevent shock can be instituted. If the patient's condition deteriorates, prompt precise diagnosis of the cause of the hemodyanamic impairment must be made at the bedside with Swan-Ganz right heart catheterization and echocardiography, and, if potentially surgically correctable disease is present, left heart

Current perspectives in cardiogenic shock.

Cardiogenic shock (CS) remains a leading cause of death in intensive cardiac care. Outcomes are limited by delayed recognition of hypoperfusion, heterogeneous phenotypes, and late escalation of therapies. Diagnosis and risk stratification have progressed with the introduction of the SCAI staging system, which provides a common language for clinical severity and guides escalation of care. Echocardiography and invasive hemodynamics remain central for defining ventricular phenotype, detecting mechanical complications, and tailoring therapy. Early activation of multidisciplinary shock teams is inc

[Thyrotoxic crisis with cardiogenic shock].

Thyrotoxic crisis is a rare, life-threatening condition requiring rapid recognition and close multidisciplinary management. In this case report, we present a 59-year-old woman who developed cardiogenic shock and cardiac arrest despite intensive therapy. Emergency thyroidectomy was performed with extracorporal membrane oxygenation prepared as rescue support, but postoperative collapse persisted, and she died shortly after. The case underscores the critical importance of early identification and coordinated interdisciplinary care.

Kidney Failure in Cardiogenic Shock.

Acute kidney injury (AKI) is a frequent and prognostically decisive complication of cardiogenic shock (CS), conferring substantially increased short- and long-term mortality. As understanding of CS has evolved beyond an isolated low-output model, AKI is increasingly recognized as a consequence of multifactorial pathophysiology encompassing venous congestion, neurohormonal dysregulation, microcirculatory dysfunction, inflammation, and iatrogenic injury from shock therapies. Despite this expanded mechanistic framework, approaches to the diagnosis and management of AKI in CS remain highly variabl

Interleukin-6 in Cardiogenic Shock.

PURPOSE OF REVIEW: Cardiogenic shock (CS) is one of the leading causes of mortality in patients with acute cardiac disease, and systemic inflammation plays a critical role in its pathophysiology. This review explores the role of interleukin-6 (IL-6) in CS, with a focus on its biological pathways, prognostic value, and potential as a therapeutic target, highlighting the importance of addressing inflammation in this context. RECENT FINDINGS: Recent evidence highlights systemic inflammation implied in CS progression. Among the various cytokines involved, IL-6 is a major pro-inflammatory cytokine

Haematological challenges in cardiogenic shock.

PURPOSE OF REVIEW: Cardiogenic shock (CS) remains a very challenging condition with high mortality, despite the advances in therapeutic options. Haematological complications, including coagulopathy and haemolysis, frequently arise in these critically ill patients in CS, especially if they need percutaneous mechanical circulatory support (pMCS), and impair the outcome. This stresses the urgent need for further advancement of this field. RECENT FINDINGS: Here, we discuss the different haematological challenges during CS and additional pMCS. Furthermore, we propose a management strategy aiming f

[Cardiogenic shock complicating myocardial infarction].

Cardiogenic shock as a complication of myocardial infarction (5-10%) increases the mortality of uncomplicated myocardial infarction from less than 10% to 40%. This is due to the development of multiple organ dysfunction syndrome triggered by the extensive shock-induced impairment of organ perfusion. Therefore, guideline-based treatment should not only be restricted to reopening of the occluded coronary artery and management of complications of the infarction: important for survival are also guideline-driven optimization of organ perfusion by inotropic and vasoactive substances and, with well-d

Key Concepts Surrounding Cardiogenic Shock.

Cardiogenic shock (CS) is the final common pathway of impaired cardiovascular performance that results in ineffective forward cardiac output producing clinical and biochemical signs of organ hypoperfusion. CS represents the most common cause of shock in the cardiac intensive care unit (CICU) and accounts for a substantial proportion of CICU patient deaths. Despite significant advances in revascularization techniques, pharmacologic therapeutics and mechanical support devices, CS remains associated with a high mortality rate. Indeed, the prevalence of CS within the CICU appears to be increasing.

Hemodynamic monitoring in cardiogenic shock.

Cardiogenic shock (CS) is a life-threatening condition characterized by acute end-organ hypoperfusion due to inadequate cardiac output that can result in multiorgan failure, which may lead to death. The diminished cardiac output in CS leads to systemic hypoperfusion and maladaptive cycles of ischemia, inflammation, vasoconstriction, and volume overload. Obviously, the optimal management of CS needs to be readjusted in view of the predominant dysfunction, which may be guided by hemodynamic monitoring. Hemodynamic monitoring enables (1) characterization of the type of cardiac dysfunction and the

Mechanical ventilation in cardiogenic shock.

PURPOSE OF REVIEW: Mechanical ventilation is frequently needed in patients with cardiogenic shock. The aim of this review is to summarize and discuss the current evidence and the pathophysiological mechanism that a clinician should consider while setting the ventilator. RECENT FINDINGS: Little attention has been placed specifically to ventilatory strategies in patients with cardiogenic shock undertaking mechanical ventilation. Lung failure in patients with cardiogenic shock is associated with worsening outcome as well as a delay in mechanical ventilation institution. The hemodynamic profile a

Molecular signature of cardiogenic shock.

The incidence of cardiogenic shock (CS) has increased remarkably over the past decade and remains a challenging condition with mortality rates of ∼50%. Cardiogenic shock encompasses cardiac contractile dysfunction; however, it is also a multiorgan dysfunction syndrome, often complicated by a systemic inflammatory response with severe cellular and metabolic dysregulations. Here, we review the evidence on the biochemical manifestations of CS, elaborating on current gold standard biomarkers and novel candidates from molecular signatures of CS. Glucose and lactate, both identified over a century a

[Cardiogenic Shock Complicating Myocardial Infarction].

In patients admitted with acute myocardial infarction, cardiogenic shock remains the most common cause of death. Multidisciplinary care in a specialized center and guideline-compliant treatment of cardiogenic shock are crucial for the survival and prognosis of affected patients. Hemodynamic monitoring and stabilization by volume expansion, vasopressors and inotropes represent initial steps in the management of patients with cardiogenic shock. Nevertheless, early revascularization of the culprit-lesion is proved to be the most important treatment modality. Although the use of active mechanical

Vasopressor use in cardiogenic shock.

PURPOSE OF REVIEW: Data and interventional trials on vasopressor use during cardiogenic shock are scarce. Their use is limited by their side-effects and the lack of solid evidence regarding their effectiveness in improving outcomes. In the present article, we review the current use of vasopressor therapy during cardiogenic shock. RECENT FINDINGS: Two recent Cochrane analyses concluded that there was insufficient evidence to prove that any one vasopressor was superior to others in terms of mortality. A recent RCT and a meta-analysis on individual data suggested that norepinephrine may be prefe

Inotropic agents in cardiogenic shock.

PURPOSE OF REVIEW: Cardiogenic shock is a multifactorial and diverse entity in which inotropes are the cornerstone therapy. Although published clinical trials have focused on pharmacologic treatment of cardiogenic shock, there is lack of an established and widely accepted decision-making algorithm on the use of inotropic agents in cardiogenic shock. RECENT FINDINGS: The current review incorporates cardiogenic shock pathophysiology, inotropes and vasopressors pharmacodynamics. It emphasizes on each agent's indications, potential adverse effects, highlights special considerations and fsummarize
