This paper summarizes the anatomical basics of the shoulder, their variations, and precise definitions, including differential diagnoses. It … own, (1) the coracoacromial ligament and its osseous fixations form an osteofibrous arch above the shoulder joint, which plays a part in impingement syndrome; (2) the superior transverse scapular ligament or its ossified correlate arches the scapular incisure and can cause a typical compression syndrome of the suprascapular nerve; (3) the inferior transverse sc …

An inflammatory rheumatic shoulder can be assessed as a forgotten joint. Apparent problems and deformities of the hands and feet are prioritized in the perception of rheumatic patients. In contrast, however, involvement of the shoulder joint in the context of an inflammatory rheumatic disease is very high with up to 85% [2]. Loss of shoulder function and pain can be well compensated for a long time. This means that further diagnostics are only carried out when the joint is already destroyed and joint-preserving treatment options are obsolete. The various tools of evidence-based conservative tr…

BACKGROUND: Afferent innervation of shoulder joints plays a fundamental role in nociception and mechanoception and its alteration result in shoulder´s disease that course with pain and functional disability.
METHODS: Joints shoulder from healthy subjects (n = 20) and with chronic pain shoulder syndromes (n = 17) were analyzed using immunohistochemistry for S100 protein to identify nerve structures (nerve fibers and sensory corpuscles), coupled with a quantification of the sensory formations. Sensory nerve formations were quantified in 13 distinct areas in healthy joint shoulder and in the ava…

INTRODUCTION: Dementia is caused by various diseases, including Alzheimer's disease dementia (ADD) and dementia with Lewy bodies (DLB). We often encounter patients with dementia who have limited shoulder joint range of motion (ROM), especially those with behavioral and psychological symptoms of dementia (BPSD). But the relationship between the diseases of dementia and restricted shoulder joint ROM is currently unclear.
METHODS: We examined cognitive function and shoulder joint ROM in 234 new outpatients at 7 memory clinics in Japan. We assessed cognitive function using the Mini-Mental State E…

Based on results of anatomical studies, we established an arthroscopic procedure to resect all degenerative changes within the joint without endangering the stabilizing structures. Since November 1989 we operated on 26 patients with this technique. There were no major complications. The mean score significantly increased from 64.9 (+/- 12.8) to 86.8 (+/- 11.5). Analysis of postoperative x rays revealed an average length of resection of 21.3 mm at the inferior border of the clavicle, of 15.4 mm in the middle, and of 10.2 mm at the superior border of the clavicle. 21 out of 26 patients were comp

We report on a 59 year old patient with repetitive shoulder trauma due to numerous grand mal convulsions and consequent severe osteoarthritis of the shoulder joint with formation of multiple loose bodies. We discuss the aetiology of the loose bodies as being different to those found in synovial chondromatosis, as well as the radiological, histological, clinical and therapeutical features of this entity.

Six of 56 patients (10.7 per cent) surgically treated for anterior habitual and recurrent dislocation of the shoulder joint exhibited congenital habitual dislocation. Surgery revealed agenesis of the anterior and upper parts of the capsular wall. The lower part of the capsular wall was used for reconstruction.

1. Arthrography of the shoulder demonstrates the gross pathological changes in adhesive capsulitis of the shoulder. In true adhesive capsuli … r cubic centimeters of 1 per cent procaine appears to be the ideal solution to use for injection of shoulder joints. This amount gives arthrograms with excellent detail, and the procaine prevents any pain when the solution is injected into the joint.

This study reports the clinical results of treatment for injection-induced pyogenic arthritis of the shoulder joint. We followed up 13 patients who underwent operation for the treatment of pyogenic glenohumeral arthritis that developed after injections around the shoulder joint. Cultures identified Staphylococcus aureus in 6 patients and methicillin-resistant S aureus (MRSA) in 4. Finally, the University of California, Los Angeles Shoulder Score improved from 12.7 to 28.3 points. Four of the 9 patients with open surgery and 1 of 4 with arthroscopic surgery had good results; however, the other …

Purtscher described sudden blindness in patients with severe head trauma due to a remote retinopathy, characterised by bilateral retinal haemorrhages, cotton wool spots, and optic disc swelling seen on fundoscopy. A similar retinopathy has been reported in compressive chest trauma, long bone fractures, and acute pancreatitis. It is less well recognised that Purtscher's retinopathy can occur unilaterally and following less severe trauma. We present a case of unilateral remote traumatic retinal angiopathy following a fracture dislocation of the shoulder joint.

In a prospective study, we examined 34 patients with shoulder instabilities and 5 patients with unclear chronic shoulder pain (4 females, 35 males; 18-56 years of age, median 28 years) by CT arthrography and MRT arthrography from August 1994 through December 1995. No complications were seen when gadolinium-DPTA was applied intra-articularly. Twenty-three patients were followed up by operation and/or arthroscopy; 20 patients underwent a modified, open Bankart operation. In this paper, we present a new classification for damage of the anterior capsule and labrum. MRT arthrography showed better r

60 arthrograms of the shoulder were explored under morphologic and pathologic-anatomic aspects. Differentiation was made between acute and chronic pain. The possible findings in leaking of the contrast material out of the joint capsule are shown and discussed. Signs are quoted of degeneration of soft tissue and bone. The findings in patients with rheumatoid arthritis and a history of shoulder dislocation are summarised. Distension of the joint capsule without shoulder dislocation is described. Filling of the periarticular lymphatics is mentioned in different cases.

Radiological diagnostics (diagnostic imaging) of the traumatised shoulder and elbow joint has expanded considerably and has become increasingly meaningful in recent years. The introduction of computed tomography and magnetic resonance imaging has enabled a significantly better assessment of soft parts and of the inner spatial structure of the joints. Hence, focus of diagnosis has now shifted from the assessment of the bones (which was previously the main aspect of diagnosis) to a different analysis of the joint as a functional unit of bone and soft part structures, an analysis that can also de
