Natural History of Isolated Skull Fractures in Children.

Head injury is the most common cause of neurologic disability and mortality in children. We had hypothesized that in children with isolated skull fractures (SFs) and a normal neurological examination on presentation, the risk of neurosurgical intervention is very low. We retrospectively reviewed the medical records of all children aged six to sixteen years presenting to our Level 1 trauma center with traumatic brain injuries between January 1, 2006 and December 31, 2014. We also analyzed the National Trauma Data Bank (NTDB) research data set for the years 2012-2014 using the same metrics. Duri…

Management and outcome of pediatric skull base fractures.

OBJECTIVE: The management of skull base fractures in the pediatric age group continues to be a major challenge even for experienced multidisciplinary teams. This retrospective study was undertaken at a tertiary care academic hospital to evaluate the management and outcome of pediatric skull base fractures. METHODS: Retrospective analysis covering a period of 13.5 years (from 1996 to 2009) and 63 patients (mean age 10.7 years; range 1-18 years) was performed. RESULTS: A road traffic accident was the most frequent etiological factor (38%). The most common skull base fracture type was temporal

[Chronic anterior hypophyseal insufficiency following frontobasal skull fractures].

It is reported on a case of chronic insufficiency of the anterior lobe of the pituitary gland after frontobasal fracture of the skull. It is referred to the rareness of traumatically caused hypophyseal endocrine deficiency syndromes and the etiopathogenesis, the differential-diagnostic problem as well as the possibilities of a hormonal substitution therapy is discussed on the basis of the specialized literature.

A base of skull fracture presenting as haematemesis.

In a patient presenting with haematemesis, known to have oesophageal varices, the bleeding would be expected to be from those varices. This case demonstrates an unusual presentation of a base of skull fracture; blood from a fracture of the temporal bone had run from the middle ear through the eustachian tube to the pharynx. It demonstrates the need to remain vigilant even when the diagnosis appears to be obvious.

Phaeohyphomycosis complicating compound skull fracture.

Intracranial infection is a well-recognized complication of compound skull fractures. In most cases various bacteria are identified as the etiologic agents. Fungal infection complicating open head trauma is unusual. We describe a patient who contracted fatal meningoencephalitis due to Drechslera spicifera, a rarely pathogenic soil saprophyte, after open head trauma.

[The expanding skull fracture (author's transl)].

An expanding skull fracture with exceptional extension into the frontal sinus, the orbit and the ethmoidal cells is discussed. A 12 year old boy fell down the stairs at the age of 8 years and fractured his left frontal bone with extension to the base of the skull. He was seen because of a swelling of the scalp, an intermittent orbital oedema and convulsions. At neurosurgery a large gaping fracture of the frontal bone was found and this was interposed with fibrous tissue and relatively denatured cerebral tissue. The management both neurosurgical and rhinological is discussed.

[Skull fracture or accessory suture in a child?].

Differentiation between accessory sutures and fractures in the skull of an infant can be difficult. Apart from the regular sutures there is a multitude of variations that may be mistaken for a fracture line. Such variations include for instance the intraparietal suture between the two ossification centers of the parietal bone or the mendosal suture between the supraoccipital and interparietal bone of the occipital squama. The presented case refers to an approximately 20-month-old female child. During autopsy, a discontinuity in the right paramedian posterior cranial fossa parallel to the inter

Skull fracture mimicking eosinophilic granuloma.

BACKGROUND: Delayed swelling after skull fractures is an uncommon complication following head trauma in children. Classically, growing skull fractures typically present in patients under 3 years of age with progressive subcutaneous fluid collections, or occasionally with neurologic symptoms. We present the case of a healthy 2-year-old boy with a lytic "punched-out" frontal skull lesion. The child presented 2 months after a minor forehead injury for which no medical attention was sought. METHODS: The skull defect had no associated leptomeningeal cyst or brain herniation. Imaging and presentati…

[Skull fracture or bipartite parietal bone].

We describe the case of a male newborn born at 37 weeks of gestation. Complete radiological assessment because of suspected skull fracture revealed the diagnosis of a bipartite parietal bone. A brief review of the literature shows the exceptional character of this anatomical variant, whose origin is unclear at the present time.

Growing skull fracture: a posttraumatic neosuture.

CONTEXT: A case of a growing skull fracture presenting in adulthood is reported. Pertinent literature was reviewed with an emphasis on pathogenesis. EVIDENCE ACQUISITION: A search through MEDLINE from 1994 through 2004 was used and supplemented by searches of secondary sources retrieved from referenced articles. EVIDENCE SYNTHESIS: Current hypotheses for the pathogenesis of growing skull fractures were critically analyzed. A new hypothesis based in more recent data is proposed in an attempt to better understand this process. CONCLUSIONS: The proposed hypothesis seems to explain better the t…
