Polysomnography Differences Between Sleepy and Non-Sleepy Obstructive Sleep Apnea (OSA) Patients.

Background/Objectives: Factors underlying excessive daytime sleepiness (EDS) in obstructive sleep apnea (OSA) are not fully understood. We investigated whether polysomnography (PSG) parameters differed between non-sleepy and sleepy (based on the Epworth Sleepiness Scale (ESS)) OSA patients with the same disease severity, which may play a role in the presence of EDS. Methods: A total of 1307 patients, without cardiovascular, metabolic, respiratory, or inflammatory comorbidities, diagnosed with OSA (apnea-hypopnea index (AHI) ≥ 5 per hour of sleep) with type 1 PSG were included. Based on the AHI…

The Cleveland adolescent sleepiness questionnaire: a new measure to assess excessive daytime sleepiness in adolescents.

STUDY OBJECTIVE: Developed the Cleveland Adolescent Sleepiness Questionnaire (CASQ), a brief, self-completed instrument to measure excessive daytime sleepiness in adolescents. DESIGN: Cross-sectional analysis. PARTICIPANTS: A subsample of 411 adolescents 11-17 years of age recruited from area schools, churches, and "control" participants in a sleep disordered breathing cohort study; a second subsample of 62 adolescents with diagnosed sleep disordered breathing also participating in the sleep disordered breathing study. MEASUREMENTS: Participants completed the CASQ along with two other avail…

Disorders of excessive sleepiness. Treatment improves ability to stay awake but does not reduce sleepiness.

A total of 47 patients with sleep disorder (36 male and 11 female) with a mean age of 47.5 +/- 15 years were evaluated for daytime symptoms … improvement in ability to stay awake as measured by the MWT and no treatment-related improvement in sleepiness as measured by the MSLT. We conclude that there is a heterogeneous subpopulation of patients with sleep disorders whose symptoms of daytime sleepiness will show no treatment-related improvement in daytime symptoms if they are evaluated only by the M …

[Driving ability and daytime sleepiness].

Daytime sleepiness is a complaint of about 5-10% in a normal population. The consequences, such as impaired performance and accidents at the workplace and while driving, have major impact on the affected and on society. According to Swiss federal statistics only 1-3% of all motor vehicle accidents are due to excessive daytime sleepiness, which is in great contrast to a figure of 10 to 20% of all accidents derived from scientific studies. Due to the inadequate statistical representation of the problem, insufficient countermeasures have been realized, and the state of drivers breaching traffic r…

Sleepiness or excessive daytime somnolence.

Excessive daytime somnolence (EDS) is associated with age-related changes, environment, circadian rhythm or sleep pattern disorder, insomnia, medications, lifestyle factors, depression, pain, and illness. The notion of "sleep architecture" connotes a structure that describes the sleep cycle (i.e., stages) and wakefulness during a single sleep period-that is, rapid eye movement (REM) and non-REM sleep. Circadian rhythms perform a variety of functions including regulation of the quality and distribution of the stages of sleep. Insomnia includes delayed sleep onset as well as premature wakening;

Armodafinil for excessive daytime sleepiness.

Armodafinil is the (R)-enantiomer of the wakepromoting compound modafinil (racemic), with a considerably longer half-life of 10-15 hours. Armodafinil (developed by Cephalon, Frazer, PA, USA) was approved in June 2007 for the treatment of excessive sleepiness associated with narcolepsy, obstructive sleep apnea syndrome and shift work disorder, and the indications are the same as those for modafinil. Like modafinil, the mechanisms of action of armodafinil are not fully characterized and are under debate. Clinical trials in these sleep disorders demonstrated an enhanced efficacy for wake promotio…

Excessive daytime sleepiness in parkinsonism.

Excessive daytime sleepiness (EDS) can affect 20-50% of patients with Parkinson's disease (PD), whereas sleep attacks (SA), which are sleep episodes without prodroma, seem infrequent. EDS is associated with more advanced disease, higher doses of levodopa-equivalent, and sometimes the use of dopamine agonists. Patients at risk for SA have higher Epworth sleepiness scores (ESS) (although an important subset of patients under-score on this scale) and a more frequent use of ergot or non-ergot dopamine agonists. Polysomnography is a valuable tool in patients with PD, because sleep apnea may occur i…

Evaluation of excessive daytime sleepiness.

Excessive Daytime Sleepiness (EDS) is a difficult clinical problem, which is often indicative of a serious underlying physiologic condition. EDS is associated with automobile accidents, work-related injury, and increased use of the health care system, but it appears to be under-appreciated despite its common occurrence. There are many causes of EDS, ranging from insufficient sleep or inadequate sleep hygiene to drug effects and serious medical conditions. Assessment of EDS should begin with a detailed clinical history, and may also include self-rated instruments and ultimately physiologic test…

Daytime sleepiness and its evaluation.

Basic models of sleepiness, focusing on the homeostatic and circadian components of sleepiness, are able to predict important fluctuations of sleepiness. However, they fail in explaining certain sleepiness phenomena, as for instance in insomnia patients. To meet this shortcoming, modern models incorporate the arousal component of sleepiness, in addition to the sleep drive. While these models mainly concentrate on short-term changes in sleepiness, "state" sleepiness, there are indications that a stable characteristic level of sleepiness, "trait" sleepiness, is also an important determinant of a…

Sleep, sleepiness, and alcohol use.

The study of alcohol's effects on sleep dates back to the late 1930s. Since then, an extensive literature has described alcohol's effects on … Researchers have investigated the interactive effects of alcohol with other determinants of daytime sleepiness. Such studies indicate that alcohol interacts with sleep deprivation and sleep restriction to exacerbate daytime sleepiness and alcohol-induced performance impairments. Alcohol's effects on other physiological functions during sleep have yet to be d …
