BACKGROUND: Fissure in ano is a prevalent and painful condition, typically treated by lateral internal sphincterotomy after conservative measures fail. Dilip's transmucosal internal sphincterotomy introduces a simplified approach that reduces the risk of damaging the external sphincter, making it particularly suitable for less experienced surgeons. This innovation offers a less invasive method with minimal risk of incontinence, providing a safer, more accessible option.
IMPACT OF INNOVATION: Transmucosal internal sphincterotomy simplifies the internal sphincterotomy process, offering a tailor…

BACKGROUND: The use of precut sphincterotomy has been criticized as potentially unsafe. Despite this, a number of tertiary referral center have reported their successful use of this technique to increase the rate of common bile duct cannulation and therapeutic procedures.
AIM: To evaluate precut papillotomy and correlate the complication rate. METHODS-PATIENTS AND RESULTS: We performed precut sphincterotomy in 120 patients in whom attempts at standard common bile duct cannulation and sphincterotomy were unsuccessful. Bile duct diameters were correlated to the complication rate. Cannulation of…

BACKGROUND/AIMS: Although periampullary carcinoma can often be diagnosed at an early stage because of its strategic location, in a substantial number of cases (23% to 25% of periampullary carcinoma cases), the papilla can be prominent, but without an identifiable mass or ulceration. As a result, duodenoscopy alone can miss the tumor. In this series, the usefulness of sphincterotomy in establishing a diagnosis of periampullary carcinoma is documented.
MATERIALS AND METHODS: A total of 664 patients, who had clinical, biochemical and/or sonographic evidence of pancreaticobiliary disorders underw…

There is now considerable evidence linking the severity of gallstone-associated acute pancreatitis with obstruction to both the bile duct and the pancreatic duct by "persisting" stones either because of continuous or repeated impaction, or repeated passage. Two prospective trials have shown endoscopic sphincterotomy to be superior to other forms of treatment, but only in severe cases, and general experience now extends to over 1000 cases so treated. Endoscopic sphincterotomy is also very effective in acute pancreatitis associated with acute cholangitis. Prophylactic endoscopic sphincterotomy m…

The authors report on a case of gallstone ileus developed after endoscopic sphincterotomy (EST). The patient had to be operated and recovered uneventfully after enterotomy. Emphasis is placed on the importance of patient's observation after EST so this rare complication could be revealed in time.

Twenty five patients were reviewed a mean of 36 months after successful endoscopic sphincterotomy for the removal of bile duct stones. All the patients had improved symptomatically but 20% had episodes of mild abdominal pain and a similar number had elevated serum gamma glutamyltranspeptidase activities (up to 3 times normal). In 12 patients (50%) biliary gas was demonstrated indicating reflux of duodenal contents. Clinical cholangitis did not occur. Aspiration liver biopsy revealed mild portal tract fibrosis and inflammation in patients with biliary reflux. Biliary reflux was significantly as…

Endoscopic sphincterotomy (ES) is a simple, effective and remarkably safe method of treatment of choledocholithiasis and papillary stenosis. In this series, ES was technically successful in 255 (86.1 per cent) of 296 patients in whom it was attempted. Forty-nine of the successful cases had papillary stenosis complicated by pain, cholangitis and/or jaundice in patients after cholecystectomy; 205 had choledocholithiasis, of whom 164 had a cholecystectomy and 41 were poor surgical risks and did not have cholecystectomy. One patient had a tumour of the ampulla of Vater. ES has been performed as an…

An attack of gallstone ileus observed in a 60-year-old female patient is reported. In this patient who previously had been cholecystectomized, instant extraction of giant residual gallstones was unsuccessful despite a large endoscopic sphincterotomy. Three days later, she developed colicky abdominal pain and vomiting. At laparotomy nine days after the endoscopic procedure an impacted gallstone measuring 3.5 cm in diameter was removed from the the jejunum, some 50 cm below the ligament of Treitz. This observation demonstrates an unusual complication of endoscopic sphincterotomy and clearly outl…

An ongoing series of patients who were treated with endoscopic papillotomy (EPT) for retained or recurrent common bile duct gallstones or papillary stenosis was analyzed for safety and efficacy of the procedure and compared with accepted surgical technics, sphincterotomy and sphincteroplasty. To date, endoscopic papillotomy was attempted in 77 patients and successful in 71 (92%). The mean hospital stay is 3.1 days (range one to 12 days), convalescence has been short and morbidity has been low. One death has occurred in the series and no patients have been subjected to surgical intervention. Ev…

Duodenoscopic sphincterotomy was attempted in 265 patients. The procedure was successful in 243 patients (92%). Indications for sphincterotomy were: 185 patients with choledocholithiasis, 52 patients with papillary stenosis, and 6 patients with ampullary carcinoma. The clinical and biochemical evidence of cholestasis resolved in 222 of the 243 successful patients (91%). Complications consisting of hemorrhage, perforation, pancreatitis, cholangitis, and instrumental injury resulted in three deaths, an over-all mortality of 1.2%. Emergency laparotomy was required in 6 cases (2.5%). Duodenoscopic…
