The myelograms of 40 patients who had been operated on because of stenosis of the lumbar vertebral canal, were retrospectively assessed and measured. As was to be expected, the diameters were reduced, especially the sagittal diameter. Three configurations of the epidural space were seen representing different spatial capacities. The type occurring particularly often in lumbar osteogenic stenosis, which is conditioned by the patient's constitution, must be considered as a predisposing factor for the manifestation of a lumbar compression syndrome on account of its small capacity.

A rare case of primitive melanotic schwannoma of the spinal canal associated with an angiomatous formation pushing against the spinal cord is reported. A study by immunohistochemistry and electron microscopy was performed. The positivity for S-100 protein, demonstrated by immunoperoxidase, could indicate that the tumor originates from the Schwann cell. Analogous to its other fields of application, CT of the spine and the spinal canal has also already achieved a permanent place among the diagnostic methods. Numerically, CT studies of the cervical, thoracic and lumbar spine for our patient population with a total of 460 examinations are in the ratio 2: 1: 4.5. The essential advantages are improved differential diagnosis for many processes, the exact breakdown of the degree of extraspinal and intraspinal involvement in the change, the increasing limitation of myelography and last but not least the possibility of reducing cost.

The cases of four patients with intermittent claudication due to compression of the cauda equina are reported. The cause of this syndrome is considered to be a compression of the cauda equina by a thickening of the ligamentum flavum. The clinical, neuro-radiological and surgical characteristics are discussed.

Air in the spinal canal after trauma is uncommon. We report a case of a motor vehicle accident resulting in a thoracic fracture-dislocation. Air was discovered within the spinal canal on a computed tomography (CT) scan. This injury pattern proved to be a diagnostic dilemma because multiple etiologies existed. Pneumothorax, ruptured esophagus, facial sinus fractures, and herniated vertebral discs can result in this entity.

Computed tomography (CT) examinations and functional scores were evaluated in 28 patients with thoracolumbar fractures with intraspinal fragments, of whom 20 underwent operation. The elucidation of possible involvement of the vertebral canal in familial spastic paraplegia is attempted. The sagittal and the transverse diameters of the cervical, thoracic and lumbar vertebral canal were estimated in 14 patients and 100 controls. The results showed that the vertebral canal of the patients is considerably smaller in its whole length and not only in the thoracic region. This finding is discussed in relation to the development of both the vertebral canal and spinal cord.
