Pediatric Spine Disorders.

Pediatric spine disorders are numerous and are quite different when compared with the adult population. This article focuses on some of the more common pediatric spine disorders. This article summarizes such disorders and discusses typical treatment options in the pediatric orthopedic armamentarium.

Cervical spine injuries.

This article describes the anatomy of the cervical spine and the most common types of fractures associated with the cervical spine. Cervical spinal cord syndromes are also reviewed because such syndromes discovered during neurologic examinations frequently provide the first clue that there is an underlying spinal cord injury. Because most associated maxillofacial and spinal injuries occur in the setting of motor vehicle accidents, it is particularly important for the maxillofacial surgeon to be cognizant of the injuries, particularly in the context of the need for facial/cranial surgery. Appro…

Thoracic spine fractures.

Thoracic spine fractures have unique characteristics, and should be managed with specific criteria that are different from those used for thoracolumbar injuries. Fracture-dislocation injuries require high-energy injury, and should always be suspected in polytrauma patients with rib cage, sternum, cardiac, or pulmonary injuries. Although treatment is individualized, multisegmental posterior fixation sometimes combined with anterior decompression, is most commonly used. The authors review the current literature on this topic, and present their opinion on the management of such injuries.

Infected Charcot spine.

STUDY DESIGN: Case report of an infected Charcot spine following spinal cord injury. OBJECTIVE: To describe this very rare pathological condition and the results of surgical treatment. SETTING: A department of orthopaedic surgery in Japan. METHODS: A 44-year-old man presented with a destructive lesion in the lumbo-sacral spine and a fistula in his back. Anterior bone graft, percutaneous external spinal fixation, and suction/irrigation of the wound were performed. After 4 months, posterior spinal instrumentation surgery was carried out. RESULTS: Primary closure of the fistula and complete b…

Cervical spine disorders.

Recent research of disorders of the neck has concentrated on the efficacy of manual and other conservative therapies for neck pain and whiplash. Systematic reviews paint a poor picture of the quality of literature upon which many conventional, conservative therapies are based. Conclusive scientific data are lacking. Despite its unsavory reputation, whiplash has attracted considerable scientific inquiry. Multiple studies have brought data to bear that indicate that chronic neck pain after whiplash is not psychogenic, and that psychologic distress is secondary to the pain. Strong studies have sh

Cervical spine trauma.

STUDY DESIGN: The authors reviewed the results of a large number of studies of patients with acute cervical spine dislocations and subluxations. OBJECTIVES: The authors make recommendations regarding the evaluation and treatment of acute cervical spine dislocations based on a thorough review of the available literature. SUMMARY OF BACKGROUND DATA: Realization that a significant percentage of patients with acute cervical dislocations also have disc herniations has led to some controversy regarding the timing of MRI evaluation and attempted closed reduction. This article review studies of spin…

Cervical spine trauma.

The conventional radiographic examination remains as the single best initial study in patients with potential cervical spine injury. Computed tomography is the best, most readily available second study. Specific cervical spine injuries are presented as they follow a classification based on mechanism of injury.

Traumatic Charcot spine.

A case report of a trauma-induced neurologic injury of the spine that resulted in spinal arthropathy, without prior laminectomy, is presented. A review of Charcot joints of the spine is discussed, as well as the three other reported cases currently in the literature, only one of which was managed surgically. Charcot arthropathy can occur at sites distal to the quadra/paraplegic level in trauma patients. Careful radiographic analysis of these areas is important. Aggressive and modern surgical techniques are recommended to achieve spinal stabilization.

[Rigid spine syndrome].

Data of the clinical and laboratory examinations of 6 children (4 girls and 2 boys) with the syndrome of rigid vertebral column are presented. In half of them the syndrome appeared to be of family character. An analysis of the family history data pointed to the recessive type of the syndrome inheritance. The disease manifested itself in an impairment of the axial muscles (because of which the flexion of the neck and the trunk got sharply limited), a slight myopathic syndrome, scoliosis and cardiomyopathy. Histochemical examinations of biopsy specimens taken from the affected muscles showed pre

[Pathological spine fractures].

CLINICAL ISSUE: Pathological vertebral body fractures are nontraumatic fractures caused by metabolic diseases, tumor infiltration, inflammatory processes or other pathological changes. They often represent the first manifestation of an undiagnosed underlying condition. The clinical presentation is variable, most commonly involving acute back pain and, in some cases, neurological deficits up to paraplegia. Tumor-related fractures are often progressive and resistant to conservative treatment. STANDARD RADIOLOGICAL METHODS: Diagnostic evaluation is stepwise, beginning with X‑ray (basic assessmen

Infected charcot spine.

Patients with an infected charcot spine (ICS) may experience little or no back pain despite severe vertebral destruction. Understanding the pathophysiology underlying ICS and its differential diagnoses is crucial for its accurate diagnosis. Worsening symptoms of chronic charcot spine should raise suspicions of an infection.

Cervical spine trauma.

Cervical spine trauma encompasses a wide of injuries, ranging from stable, minor lesions to unstable, complex lesions that can lead to neurologic sequelae or vascular involvement. The Canadian C-Spine Rule and the NEXUS criteria aim to identify individuals with a low risk of cervical spine trauma who can safely forgo imaging tests. In high-risk patients, an imaging test is indicated. In adult patients the imaging test of choice is multidetector computed tomography. Complementary imaging tests such as CT angiography of the supra-aortic vessels and/or magnetic resonance imaging are occasionally …

Cervical spine tuberculosis.

Tuberculosis of the cervical spine differs from other vertebral localizations by its extreme rarity, the clinical images are very diversified, the radiological measurements allow a good diagnostic orientation and specifically the MRI which allows a multi-planar study of the various lesions. Only bacteriological evidence can confirm the diagnosis. The treatment is based on a 12-month antituberculosis multidrug therapy and much debate upon the surgical indication. In our case, the patient presented with bilateral cervicobrachialgia with pain on examination at the mobilization of the cervical spi…

Pediatric Spine Emergencies.

Acute onset of neurological signs and symptoms referable to the spine including difficulty walking, bowel and bladder dysfunction, and paresthesias in the extremities may be manifestations of pediatric spine emergency. Trauma is the most common etiology for a pediatric spinal emergency. Because a history of trauma is often available, the role of imaging involves detection of spinal injuries. Other pathologies such as infection, inflammation, vascular, and neoplasms may lead to pediatric spinal emergencies that also require imaging for assistance in determining the etiology and appropriate mana…

Endoscopic Spine Surgery.

Surgical treatment of the degenerative disc disease has evolved from traditional open spine surgery to minimally invasive spine surgery including endoscopic spine surgery. Constant improvement in the imaging modality especially with introduction of the magnetic resonance imaging, it is possible to identify culprit degenerated disc segment and again with the discography it is possible to diagnose the pain generator and pathological degenerated disc very precisely and its treatment with minimally invasive approach. With improvements in the optics, high resolution camera, light source, high speed…
