Opioid withdrawal symptoms after neurolytic splanchnic nerve block in cancer patients.

PURPOSE: Few reports on opioid withdrawal (OW) due to opioid tapering in cancer patients have been published. The incidence of and risk factors for OW after neurolytic splanchnic nerve block (NSNB) are unknown. This study aimed to elucidate the incidence of and risk factors for OW among cancer patients who could have reduced opioid doses after NSNB. METHODS: This was a multicenter, retrospective, observational study. We reviewed the medical charts of patients who underwent NSNB for intractable cancer pain at four tertiary hospitals in Yokohama City from April 2005 to October 2020. We included

Thoracic osteophyte as a cause of symptomatic greater splanchnic nerve compression. Case report.

Thoracic osteophytes are a common feature of degenerative spine disease. However, it is rare that osteophytes overgrowth on the anterior surface of thoracic spine results in the compression of vital structures and causes symptoms. A 39-year old man with a two-year history of thoracic and upper abdominal pain was admitted to the neurosurgery ward after having been seen by several specialists. Thoracic spine MRI and CT scans were analyzed. Osteophytes were identified on the anterolateral right side at level T8-T9 and were indicated as a possible cause of the symptoms. After obtaining patient con

Relationship Between the Azygos Vein and the Greater Splanchnic Nerve: Application to Nerve Ablation Procedures for Improving Cardiac Function.

Although ablation of the greater splanchnic nerve is now being used to treat some patients with heart failure, the anatomical relationships between this nerve and the adjacent posterior intercostal veins used for the procedure are not well elucidated. Therefore, the present anatomical study was performed. The anatomy and relationships of the right GSN and the azygos vein were investigated in 50 adult anatomical donors. In the supine position, the anterior wall of the thorax was removed, and the relationship between the azygos vein and its tributaries and the overlying GSN was observed and docu

Ten-kHz Spinal Cord Stimulation vs Radiofrequency Ablation of Splanchnic Nerves: A Single-Site Retrospective Comparison of 12-Month Outcomes.

OBJECTIVES: A prospective study on 10-kHz spinal cord stimulation (SCS) for various causes of chronic abdominal pain (CAP) showed robust improvements in subjects' pain and function. Radiofrequency ablation of splanchnic nerves (snRFA) has been used in advanced pain management treatment algorithms for CAP. This analysis was designed to provide what we believe is the first comparison of the efficacy of these two therapies. Propensity-score matched analysis (PMA) was performed to compare pain relief and decrease in medication usage in snRFA and SCS for treating refractory CAP. MATERIALS AND METH…

Comparison of Outcomes in Splanchnic Nerve and Celiac Plexus Cryoneurolysis for the Treatment of Refractory Abdominal Pain.

BackgroundVisceral upper abdominal pain caused by malignant and non-malignant etiologies can be treated by neurolysis of the splanchnic nerves and the celiac plexus. The purpose of this report is to compare the effect of cryoneurolysis of the celiac plexus vs the splanchnic nerves in patients with intractable upper abdominal pain as well as report outcomes and adverse events (AEs) of these procedures.MethodsA retrospective study of 36 patients who underwent either CT-guided celiac plexus (n = 9) or CT-guided splanchnic nerve (n = 27) cryoneurolysis for treatment of intractable upper abdominal …

A case series of fluoroscopy-guided neurolytic splanchnic nerve block for chronic pancreatitis pain.

CONTEXT: Splanchnic nerve block (SPN) with local anesthetic and steroid is used to relieve the pain of chronic pancreatitis. However, it does not provide long-lasting relief. We hypothesize that the use of 70% alcohol will give adequate analgesia for more than months. AIMS: The primary objective was to find out the analgesic efficacy of the use of 70% alcohol with SPN. Secondary objectives included the incidence of side effects, analgesic consumption postintervention, quality of life (QOL) assessed via a self-reported quality of life scale, and repeat block during the 1-year follow-up period.

Intercostal Neuralgia Occurring as a Complication of Splanchnic Nerve Radiofrequency Ablation in a Patient with Chronic Pancreatitis.

UNLABELLED: Our intent is to report a case of intercostal neuralgia occuring as a complication of splanchnic radiofreqency ablation (RFA), due to a breach in the integrity of the insulating sheath of the RFA needle.A 48-year-old man presented to our pain clinic with upper abdominal pain due to chronic pancreatitis, recalcitrant to medical management. We decided to perform bilateral splanchnic nerve RFA in this patient. After confirmation of bilateral correct needle placement under fluoroscopic guidance and sensorimotor testing, RFA was performed on the right side uneventfully. However, during

Optimising palliative care: the role of early splanchnic nerve block in advanced hepatobiliary cancer.

Patients with inoperable hepatobiliary cancers often endure severe abdominal pain, which impacts their quality of life. This pain is usually a combination of both nociceptive and neuropathic pain, which necessitates a comprehensive multimodal approach for effective management. Splanchnic nerve plexus block (SNB) has shown promising results in this regard. This case series reviews the literature and clinical practices, exploring the potential benefits of early SNB in the palliative care of patients with hepatobiliary cancers. We describe three cases illustrating SNB's potential in achieving not

5-Hydroxytryptamine does not reduce sympathetic nerve activity or neuroeffector function in the splanchnic circulation.

Infusion of 5-hydroxytryptamine (5-HT) in conscious rats results in a sustained (up to 30 days) fall in blood pressure. This is accompanied by an increase in splanchnic blood flow. Because the splanchnic circulation is regulated by the sympathetic nervous system, we hypothesized that 5-HT would: 1) directly reduce sympathetic nerve activity in the splanchnic region; and/or 2) inhibit sympathetic neuroeffector function in splanchnic blood vessels. Moreover, removal of the sympathetic innervation of the splanchnic circulation (celiac ganglionectomy) would reduce 5-HT-induced hypotension. In anae

Muscarinic involvement in vascular and adrenal medullary responses to splanchnic nerve stimulation in conscious calves.

Stimulation of the peripheral end of the right splanchnic nerve (4 Hz for 10 min) in the presence of hexamethonium caused a small but significant rise in mean aortic blood pressure which was subsequently abolished by atropine. There were also small but significant increases in the outputs of catecholamines, [Met5]-enkephalins and corticotrophin releasing factor (CRF) from the right adrenal gland. The catecholamine response was roughly halved after atropine while the outputs of enkephalins and CRF were unaffected. It is concluded that splanchnic sympathetic postganglionic neurones supplying the

The greater splanchnic nerve preferentially regulates neutrophils over macrophages in a rat model of septic peritonitis.

The sympathetic splanchnic nerve is a major player in immunoregulation, but its specific roles during infection have yet to be elucidated. Here, we evaluated how bilateral ablation of the greater splanchnic nerve (SplancX) impacts bacterial burden and immune function in a rat model of E. coli-induced septic peritonitis. SplancX had a major effect on bacterial burden within 24 h, reducing it to 4 % in the peritoneum and to 8 % in the spleen of what was found in the sham-operated controls. Such a major effect was not explained by gross changes in the infiltration of these sites with innate immun

Splanchnic nerve modulation in heart failure: mechanistic overview, initial clinical experience, and safety considerations.

Volume recruitment from the splanchnic compartment is an important physiological response to stressors such as physical activity and blood loss. In the setting of heart failure (HF), excess fluid redistribution from this compartment leads to increased cardiac filling pressures with limitation in exercise capacity. Recent evidence suggests that blocking neural activity of the greater splanchnic nerve (GSN) could have significant benefits in some patients with HF by reducing cardiac filling pressures and improving exercise capacity. However, to date the long-term safety of splanchnic nerve modul
