Computed tomography (CT) was used to examine 17 patients with various pathologic conditions (trauma, neoplasm, infection, or arthropathy) involving the sternoclavicular joint or sternum. CT required less time to perform and caused less discomfort to the patient than did conventional linear tomography. CT also demonstrated abnormalities better or provided additional useful diagnostic information in the majority of patients. When plain radiographs are not conclusive, CT is the preferred radiologic modality for the evaluation of the sternoclavicular joint and sternum.

Evolutionary transitions are frequently associated with novel anatomical structures,1 but the origins of the structures themselves are often poorly known. We use developmental, genetic, and paleontological data to demonstrate that the therian sternum was assembled from pre-existing elements. Imaging of the perinatal mouse reveals two paired sternal elements, both composed primarily of cells with lateral plate mesoderm origin. Location, articulations, and development identify them as homologs of the interclavicle and the sternal bands of synapsid outgroups. The interclavicle, not previously rec…

The sternal cleft is a rare congenital abnormality that is a result of failure of sternal fusion, varying from complete to incomplete (bifid), and may be associated with ectopia cordis. The surgical correction of the bifid sternum and its respective anesthetic concerns are presented.

A 49-year-old man with metastatic melanoma and pathologic fracture of the sternum was deemed to be a poor candidate for general anaesthesia. He suffered severe pain and range of motion limitation that did not respond to narcotic therapy. Ultimately, the lesion was managed with computed tomography-guided cryoablation and subsequent cementoplasty, and saw good initial clinical results.

The sternum and sternoclavicular joints--critical structures of the anterior chest wall--may be affected by various anatomic anomalies and pathologic processes, some of which require treatment. Pectus excavatum and pectus carinatum are common congenital anomalies that are usually benign but may warrant surgical treatment if they cause compression of vital internal structures. By contrast, developmental variants such as the sternal foramen are asymptomatic and do not require further evaluation or treatment. Arthritides of the sternoclavicular joint (osteoarthritis, septic arthritis, and seroneg…

Fourteen years have passed since video-assisted thoracic thymectomy (VATS-T) began to be used to treat diseases of the thymus. Indications for VATS-T have been expanded by ensuring sufficient space for easy endoscope manipulation using the sternum-lifting method. A variety of VATS-T techniques are available, including the cervical approach, bilateral VATS, infrasternal approach, and others. Although these techniques have the common feature of not involving sternotomy, they differ from each other in terms of the extent of thymectomy. In Japan, bilateral VATS (involving neck incision combined wi…

From October 1996 to August 2001, 721 patients with vertebral fractures were admitted to our unit. Ten patients suffered from vertebral fractures and concomitant sternal fractures. The clinical notes and plain film radiographs of these patients were studied. The average age of the patients was 37 (20-69) years. Nine had been involved in road traffic accidents. Three patients had fractures of the cervical spine, six of the upper thoracic spine (T1-T6) and one had a lumbar spine fracture. The extra-thoracic fracture group included two patients with neurological compromise and two patients who we

The case is presented of a 59 year old Saudi Arabian woman with sternal and vertebral tuberculous osteomyelitis and a benign breast mass simulating metastatic disease. This case illustrates the diagnostic difficulties that may be encountered in the diagnosis of tuberculous osteomyelitis.

Apophysomyces elegans, a member of the family Mucoraceae, was found to infect the chest wall and sternum of an immunocompetent man following minor trauma. As in previous cases, amphotericin B therapy alone was inadequate. Extensive surgical debridement was required in order to eradicate the infection.

Various skeletal abnormalities, including chest deformity, have been reported with Turner's syndrome. We report radiologic findings of the sternum on lateral chest roentgenograms in 15 children and adolescents with Turner's syndrome, whose ages ranged between 2 weeks and 20 years. Eight had associated congenital heart disease (CHD). Twelve patients (80 percent) had various sternal abnormalities; 5 had short sternum, 3 had premature fusion of the manubrio-sternal junction, and 4 had premature fusion of the mesosternum. Five had decreased ratio of sternal body to manubrium. Three patients had tw…

A flail chest with a manubriosternal separation in a man with multiple injuries was treated with an external fixator applied to the sternum and the manubrium. Pain and ventilatory function were improved, permitting immediate postoperative extubation and prompt patient mobilization. The external fixator was removed after fracture union at 2 months. One year after injury, the patient's pulmonary function was normal. External fixation is an alternative to other methods of sternal fracture stabilization.

The posteroanterior and lateral chest radiographs of 72 consecutive patients with sternal depression, together with 72 age- and sex-matched controls, have been reviewed. We assessed size and position of the cardiac silhouette, mediastinal contour, radio-opacity in the right lower zone and rib configuration. The degree of depression was determined from the lateral radiograph. The most significant signs of sternal depression on the posteroanterior chest radiograph are a straight left heart border, displacement of the cardiac silhouette to the left and an indistinct right heart border. These thre
