Chlordiazepoxide-induced Stevens-Johnson syndrome.

The causes of Stevens-Johnson syndrome (SJS) can be categorized as iatrogenic, infectious or idiopathic. Drug-induced SJS is associated with various antibiotics, anticonvulsants, and other drugs. However, no previous reports have mentioned an association between chlordiazepoxide, a benzodiazepine sedative, and SJS. Here, we present a case of SJS induced by chlordiazepoxide overdose. This case reminds us that SJS may be an adverse effect of chlordiazepoxide. Further, overdosage with benzodiazepine sedatives should be added to the list of potential causes of SJS.

Cocaine-related Stevens-Johnson syndrome.

We report the case of a 26-year-old student with erosive skin changes on his oral and genital mucosa and skin as well as on his thighs twice after cocaine snorting. The second of these two episodes was documented histologically, and other underlying causes of disease were excluded. A diagnosis of recurring Stevens-Johnson syndrome was made. Treatment was initiated with bed rest, systemic prednisone at 1 mg/kg body weight and liquid nutrition. Mucosal changes resolved within 4 days. On follow-up the patient remained free of symptoms while abstaining from the particular cocaine street mix in que…

Vancomycin-induced Stevens-Johnson syndrome.

Stevens-Johnson syndrome is a rare immunologic reaction that may involve skin or various mucosal surfaces. The etiology may range from multiple pharmacologic agents to viral infections. Associated findings can range from minimal skin and mucosal involvement to extensive dermal exfoliation, nephritis, lymphadenopathy, hepatitis, and multiple serologic abnormalities. We report a 36 year-old caucasian male who developed a pruritic, raised maculopapular eruption on Day 17 of intravenous vancomycin for treatment of probable bacterial endocarditis. The vancomycin was discontinued. The patient had re…

Cephalexin-induced Stevens-Johnson syndrome.

OBJECTIVE: To report a case of cephalexin-induced Stevens-Johnson syndrome (SJS), a devastating adverse drug reaction that involves the entire skin surface and mucosal areas of the body. DATA SOURCES: MEDLINE search (key terms cephalosporins, Stevens-Johnson syndrome, erythema multiforme, and systemic lupus erythematosus) and references identified from bibliographies of pertinent articles. DATA SYNTHESIS: Clinical presentation and manifestations of SJS include the skin, eyes, gastrointestinal tract, and pulmonary system. Infectious complications are the leading cause of mortality. Early inte…

Stevens-Johnson syndrome after radiotherapy.

We have reported a case of classic Stevens-Johnson syndrome in association with treatment using a cobalt radiation therapy unit. Previous reports of such an association have been extremely rare. At the time of the reaction, the patient's only medications were methyldopa and trichlormethiazide. Other drugs in the thiazide class have been associated with such a reaction. We feel this is an unlikely association in our patient, however, because she had been on trichlormethiazide for a number of years. This report suggests the role of radiotherapy as a rare cause of Stevens-Johnson syndrome.

Verapamil-associated Stevens-Johnson syndrome.

Stevens-Johnson syndrome (SJS) is a complication infrequently associated with the use of calcium-channel blocking agents. A 59-year-old woman developed symptoms typical of SJS approximately ten days following the initiation of sustained-release verapamil for hypertension management. Symptoms consisted of a generalized red macular rash associated with swelling of the face, tongue, lips, and mouth as well as a fever of 40 degrees C. Resolution of symptoms began approximately four days after verapamil discontinuation. Rechallenge with verapamil was not performed. Although SJS appears to be a rare…
