The authors present data of the clinical X-ray and morphological diagnostics of malignant gastric ulcers revealed in 96 patients. The total number of patients with gastric ulcer operated upon was 642. The morphological findings may serve the only reliable criterium of malignancy. Results of the operative treatment of such a malignant ulcer of the stomach are dependent on the spread of malignant invasion in the gastric wall. Geographical and epidemiologic investigations in recent years have thrown light on the determining factors in chronic atrophic gastritis and the intestinal type of gastric carcinoma. It is now proven that the majority of early gastric cancers have the exact symptoms of the ulcer type. Diagnosis is by endoscopy and biopsy. Uncritical therapy of an ulcer-like symptomatology by H2-receptor blockers must be rejected.

Increased mortality occurred among sows in a pig-breeding unit during a few months. It was concluded that gastric torsion was one of the causes of death. This disease is becoming increasingly important in large intensive units, also in the Netherlands. Little is so far known of the origin of this syndrome. The feeding system and the quality of the food (fine-grained feed, contamination by toxic agents) could be a factor in the pathogenesis. Closer investigation of this economic problem would appear to be advisable.

Helicobacter pylori has been implicated as a possible etiologic factor in gastric cancer. Recent epidemiologic studies, in fact, have demonstrated that there is a significantly higher prevalence of H. pylori in patients with gastric cancer than in matched controls. Comparative vertebrate anatomy shows that acid secretion and wall stress processes typically occur in different compartments. In the human single-chamber stomach both processes are physiologically separated in time. If both processes take place in one hollow organ at the same time, focal ischemia will occur. Using the microsphere technique a submucous steal-phenomenon, which the effect is due to, has been reproduced experimentally. A network model of the human gastric vascular bed produces borderline infarcts at the preferred sites of human gastric ulcer.

Results of surgical treatment of 170 patients with early cancer of the stomach are analyzed. There were 113 patients with tumor invasion within the bounds of the mucous membrane (m -- 113), the others had invasion within the bounds of the submucous membrane (sm -- 57 patients).
