Surgical strategies in the treatment of complex obstructive sleep apnoea in children.

Complex OSA in children is a challenging problem that requires careful team management. Evaluation includes history, physical examination and appropriate radiological and endoscopic studies. Twelve-channel polysomnography, with or without a pH probe, is mandatory in children with OSA to determine the severity and to document the efficacy of treatment. A variety of individual problems and nuances arise and will continue to do so. Clinical experience and a high degree of suspicion in the craniofacial population is necessary to rule out the existence of OSA in a given patient. Newer treatment mod

Gardner's syndrome: a clinical and genetic study of a family.

OBJECTIVE: Gardner syndrome (GS) is an autosomal dominant genetic disorder with almost complete penetrance (80%) and variable expression. GS … ated with clinical-instrumental monitoring for a period of 5 years had no major disturbances of the stomatognathic system and no clinical signs of pathology of the gastrointestinal tract, eyes, or endocrine systems. CONCLUSIONS: The orofacial complex disorders are exclusively functional and esthetic, concerning primarily the stomatognathic system. We had no cases of m …

Myths surrounding vertical dimension of occlusion in restorative dentistry: A scoping review.

OBJECTIVE: This scoping review aims to clarify the concept of vertical dimension of occlusion (VDO) in prosthetic dentistry, addressing prevalent myths, and controversies regarding its clinical management and impact on the stomatognathic system. METHODS: This paper critically examines common beliefs about VDO alteration and its effects on temporomandibular disorders (TMD) through an extensive literature review and an international survey. The survey included 862 general dentists and prosthodontists and explored their approaches to altering VDO and their perceptions of the clinical implication…

Temporomandibular disorders and bruxism. Part I.

Correct functioning of the entire stomatognathic system is achieved by a compatible relationship of all its parts. Four determinants, by their mutual harmonious activity, dictate the function of the entire system: the teeth, periodontium with supporting structure, muscles, temporomandibular joint (TMJ) and CNS. In such a complex system a disorder of any integrative part causes disturbances also in other parts of the system. Changed functions can arise through organic disorder, and also iatrogenically by inadequate conservative, prosthetic, surgical or orthodontic therapy. For this reason it is…

Standard Correction of Vision Worsens EMG Activity of Pericranial Muscles in Chronic TMD Subjects.

Recent studies showed an evident correlation between the stomatognathic system and the visual system. These results suggest that subjects who are affected by both temporomandibular (TMD) disorders and refractive disorders present with altered control of pericranial musculature tone and higher open-eye electromyographic (EMG) values. The objective of this work was to evaluate the effects of standard vision correction on EMG in subjects suffering from TMD compared with application of the same vision treatments to non-TMD subjects. 40 subjects were enrolled in this study. The test group included …

sEMG and Vibration System Monitoring for Differential Diagnosis in Temporomandibular Joint Disorders.

The stomatognathic system represents an important element of human physiology, constituting a part of the digestive, respiratory, and sensory systems. One of the signs of temporomandibular joint disorders (TMD) can be the formation of vibroacoustic and electromyographic (sEMG) phenomena. The aim of the study was to evaluate the effectiveness of temporomandibular joint rehabilitation in patients suffering from locking of the temporomandibular joint (TMJ) articular disc by analysis of vibrations, sEMG registration of masseter muscles, and hypertension of masticatory muscles. In this paper, a new…

Influence of dental occlusion on oculomotory function-A pilot study.

The aim of this study was to examine the interaction between the oculomotory system and the stomatognathic system based on morphofunctional relationships between the corresponding cranial nerves, their core areas and the reticular formation as a "link" by means of optometric examinations. Test subjects (N = 100) were prospectively recruited and divided into two groups: a young-age (N = 53, age 6-16 years) and old-age group (N = 47, 23-78 years). We determined the horizontal range of convergence and divergence as well as the near convergence point. These optometrically measured parameters were …

The neuromuscular approach towards interdisciplinary cooperation in medicine.

BACKGROUND: The contemporary approach of dentistry towards interdisciplinary cooperation is based on the neuromuscular concept. In recent years many authors have pointed out a correlation between orthopaedic and dental findings. Furthermore, there is an intimate biomechanical interrelationship of occlusion with cerebral fluid circulation, left and right equilibrium, gaze stabilisation and headache. The influence of a dental occlusion and temporomandibular joint (TMJ) status on general health has been widely analysed in the current scientific literature. AIM: The aim of this analytic study was

Four-dimensional analysis of stomatognathic function.

Many researchers have attempted to clarify the complex relationships between stomatognathic function and craniofacial morphology. Most studies investigated the trajectories of incisal or condylar points and measured temporomandibular morphology projected onto 2-dimensional radiographic films. Although these methods provided valuable information, their diagnostic capabilities were limited. We introduce a new 4-dimensional (4D) analysis of stomatognathic function that combines the 3-dimensional (3D) computed tomography of the cranium and mandible, dental surface imaging with a noncontact 3D lase

Infant and baby feeding and the development of the maxillofacial complex based on own observations and the literature.

The method and technique of feeding a young child affect the shape of the maxillofacial complex. Breastfeeding is the recommended method of feeding in the first six months of life. It is encouraged to continue natural feeding in later months, simultaneously developing other food extraction techniques. The correct formation of the stomatognathic system is a result of the correct organization of the motor patterns during feeding.

A suggested universal protocol for dental examination in sports.

The athletes of any sport and level submit their bodies to constant exercise. Any given pathology can increase the risk of injury, illness, or even reduced performance. The medical examination is valuable in diagnosing existing health problems and preventing medical issues that might compromise the athlete's overall health when exercising. The stomatognathic system is not an exemption, as oral pathologies, including dental caries and periodontal diseases, are found in high incidence in sports. The need for accurate and detailed dental examination in sports leaded dentists from the European Ass…

Does altering the occlusal vertical dimension produce temporomandibular disorders? A literature review.

The purpose of this review was to present a comprehensive review of the scientific evidence available in the literature regarding the effect … n on well-controlled clinical trials. The available evidence is weak and seems to indicate that the stomatognathic system has the ability to adapt rapidly to moderate changes in occlusal vertical dimension (OVD). Nevertheless, it should be taken into consideration that in some patients mild transient symptoms may occur, but they are most often self-limiting and without …

Dental abnormalities associated with campomelic syndrome: case report.

Campomelic syndrome is a rare autosomal recessive disease. It is characterized by short stature with angulation and bowing of the lower limbs, hypoplasia of the facial bones, and various other skeletal anomalies. The facies are unusually flat with micrognathia, frequent cleft palate, hypertelorism, and micro-ophthalmia. Most infants with this syndrome die from severe respiratory distress within the first hours or days of life. Those who survive infancy are mentally retarded. This case report concerns the dental treatment of a 13-year-old female with campomelic syndrome.
