Contingent self-stimulation for stuttering.

In this investigation, 10 adult stutterers were given time-out contingent on each stuttering. Ten other stutterers administered themselves time-out contingent on each stuttering. Stutterers in both time-out conditions evidenced marked reductions in stuttering compared with spontaneous speaking base rates. Stutterers who presented their own time-outs for stuttering, however, showed significantly less extinction of reduced stuttering following the time-out period, and significantly greater generalization to a telephone speaking situation, than did stutterers who were given time-out by an experim…

Recurrent involuntary imagery in people who stutter and people who do not stutter.

OBJECTIVE: To compare intrusive memories in groups of people who do (PWS), and who do not (PWNS), stutter. METHOD: Twenty-one participants who stuttered and 21 matched controls were given a semi-structured interview which explored imagery in speaking situations. The data were analyzed using a Content Analysis approach. Other outcome measures were the Beck Anxiety Inventory, the Beck Depression Inventory, the Post Traumatic Stress Disorder, PTSD, Symptom Scale: Self-Report Version. RESULTS: Significantly more stuttering participants than control participants indicated both recurrent imagery a…

Stuttering severity, psychosocial impact and lexical diversity as predictors of outcome for treatment of stuttering.

OBJECTIVE: This study assessed factors that predicted therapy outcome for children and adolescents who stuttered after attendance at an intensive therapy course. The factors examined were stuttering severity, lexical diversity measured by Type Token Ratio, and psychosocial impact of stuttering on the child's life. DESIGN: Fifty-four children who stuttered (CWS) participated in the study. The hypotheses were: (1) CWS with high initial stuttering severity would be more likely to persist than those with low initial severity; (2) lexical diversity before treatment should be related to therapy out…

Effects of stuttering severity and therapy involvement on role entrapment of people who stutter.

The primary purpose of this study was to examine whether a group of university students would report role entrapment of people who stutter (PWS) in the form of occupational stereotyping. The study also examined whether severity of stuttering (mild or severe) and level of therapy involvement (choosing or not choosing to attend therapy) affected the perceptions of role entrapment. To examine these issues, 260 students completed the Vocational Advice Scale (VAS) [Gabel, R. M., Blood, G. W., Tellis, G., & Althouse, M. T. (2004). Measuring role entrapment of people who stutter. Journal of Fluency D…

Connecting stuttering management and measurement: IV. Predictors of outcome for a behavioural treatment for stuttering.

BACKGROUND: Clinical trials have shown that behavioural treatments based on variants of prolonged-speech (PS) are best practice for reducing the stuttering rate in adults. However, while stuttering is significantly reduced or eliminated for most adults in the short-term with such treatment, relapse in the longer-term is common. Consequently, there has been interest in attempting to establish variables that predict responsiveness to PS-based treatments. Identifying such variables would enable risk prediction and also contribute to the tailoring of treatments to suit individuals who are less lik…

Dismantling the Lidcombe Program of early stuttering intervention: verbal contingencies for stuttering and clinical measurement.

BACKGROUND: Data have accumulated to show that the Lidcombe Program of early stuttering intervention is a safe treatment with positive outcomes for preschoolers who stutter, and a randomized controlled trial is under way at the time of writing. Program components have not been investigated experimentally so the functionality of each component is unknown. AIMS: The aim of this preliminary study was to establish the viability of a programme of research that would assess the clinical value of two program components. These components were parental verbal contingencies for stuttering and severity …

The Effects of Different Sources and Modalities of Stuttering Disclosure on Listeners' Perceptions of a Child Who Stutters.

INTRODUCTION: Research reveals the clinical efficacy of both verbal and written stuttering disclosure statements provided by a child who stutters (CWS) and his advocates (i.e., mother or teacher) [Lang Speech Hear Serv Sch. 2020 Jul;51(3):745-60 and Lang Speech Hear Serv Sch. 2021 Aug;52(4):1031-1048]. Although existing data reveal that both the source (i.e., self- vs. advocate disclosure) and modality (i.e., verbal or written) of stuttering disclosure yields significant improvements in the perceptions of speech skills and personality characteristics of CWS, there is a paucity of research dire…

Comparison of social anxiety between Japanese adults who stutter and non-stuttering controls.

PURPOSE: Adults who stutter (AWS) often develop social anxiety disorder. This study was to provide comparative data on the Liebowitz Social Anxiety Scale-Japanese version (LSAS-J) from AWS and non-stuttering adult controls. METHODS: LSAS-J, a 24-item self-reported survey of social phobia and avoidance across various daily situations, was administered to 130 AWS (Mean Age = 41.5 years, SD = 15.8, 111 males) and 114 non-stuttering adults (Mean Age = 39.5, SD = 14.9, 53 males). The test-retest reliability and internal consistency of the LSAS-J were assessed. A between-subject multivariate analys…

Adults who stutter lack the specialised pre-speech facilitation found in non-stutterers.

OBJECTIVES: Persistent developmental stuttering is a speech fluency disorder defined by its symptoms, where the underlying neurophysiological causes remain uncertain. This study examined the underlying neurophysiological mechanisms of the speech planning process, using facilitation in the motor cortex during speech preparation as an analogue. METHODS: transcranial magnetic stimulation (TMS) pulses induced motor evoked potentials (MEPs), which were recorded from the tongue. Eighteen adults who stutter (AWS) and 17 adults who do not stutter (ANS) completed three experiments, which involved read…

Stuttering: neuropsychiatric features measured by content analysis of speech and the effect of risperidone on stuttering severity.

Positron-emission tomographic (PET) studies and genetic research of stuttering have recently revealed underlying cerebral neurobiologic contributing factors in this disorder. We aimed to assess whether cognitive impairment and other neuropsychiatric dimensions could be detected through computerized content analysis of short samples of speech from stutterers, and whether administration of risperidone in a double-blind placebo-controlled study could decrease the severity of stuttering, as well as any of the neuropsychiatric features of these stutterers. A group of 21 stutterers with the developm…

Stuttering, language, and cognition: a review and a model of stuttering as suprasegmental sentence plan alignment (SPA).

Extant models of stuttering do not account for the emergence of stuttering at the onset of productive language use; the greater incidence of stuttering during spontaneous speech, on complex sentences, and at sentence-initial positions; the greater incidence of stuttering in bilinguals' 2nd language; the apparent deficiency of stutterers in expressive and receptive language skills; the prevalence of spontaneous recovery from stuttering; and the lack of chronic physiological or articulatory deficits in stuttering children's fluent speech. The author presents a model of stuttering as points of su…

The relationship of stuttering severity and treatment length to temporal measures of stutterers' perceptually fluent speech.

The purpose of this study was to investigate the relationship between stuttering severity and treatment length to temporal measures of stutterers' perceptually fluent speech. Seventeen adult stutterers (4 mild, 8 moderate, and 5 severe) were divided into two treatment categories. One group with limited exposure to treatment (n = 7) consisted of stutterers who had completed 0-10 hr of treatment. Another group with an extensive period of treatment (n = 10) consisted of stutterers who had completed 47-212 hr of treatment. All subjects were asked to repeat a simple phrase and read a prose passage.…

German Non-Stuttering Primary School Children's Knowledge and Attitudes Towards Stuttering: International Comparison and Influencing Factors.

BACKGROUND: Stuttering, as a visible and audible deviation from the norm, often provokes negative reactions and leads to social stigmatisation. Studies have shown that there is a negative stuttering stereotype, the origins of which can be traced back to early life. As environmental reactions have a significant impact on the success of therapeutic interventions, understanding public knowledge and attitudes towards stuttering is essential to mitigate these prejudices at an early stage. AIMS: The present study collects the first specific data on the knowledge of German non-stuttering primary sch…

Exploring the nature of stuttering through a behavioral-neuro-modulation intervention program in bilinguals with stuttering.

PURPOSE: Investigations on identifying the nature of stuttering present varying views. The argument remains whether the stuttering dysfluencies have a motor or a linguistic foundation. Though stuttering is considered a speech-motor disorder, linguistic factors are increasingly reported to play a role in stuttering. Current literature points towards deficits in speech-related motor areas of the brain to abnormalities in linguistic planning and phonological memory playing a role in stuttering. Examining cross-linguistic generalization of treatment gains from treated to untreated language in bili…

Drug-induced stuttering: A case study.

A 26-year-old female presented with an onset of severe stuttering four days after taking the medications Gabapentin and Lexapro. Gabapentin has been associated with acquired stuttering previously but, unlike previous cases, the stutter persisted for years after the discontinuation of the drug. Neurological and motor speech exams were normal aside from stuttering. The patient did not respond to fluency shaping strategies. Speech therapy focused on reducing secondary behaviors, affective, and cognitive aspects related to stuttering and was successful in improving cognitive, affective, quality of…

Streptococcal Serology in Children With Stuttering.

Introduction: Stuttering is a pronunciation disorder represented by repetitive perpetuations, duplications, or freezes of spoken words or syllables, as well as nervousness and cognitive shunning. Group A beta-hemolytic Streptococcus (GABHS) can lead to pediatric autoimmune neuropsychiatric disorders associated with streptococcal infection (PANDAS). Many case reports have proposed that stuttering is the result of a PANDAS, and that it can be identified together with Tourette syndrome, which shares many clinical characteristics with stuttering. Aim: The purpose of this study was to examine the a…

Variability of Stuttering: Behavior and Impact.

Purpose It has long been known that stuttering behaviors vary across time and situation. Preliminary evidence suggests that this variability negatively affects people who stutter and that stuttering behaviors are more variable than adverse impact associated with stuttering. More information is needed to determine how variability affects people who stutter and what the clinical and research implications of variability may be. Method Two hundred and four adults who stutter participated in a mixed-methods study exploring (a) how variability of stuttering affects people who stutter in comparison t…
