18F-FDG PET/CT Demonstrating Crossed Cerebellar Diaschisis Due to Germ Cell Tumor of the Basal Ganglia: A Case Report.

Germ cell tumors of the basal ganglia are rare neoplasms that usually present with hemiparesis or other neurological symptoms. MR and PET/CT with different tracers have demonstrated their usefulness for the correct assessment of disease. Crossed cerebellar diaschisis refers to a depression in metabolism of a cerebellar hemisphere as a result of contralateral supratentorial lesions. We present a case in which MR and PET/CT were crucial for the assessment of left basal ganglia germ cell tumors with the presence of crossed cerebellar diaschisis.

Glioblastoma multiforme in the posterior cranial fossa in a patient with neurofibromatosis type I.

Patients with Neurofibromatosis type 1 (NF1) have an increased risk of developing neoplasms. The most common brain tumors, found in 15%-20% of NF1 patients, are hypothalamic-optic gliomas, followed by brainstem and cerebellar pilocytic astrocytomas. These tumors generally have a benign nature. NF1 patients are predisposed to a 5-fold increased incidence of high-grade astrocytomas, which are usually located in supratentorial regions of the brain. We present an NF1 patient who developed a high-grade astrocytoma in the posterior fossa and discuss possible pathophysiological mechanisms.

Atypical and polymorphic angioblastic tumors of the central nervous system.

Five central nervous system (CNS) polymorphic angioblastic tumors have been studied. Four were located in the posterior fossa and one involved the temporal lobe. In reviewing the literature, the authors point out the rarity of the supratentorial locations and of malignant hemangioblastomas. The clinical and anatomopathological peculiarities are discussed, as is the uncertain classification of two cases. The discrepancy between histology and outcome is focused upon in the other three cases. Finally, the diagnostic, prognostic, and nosographic difficulties posed by CNS angioblastic neoplasms are

WHO Grade II and III supratentorial hemispheric ependymomas in adults: case series and review of treatment options.

Supratentorial ependymomas and their anaplastic variants are relatively uncommon central nervous system neoplasms that afflict both adults and children. Whereas the treatment algorithm in the pediatric population is well established, however, treatment in the adult population is less defined. In our case series of three adult patients with supratentorial ependymomas, two patients had tumors of WHO Grade III (anaplastic variant) and one had tumor of WHO Grade II. In all patients, gross total resection was achieved. Additional radiation therapy was administered in the Grade III patients. Twenty-

Computed tomography of primitive neuroectodermal tumors.

The primitive neuroectodermal tumor is a neoplasm of young individuals that occurs predominantly in the supratentorial compartment. It appears on computed tomography as a bulky mass, often with calcification with predominantly homogeneous enhancement but on occasion with cystic or necrotic areas. Cerebrospinal fluid seeding was observed in more than half the cases in our series. The patients had a short survival period (7-24 months). Despite surgery, radiation, and chemotherapy, further growth of this tumor has not been prevented.

Radiation-induced Supratentorial Osteosarcoma Following Curative Treatment of Infratentorial Ependymoma in a Child.

The development of secondary neoplasms following therapeutic cranial irradiation is rare and quite often lethal. Meningiomas, sarcomas, and high-grade gliomas are the most common tumors that manifest as a result of radiation therapy. We report the case of an 11-year-old child who presented with symptoms of supratentorial space-occupying lesion 7 years after curative surgery and cranial irradiation for a posterior fossa ependymoma. Magnetic resonance imaging of the brain revealed a right-sided temporoparietal dural-based contrast-enhancing lesion with evidence of overlying bone and skin involve

Malignant Ependymoblastoma Mimicking a Benign Pilocytic Astrocytoma.

Ependymoblastoma is an uncommon, exceedingly malignant brain neoplasm that adversely influences children's quality of life. Ependymoblastoma represents a subtype of primitive neuroectodermal tumors, categorized as grade IV, according to the 2007 World Health Organization (WHO) classification of central nervous system tumors. Ependymoblastomas are often located in the supratentorial zone and often associated with the ventricular system. Histopathological sections of the tumor revealed uniform, primitive, small blue cells, with multi-layered rosettes, accompanied by abundant mitoses. The clinica

A rare, giant, anaplastic oligodendroglioma.

Oligodendroglioma, the third most common glioma, accounts for 5% of primary brain tumors and around 20% of all glial neoplasms. They are quite uncommon in children. Here, we aimed to show an unusual case of a 9-year-old boy developing a huge anaplastic oligodendroglioma. A high-grade astrocytoma-like supratentorial tumor was discovered by a sophisticated brain scan employing magnetic resonance imaging. The tumor was identified by histopathology as an anaplastic oligodendroglioma. Anaplastic oligodendroglioma should be considered while making the differential diagnosis of high-grade astrocytoma

Supratentorial hemangioblastoma in a child. Case report.

Hemangioblastomas are rarely seen in the supratentorial region, especially in childhood. The case of a 10-year-old boy with left parieto-occipital cystic hemangioblastoma is reported. The neoplasm was not attached to the dura, nor was it associated with von Hippel-Lindau syndrome or with erythrocythemia. Cerebral angiography showed no evidence of a vascularized tumor, whereas computerized tomography gave more accurate information. The gross and microscopic appearance of the lesion is described, and a review of similar cases previously reported in the literature is presented.

Acute Massive Spontaneous Intratumoral Hemorrhage in a Pediatric Atypical Teratoid/Rhabdoid Tumor: A Case Report.

The majority of atypical teratoid/rhabdoid tumors (ATRTs) appear as heterogeneous masses with solid, cystic, necrotic components and calcification in the cerebral hemispheres; however, acute hemorrhagic onset is rare. The aim of this study is to describe a patient with an acute hemorrhagic supratentorial ATRT. A 4-year-old patient presented with right-sided weakness, nausea, vomiting, and loss of consciousness. Cranial magnetic resonance imaging revealed a large solid and cystic mass in the left posterior frontotemporal region with evidence of acute hemorrhage. The patient underwent urgent sur

Intraventricular supratentorial tumors in children.

Overall, intraventricular supratentorial tumors are rare in childhood. Classification can be based on the separation of lesions originating in intraventricular structures, such as choroid plexuses, from glial neoplasms of the ventricular wall which tend to infiltrate the ventricular cavities. Aim of the present study is to review the most common neoplasms of this region in childhood. Choroid plexus tumors (papillomas and carcinomas) and subependymal giant cell astrocytomas are dealt with, while for the other neoplasms which are rarer or more typic of other age ranges, specific reports should b

Epidemiological features of brain tumors in the first 3 years of life.

We investigated the age-related location, gender distribution, and histology of 75 brain tumors in children under 3 years of age seen in our department between 1984 and 1997. These were characterized by a higher overall incidence in boys (42/33 cases; ratio 1.3) and a prevalence for a supratentorial location (48/27 cases; ratio 1.7); the most common histological entities were astrocytoma (25.3%) and ependymoma (17.3%), followed by medulloblastoma (13.3%) and PNET (10.8%); 44% were high-grade tumors corresponding to WHO grades III and IV. In the 1st (22 cases), 2nd (25 cases) and 3rd (28 cases)

Cystic angiocentric glioma: a case report and literature review.

Angiocentric glioma (AG) is a rare form of brain tumor characterized by cortical epileptogenic lesions that show angiocentric patterns upon histopathological examination. Cystoid degeneration is a rare radiological manifestation of this condition. We report a case involving the left eloquent cortical areas and review the clinical features of angiocentric glioma. A 10-year-old girl presented with a history of refractory drug-resistant epilepsy. Imaging revealed a cystic-solid neoplasm within the left frontal lobe. Surgical procedures were performed, and it was observed that the pathological man

Secondary supratentorial primitive neuroectodermal tumor following irradiation in a patient with low-grade astrocytoma.

We report a case of a supratentorial primitive neuroectodermal tumor (PNET) that occurred 12 years after cranial irradiation for a grade II astrocytoma. Neuroimaging was unable to distinguish between a recurrence of the original neoplasm and the development of a new, distinct entity. Pathologic review assisted by immunohistochemical staining, however, revealed a high-grade PNET. Although rare, PNET needs to be included in the differential diagnoses for previously irradiated patients, who develop recurrent brain tumors in the presence of uncharacteristic imaging features.
