BACKGROUND: Sural nerve related symptoms following the extensile lateral approach to the calcaneus (ELA) and the sinus tarsi approach (STA) are a known postoperative complication despite awareness of the course the sural nerve. While the main trunk of the sural nerve and its location relative to the approaches have been previously described, the nerve gives rise to lateral calcaneal branches (LCBs) and an anastomotic branch (AB) that may be at risk of injury. The purpose of this study was to describe the course of the sural nerve, its LCBs and the AB in relation to the ELA and STA.
METHODS: 1…

BACKGROUND: There is no consensus regarding the most appropriate surgical approach for the treatment of posterior malleolar fractures. The posterolateral approach facilitates more accurate reduction, but the sural nerve is potentially at risk during the approach. The location of this nerve in relation to this approach has not been clearly described in the literature.
MATERIALS AND METHODS: We performed cadaveric dissection of 12 legs using the posterolateral approach, a 10-cm incision midway between the tendo-achilles and the lateral malleolus, commencing at the tip of the lateral malleolus a…

Sural nerve injury is a complication of Achilles Tendon (TA) rupture. We dissected 30 cadaveric lower limbs to describe the course of the sural nerve in relation to the TA. At the level of insertion of the TA into the calcaneum, the sural nerve was a mean 18.8 mm from the lateral border of the TA. The proximal course of the nerve was towards the midline such that it crossed the lateral border of the TA at a mean distance of 9.8 cm from the calcaneum. The significant individual variation in the position of the sural nerve in relation to the achilles tendon should be borne in mind when placing s…

Delineation of an area of sensory loss is often helpful in localizing a lesion at the spinal or root level. We have studied the segmental innervation of two cutaneous nerves, the saphenous and the sural, during selective posterior rhizotomy. Each nerve was stimulated electrically, and recordings were made from dorsal roots L-3 to S-2 in 30 patients. We found that both nerves received innervation through at least three spinal levels. The level of maximum innervation was approximately equally divided between L-3 and L-4 for the saphenous nerve. The sural nerve received maximum innervation throug…

In 167 consecutive patients with various types of neuropathy, the amplitude of the sensory potential and the maximum conduction velocity along the sural nerve were compared with conduction in other sensory nerves, and were related to structural changes revealed by nerve biopsy. Electrophysiological findings in the sural nerve were similar to those in the superficial peroneal and the median nerve, though the distal segment of the median nerve was normal in 20 per cent of the patients when it was abnormal in the sural nerve. Quantitation of histological findings was a more sensitive method than …

The lack of a clinically relevant animal models for research in facial nerve reconstruction is challenging. In this study, we investigated the surgical anatomy of the ovine sural nerve as a potential candidate for facial nerve reconstruction, and performed its histological quantitative analysis in comparison to the buccal branch (BB) of the facial nerve using cadaver and anesthetized sheep. The ovine sural nerve descended to the lower leg along the short saphenous vein. The length of the sural nerve was 14.3 ± 0.5 cm. The distance from the posterior edge of the lateral malleolus to the sural n…

The 10 mm rat sciatic nerve defect model is commonly used to investigate new strategies to improve functional recovery with segmental nerve … m. The results from the cadaveric study allowed us to create an ipsilateral, three-cable autologous sural nerve graft technique in the rat. This repair construct was evaluated with functional and histomorphometric analysis of nerve regeneration. The results support functional recovery of the sciatic nerve in all grafted animals. The use of an ipsilateral cabl …

AIMS: To investigate the efficacy of chitin biological absorbable catheters in a rat model of autologous nerve transplantation.
METHODS: A segment of sciatic nerve was removed to produce a sciatic nerve defect, and the sural nerve was cut from the ipsilateral leg and used as a graft to bridge the defect, with or without use of a chitin biological absorbable catheter surrounding the graft. The number and morphology of regenerating myelinated fibers, nerve conduction velocity, nerve function index, triceps surae muscle morphology, and sensory function were evaluated at 9 and 12 months after sur…

BACKGROUND: Neurosurgical interventions and trauma are common causes of damage to the optic nerve. This determines the relevance of research for solutions aimed at restoration of the nerve's anatomical integrity, electrical conductivity, and subsequently - restoration of its function. Restore a damaged (cut) optic nerve using n. suralis autograft in vivo.
METHODS: The experiment involved reconstruction of the optic nerve through injury modulation, graft placement and restored nerve harvest and evaluation. Injury modulation included removal of a fragment of the optic nerve. Autograft harvestin

OBJECTIVES: This study evaluates the results of an alternative technique developed to minimize the risk of complications associated with sural nerve biopsy for histopathological analysis.
METHODS: Twelve subjects underwent sural nerve biopsy and the defect created in the nerve was bridged by a 50-mm-length segment of the saphenous vein; the control group enrolled 23 patients in whom the entire length of the nerve was harvested to be used as autograft for reconstruction of nerves in the upper limb. Sensory reinnervation was quantified by use of the monofilament test and the static 2-point disc…

Recognition of anomalous innervation patterns may be important in dealing with peripheral nerve injuries. We describe a patient who suffered injury to the common peroneal nerve (CPN) at the knee when he laterally dislocated his knee. Intraoperative nerve conduction studies during surgical exploration revealed that nerve fibers from the tibial nerve crossed over to join the CPN immediately distal to a neuroma incontinuity. The sural nerve was also found to arise totally from the CPN.

This study is designed to evaluate twenty patients after sural nerve harvest using the single longitudinal incision method. The area of sensory loss to pin prick was assessed by the authors at different intervals. Donor site factors, scar cosmesis, functional status and complications were assessed using a questionnaire. The mean area of anaesthesia at one month was 65 cm2 and it had reduced by 77% to 15 cm2 at one and a half years. The mean area of reduced sensation was 49 cm2 at one month and it increased by 18% to 58 cm2 over the same period. Therefore, the total area of sensory deficit was …

BACKGROUND: This study presents a clinical series evaluating the effectiveness of nerve gap reconstruction using a vascularized turnover fascicular flap.
METHODS: A pilot study included 12 patients, aged 39 to 70 years (average: 54.6 y), with sural nerve gaps requiring reconstruction after injury. Sensory function was assessed using the British Medical Research Council Scale (BMRCS) and electroneuromyography preoperatively and at 12 months postoperatively. The level of neuropathic pain was assessed using a Pain Detect questionnaire. All patients underwent surgical exploration, neuroma excisio…

More than 50,000 patients were affected in Cuba during an epidemic outbreak of peripheral neuropathy from January 1992 until September 1993. The disease presented as either a retrobulbar optic neuropathy, a predominantly sensory peripheral neuropathy, a dorsolateral myeloneuropathy, or as mixed forms. The morphological findings in sural nerve biopsies from 34 patients with various forms of the disease are presented here. Frozen, paraffin and semi-thin sections were prepared for light and electron microscopy, immunohistochemistry and morphometric analysis. Every case presented morphological alt…
